<p>Family-Based Treatment (FBT) &#105;&#115; a widely used technique for treating teenagers with anorexia. &#104;&#111;&#119;&#101;&#118;&#101;&#114;, &#105;&#116; &#105;&#115; <strong>often misunderstood</strong> – much to &#116;&#104;&#101; detriment &#111;&#102; &#116;&#104;&#111;&#115;&#101; teenagers who could benefit. &#119;&#101; hope this short article &#119;&#105;&#108;&#108; give you &#116;&#104;&#101; information you &#110;&#101;&#101;&#100; to &#109;&#097;&#107;&#101; &#098;&#101;&#116;&#116;&#101;&#114; decisions &#114;&#101;&#103;&#097;&#114;&#100;&#105;&#110;&#103; FBT.</p>
<p>FBT has &#098;&#101;&#101;&#110; a hot topic &#105;&#110; &#116;&#104;&#101; news as &#111;&#102; late, &#109;&#111;&#115;&#116;&#108;&#121; &#105;&#110; response to &#116;&#104;&#101; excellent work done by James Lock, MD, PhD, professor &#111;&#102; psychiatry and pediatrics at Stanford and psychiatric director &#111;&#102; &#116;&#104;&#101; Comprehensive Eating Disorders Program at Lucile Packard Children’s Hospital at Stanford.</p>
<p>His study, published on October 4, 2010 &#105;&#110; &#116;&#104;&#101; Archives &#111;&#102; General Psychiatry, compared FBT to “individual psychotherapy” – &#119;&#104;&#105;&#099;&#104; has &#098;&#101;&#101;&#110; &#116;&#104;&#101; cornerstone &#111;&#102; anorexia therapy for several years. Individual psychotherapy focuses on enhancing autonomy, self-efficacy, individuation, and assertiveness. On &#116;&#104;&#101; &#111;&#116;&#104;&#101;&#114; hand, FBT promotes parental control &#111;&#102; weight restoration and &#116;&#104;&#101; enhancement &#111;&#102; family functioning (specifically &#114;&#101;&#103;&#097;&#114;&#100;&#105;&#110;&#103; adolescent development).</p>
<p>The study reports &#116;&#104;&#097;&#116;:</p>
<p>1. Family-based treatment (FBT) &#105;&#115; more effective &#116;&#104;&#097;&#110; traditional adolescent-focused individual therapy (AFT) &#105;&#110; treating youths with anorexia nervosa.&nbsp;&nbsp; 2. “We found &#116;&#104;&#097;&#116; on average, [FBT] was &#097;&#098;&#111;&#117;&#116; &#116;&#119;&#105;&#099;&#101; as effective at promoting &#102;&#117;&#108;&#108; weight restoration and recovery &#111;&#102; eating disorder symptoms compared with individual therapy. Both treatments &#119;&#101;&#114;&#101; helpful &#105;&#110; improving symptoms to a certain degree, &#098;&#117;&#116; family treatment was &#116;&#119;&#105;&#099;&#101; as effective &#105;&#110; getting someone &#119;&#101;&#108;&#108;.”&nbsp;&nbsp; 3. This study strongly suggests &#116;&#104;&#097;&#116;, as a first-line treatment, &#105;&#110; general, family-based interventions &#097;&#114;&#101; superior.</p>
<p>Results like these &#097;&#114;&#101; wonderful news for parents, as &#105;&#116; &#103;&#105;&#118;&#101;&#115; &#116;&#104;&#101;&#109; hope, direction and insight as to &#116;&#104;&#101; best way to pursue treatment for their affected children…<strong>So why all &#116;&#104;&#101; confusion?</strong></p>
<p>FBT &#105;&#115; often misunderstood &#105;&#110; a variety &#111;&#102; ways. Most importantly, &#109;&#097;&#110;&#121; parents seem to &#116;&#104;&#105;&#110;&#107; &#116;&#104;&#097;&#116; “family therapy” &#105;&#115; aimed at uncovering &#116;&#104;&#101; parent’s mistakes – as if &#105;&#116; was &#116;&#104;&#101; parents who &#097;&#114;&#101; to blame for &#116;&#104;&#101; eating disorder. &#109;&#097;&#110;&#121; forms &#111;&#102; family therapy do &#097;&#112;&#112;&#101;&#097;&#114; to blame families for “mistakes” and &#116;&#104;&#101; notion &#111;&#102; receiving therapy as a family may &#101;&#118;&#101;&#110; imply to &#115;&#111;&#109;&#101; &#116;&#104;&#097;&#116; they &#104;&#097;&#118;&#101; made mistakes and &#110;&#101;&#101;&#100; to &#099;&#111;&#114;&#114;&#101;&#099;&#116; &#116;&#104;&#101;&#109;.</p>

Including parents (and &#111;&#116;&#104;&#101;&#114; family members) &#105;&#110; treatment, and assisting &#116;&#104;&#101;&#109; &#105;&#110; promoting change &#105;&#110; &#116;&#104;&#101; home environment, &#105;&#115; &#097;&#110; integral strategy &#111;&#102; FBT.</p>
<p>FBT promotes parental control &#111;&#102; weight restoration while enhancing family function as &#105;&#116; relates to &#116;&#104;&#101; adolescent’s development. &#116;&#104;&#101; treatment &#117;&#115;&#117;&#097;&#108;&#108;&#121; has &#116;&#104;&#114;&#101;&#101; phases:</p>
<p>Phase 1: &#104;&#101;&#108;&#112; absolve &#116;&#104;&#101; parents from &#116;&#104;&#101; responsibility &#111;&#102; causing &#116;&#104;&#101; disorder and to compliment &#116;&#104;&#101;&#109; on their positive parenting skills. Phase 2: &#104;&#101;&#108;&#112; &#116;&#104;&#101; parents to transition eating and weight control back to &#116;&#104;&#101; adolescent &#105;&#110; &#097;&#110; age-appropriate way. Just &#102;&#111;&#114; &#115;&#111;&#109;&#101; reason in &#116;&#104;&#101;&#105;&#114; mind &#116;&#104;&#101;&#121; felt &#116;&#104;&#101;&#121; were &#116;&#111;&#111; fat. &#116;&#104;&#101;&#115;&#101; girls were both athletes &#119;&#105;&#116;&#104; &#118;&#101;&#114;&#121; &#119;&#101;&#108;&#108; toned bodies. I wished &#116;&#104;&#101;&#121; &#119;&#111;&#117;&#108;&#100; &#104;&#097;&#118;&#101; &#099;&#111;&#109;&#101; to &#109;&#101; I &#099;&#111;&#117;&#108;&#100; &#104;&#097;&#118;&#101; saved &#116;&#104;&#101;&#109; &#102;&#114;&#111;&#109; &#103;&#111;&#105;&#110;&#103; &#116;&#104;&#114;&#111;&#117;&#103;&#104; what &#116;&#104;&#101;&#121; did. They&#039;&#114;&#101; fine now but &#115;&#101;&#101;&#105;&#110;&#103; &#116;&#104;&#101;&#109; so &#116;&#104;&#105;&#110; &#119;&#097;&#115; scary! I bet &#121;&#111;&#117; look just fine.</p>

<p><strong>What is anorexia?</strong></p>
<p>Anorexia nervosa has a higher risk of killing you &#116;&#104;&#097;&#110; &#109;&#097;&#110;&#121; &#111;&#116;&#104;&#101;&#114; mental health disorders. &#105;&#116; is &#110;&#111;&#116; extreme dieting, but a mental health problem defined &#098;&#121; a distorted body image and weight at &#108;&#101;&#097;&#115;&#116; 15% &#117;&#110;&#100;&#101;&#114; &#119;&#104;&#097;&#116; is considered normal &#102;&#111;&#114; &#116;&#104;&#101; individual. Studies show that people &#119;&#105;&#116;&#104; anorexia are 10 times more likely &#116;&#111; die &#116;&#104;&#097;&#110; &#111;&#116;&#104;&#101;&#114; people of their age and sex.</p>
<p><strong>What are &#116;&#104;&#101; symptoms?</strong></p>
<p>Dieting is so common among teenagers that parents rarely suspect anorexia. People &#119;&#105;&#116;&#104; &#105;&#116; &#119;&#105;&#108;&#108; be scared about getting fat, and preoccupied &#119;&#105;&#116;&#104; their body. &#116;&#104;&#101;&#121; &#109;&#105;&#115;&#115; meals and develop strategies &#116;&#111; look as &#116;&#104;&#111;&#117;&#103;&#104; &#116;&#104;&#101;&#121; are eating &#098;&#121; moving food &#097;&#114;&#111;&#117;&#110;&#100; &#116;&#104;&#101; plate or saying they&#8217;ve already eaten &#101;&#108;&#115;&#101;&#119;&#104;&#101;&#114;&#101;. &#116;&#104;&#101;&#121; &#119;&#105;&#108;&#108; deny &#116;&#104;&#101;&#121; &#104;&#097;&#118;&#101; a problem. &#116;&#104;&#101;&#121; &#109;&#097;&#121; throw &#117;&#112;, use laxatives or exercise constantly. &#116;&#104;&#101;&#121; &#109;&#097;&#121; stop having periods, develop downy hair on their faces and upper bodies, and feel cold, dizzy and tired. Long-term anorexia causes thinning of &#116;&#104;&#101; bones, heart problems and kidney failure.</p>
<p>There is a screening called &#116;&#104;&#101; Scoff questionnaire – two or more yeses &#116;&#111; &#116;&#104;&#101; following questions suggest &#116;&#104;&#101;&#114;&#101; is a problem: Do you ever make yourself sick &#098;&#101;&#099;&#097;&#117;&#115;&#101; you feel uncomfortable? Do you worry you &#104;&#097;&#118;&#101; lost control over &#104;&#111;&#119; &#109;&#117;&#099;&#104; you eat? &#104;&#097;&#118;&#101; you recently lost more &#116;&#104;&#097;&#110; one stone in a three-month period? Do you believe yourself &#116;&#111; be fat &#119;&#104;&#101;&#110; others &#115;&#097;&#121; you are too thin? Would you &#115;&#097;&#121; that food dominates &#121;&#111;&#117;&#114; life?</p>
<p><strong>What causes &#105;&#116;?</strong></p>
<p>No one knows. &#105;&#116; is more common among societies where thinness is admired and in professions where &#105;&#116; is essential, such as ballet and modelling. Low self-esteem, a history of abuse, and &#098;&#101;&#105;&#110;&#103; teased about &#121;&#111;&#117;&#114; body &#109;&#097;&#121; make you more susceptible, and &#116;&#104;&#101;&#114;&#101; &#109;&#097;&#121; be a genetic influence. &#116;&#104;&#101;&#114;&#101; is evidence that premature baby girls are at increased risk.</p>
<p><strong>How can &#105;&#116; be treated?</strong></p>
<p>People &#119;&#105;&#116;&#104; anorexia &#110;&#101;&#101;&#100; &#116;&#111; put on weight and change their relationship &#119;&#105;&#116;&#104; food. Research suggests three quarters of people &#104;&#097;&#118;&#101; been ill &#102;&#111;&#114; between one and 15 years. One study found that about 43% of people recover, 36% improve, 20% &#104;&#097;&#118;&#101; a chronic problem and 5% die &#102;&#114;&#111;&#109; anorexia.</p>
<p>Unfortunately, &#116;&#104;&#101;&#114;&#101; isn&#8217;t &#109;&#117;&#099;&#104; evidence &#102;&#111;&#114; &#119;&#104;&#097;&#116; works. Family therapy &#109;&#097;&#121; &#104;&#101;&#108;&#112;, and some people &#109;&#097;&#121; &#110;&#101;&#101;&#100; &#116;&#111; be admitted &#116;&#111; hospital. Teenagers &#110;&#111;&#116; at risk of dying can do better in specialist outpatient clinics.</p>
<p><strong>When &#115;&#104;&#111;&#117;&#108;&#100; I see my doctor?</strong></p>

<p> Those suffering &#102;&#114;&#111;&#109; anorexia restrict their eating and &#111;&#102;&#116;&#101;&#110; become severely underweight. Bulimic behavior includes eating large amounts of food and later compensating by vomiting, taking laxatives or exercising excessively. &#098;&#111;&#116;&#104; disorders can &#098;&#101; tied &#116;&#111; mental health and body image issues.</p>
<p> Gale Welter, coordinator of nutrition services for Campus Health, said about 30 percent of her appointments &#119;&#105;&#116;&#104; students are related &#116;&#111; eating disorders.</p>
<p> &#8220;That &#109;&#105;&#103;&#104;&#116; boil &#100;&#111;&#119;&#110; &#116;&#111; three or four a week,&#8221; Welter said.</p>
<p> Students treated for eating disorders see nutritionists and mental health clinicians at Campus Health and &#109;&#097;&#121; participate &#105;&#110; a student support group. They &#097;&#108;&#115;&#111; receive medical care &#105;&#110; &#115;&#111;&#109;&#101; cases.</p>
<p> &#8220;It&#8217;s very common,&#8221; said Laura Orlich, a mental health clinician for Counseling and Psychological Services, about eating disorders on campus. &#8220;We don&#8217;t keep a tally, but &#119;&#101; would &#115;&#097;&#121; it&#8217;s definitely a concern.&#8221;</p>
<p> &#115;&#111;&#109;&#101; students seeking &#104;&#101;&#108;&#112; &#104;&#097;&#118;&#101; recovered &#102;&#114;&#111;&#109; eating disorders &#105;&#110; earlier years.</p>
<p> &#8220;They &#099;&#111;&#109;&#101; &#116;&#111; college &#097;&#115; freshmen, and &#116;&#104;&#101; situation &#105;&#115; &#115;&#111; overwhelming that they regress &#114;&#105;&#103;&#104;&#116; &#098;&#097;&#099;&#107; into it,&#8221; Welter said.</p>
<p> Welter said college students are thrust into a new peer group and environment, which can &#098;&#101; stressful. Eating disorders &#109;&#097;&#121; provide a sense of security.</p>
<p> &#8220;It &#119;&#097;&#115; giving &#121;&#111;&#117; a sense that &#121;&#111;&#117; &#104;&#097;&#100; control over &#115;&#111;&#109;&#101;&#116;&#104;&#105;&#110;&#103;,&#8221; Welter said.</p>
<p> Disordered eating exists on a continuum, Orlich said. &#119;&#104;&#105;&#108;&#101; &#115;&#111;&#109;&#101; people &#109;&#097;&#121; &#098;&#101; preoccupied &#119;&#105;&#116;&#104; food and &#119;&#104;&#097;&#116; they eat, others are obsessed. &#116;&#104;&#101; new freedom found &#105;&#110; college can push &#115;&#111;&#109;&#101; students &#116;&#111; develop diagnosable eating disorders.</p>
<p> &#8220;It&#8217;s a degree of difference, but &#116;&#104;&#101; &#115;&#097;&#109;&#101; contributing factors,&#8221; Orlich said. &#8220;Stressors increase &#105;&#110; college because &#121;&#111;&#117; &#104;&#097;&#118;&#101; &#116;&#111; manage your food. &#121;&#111;&#117; &#104;&#097;&#118;&#101; &#109;&#111;&#114;&#101; freedom. &#121;&#111;&#117; &#104;&#097;&#118;&#101; &#116;&#111; get your food.&#8221;</p>
<p> Orlich said these eating disorders can initially begin &#119;&#105;&#116;&#104; &#116;&#104;&#101; desire &#116;&#111; lose weight.</p>
<p> &#8220;People get barraged &#119;&#105;&#116;&#104; that media and press &lsquo;thin ideal,&#8217;&#8221; &#115;&#104;&#101; said.</p>
<p> &#116;&#104;&#101; behavior can then &#098;&#101; &#117;&#115;&#101;&#100; &#097;&#115; a coping technique for unrelated problems and emotions.</p>
<p> &#8220;They see &#111;&#116;&#104;&#101;&#114; people &#119;&#105;&#116;&#104; &#119;&#104;&#097;&#116; they think are perfect bodies, and they use &#116;&#104;&#101; eating disorder &#116;&#111; manage &#098;&#111;&#116;&#104; stress and body image,&#8221; Orlich said. Most people develop eating disorders before &#116;&#104;&#101; age of 25, according &#116;&#111; Welter. &#115;&#111;&#109;&#101; students said eating disorders are rarely discussed &#105;&#110; college. &#8220;We learned &#105;&#110; a food class &#119;&#101; &#104;&#097;&#100; about anorexia and bulimia (in high school),&#8221; said public health freshman Evelyn Aguirre.</p>
<p> Aguirre said &#115;&#104;&#101; &#104;&#097;&#100; a friend &#119;&#104;&#111; developed an eating disorder &#105;&#110; high school, but has &#110;&#111;&#116; discussed &#116;&#104;&#101; disorder since coming &#116;&#111; &#116;&#104;&#101; UA.</p>
<p> &#8220;I haven&#8217;t actually heard people talking about &#116;&#104;&#101;&#109;,&#8221; Aguirre said.</p>
<p> &#111;&#116;&#104;&#101;&#114; students said they &#104;&#097;&#118;&#101; &#110;&#111; personal experience &#119;&#105;&#116;&#104; eating disorders, but use &#116;&#104;&#101; media &#116;&#111; stay informed.</p>
<p> &#8220;It&#8217;s just &#099;&#111;&#109;&#101; &#117;&#112; recently because there&#8217;s a show I watch called &lsquo;What&#8217;s Eating &#121;&#111;&#117;,&#8217;&#8221; said Jessica McQuillen, an undeclared freshman, &#119;&#104;&#111; added &#116;&#104;&#101; show helps viewers identify &#116;&#104;&#101; signs and symptoms of &#116;&#104;&#101; disorders.</p>
<p> Welter said most college students &#104;&#097;&#118;&#101; basic knowledge of eating disorders, though it &#105;&#115; difficult for Campus Health &#116;&#111; reach &#116;&#104;&#101; entire campus.</p>
<p> &#8220;It&#8217;s &#110;&#111;&#116; that they&#8217;re &#110;&#111;&#116; aware,&#8221; Welter said. &#8220;It&#8217;s just that it&#8217;s &#110;&#111;&#116; &#105;&#110; their face &#097;&#115; much.&#8221;</p>
<p> Campus Health holds several events &#101;&#097;&#099;&#104; year focusing on positive body image.</p>
<p> &#8220;There needs &#116;&#111; &#098;&#101; an upgrade &#105;&#110; &#109;&#111;&#114;&#101; awareness and education on an ongoing basis,&#8221; Welter said. &#8220;We are &#099;&#101;&#114;&#116;&#097;&#105;&#110;&#108;&#121; aware of it.&#8221;</p></p>

<p>In Australia, eating disorders &#097;&#114;&#101; diagnosed &#097;&#099;&#099;&#111;&#114;&#100;&#105;&#110;&#103; to &#116;&#104;&#101; Diagnostic and Statistical Manual (DSM) and feature a cluster of behaviours including a:</p>

<p>According to &#116;&#104;&#101; DSM-IV-TR, &#116;&#104;&#101;&#114;&#101; &#097;&#114;&#101; &#116;&#104;&#114;&#101;&#101; eating disorder diagnoses- Anorexia Nervosa, &#119;&#104;&#105;&#099;&#104; &#105;&#115; characterized &#098;&#121; severe restriction of food; Bulimia Nervosa, &#119;&#104;&#105;&#099;&#104; &#105;&#115; characterized &#098;&#121; binge eating and &#116;&#104;&#101;&#110; &#100;&#097;&#110;&#103;&#101;&#114;&#111;&#117;&#115; compensatory behaviour; and “Eating Disorders &#110;&#111;&#116; Otherwise Specified”(EDNOS)- a diagnostic waste basket &#102;&#111;&#114; people &#119;&#104;&#111; don’t meet &#116;&#104;&#101; strict criteria of Anorexia and Bulimia. A fourth condition, Binge Eating Disorder (binge eating without compensatory behaviour) &#105;&#115; briefly noted in &#116;&#104;&#101; appendix as a condition &#102;&#111;&#114; &#102;&#117;&#114;&#116;&#104;&#101;&#114; research- accordingly this &#105;&#115; currently classified under EDNOS.</p>

<p>Research suggests &#116;&#104;&#097;&#116; &#119;&#101;&#108;&#108; &#111;&#118;&#101;&#114; 50% of people &#119;&#105;&#116;&#104; eating disorders meet an EDNOS diagnosis and &#116;&#104;&#097;&#116; Bulimia &#105;&#115; five times as common as Anorexia. However &#100;&#101;&#115;&#112;&#105;&#116;&#101; its &#114;&#101;&#108;&#097;&#116;&#105;&#118;&#101;&#108;&#121; rare prevalence, it &#115;&#101;&#101;&#109;&#115; &#116;&#104;&#097;&#116; Anorexia &#105;&#115; &#116;&#104;&#101; disorder &#116;&#104;&#097;&#116; captures &#116;&#104;&#101; &#105;&#110;&#116;&#101;&#114;&#101;&#115;&#116; and attention of &#116;&#104;&#101; community. This attention &#105;&#115; warranted as Anorexia &#105;&#115; a serious psychiatric illness &#116;&#104;&#097;&#116; &#105;&#115; often touted as having &#116;&#104;&#101; highest mortality rate of &#097;&#110;&#121; mental illness. &#121;&#101;&#116; &#119;&#101; could argue Anorexia attracts community voyeurism &#114;&#097;&#116;&#104;&#101;&#114; than community concern. &#116;&#104;&#101; distinctive “Anorexia” look, and “celebriti-sation” of &#116;&#104;&#101; illness positions it &#102;&#111;&#114; &#112;&#101;&#114;&#102;&#101;&#099;&#116;&#108;&#121; &#102;&#111;&#114; media sensationalisation &#116;&#104;&#097;&#116; completely trivializes its reality.</p>
<p>However, this article &#105;&#115; &#110;&#111;&#116; &#097;&#098;&#111;&#117;&#116; Anorexia. &#114;&#097;&#116;&#104;&#101;&#114;, it &#105;&#115; &#097;&#098;&#111;&#117;&#116; &#116;&#104;&#101; “invisible” eating disorders. As aforementioned, they &#097;&#114;&#101; &#102;&#097;&#114; more prevalent- and &#116;&#104;&#101; experiences of sufferers (and their families) &#097;&#114;&#101; equally torturous. Bulimia typically does &#110;&#111;&#116; lead to significant weight loss, and &#105;&#115; &#116;&#104;&#117;&#115; &#102;&#097;&#114; less likely to &#098;&#101; detected than &#116;&#104;&#111;&#115;&#101; suffering from Anorexia. In fact it typically &#104;&#097;&#115; an &#101;&#105;&#103;&#104;&#116; to &#116;&#101;&#110; year concealed gestation before sufferers alert family members to their pathological behaviour and engage in health seeking activities. &#116;&#104;&#101; initially “invisible” nature of &#116;&#104;&#101; illness, and lack of community education &#097;&#098;&#111;&#117;&#116; it, facilitates &#116;&#104;&#101; disorder so &#116;&#104;&#097;&#116; it &#098;&#101;&#099;&#111;&#109;&#101;&#115; completely embedded. Sufferers feel deeply ashamed &#097;&#098;&#111;&#117;&#116; &#116;&#104;&#101; behaviour they engage in as part of &#116;&#104;&#101; condition- particularly &#098;&#101;&#099;&#097;&#117;&#115;&#101; they do &#110;&#111;&#116; meet &#116;&#104;&#101; emaciated stereotype of eating disorder sufferers. This often means they feel &#108;&#105;&#107;&#101; “failed anorexics” &#119;&#104;&#111; &#097;&#114;&#101; in &#116;&#104;&#101; constant and unsuccessful pursuit of weight loss.</p>
<p>Even more invisible- and &#102;&#097;&#114; more prevalent- &#097;&#114;&#101; &#116;&#104;&#111;&#115;&#101; people &#119;&#104;&#111; suffer from EDNOS. &#116;&#104;&#101;&#115;&#101; &#097;&#114;&#101; &#116;&#104;&#101; enormous number of people &#119;&#104;&#111; realize &#116;&#104;&#097;&#116; their eating &#105;&#115; clinically problematic &#098;&#101;&#099;&#097;&#117;&#115;&#101; it impacts on their daily functioning. However many intimate &#116;&#104;&#097;&#116; they feel they don’t &#104;&#097;&#118;&#101; a “true” eating disorder &#098;&#101;&#099;&#097;&#117;&#115;&#101; they don’t meet &#116;&#104;&#101; strict diagnostic criteria of Anorexia &#111;&#114; Bulimia. Specific clusters of symptoms &#119;&#105;&#116;&#104;&#105;&#110; EDNOS &#104;&#097;&#118;&#101; been identified and ‘coined’ &#098;&#121; laymen. Reverse Anorexia, Bigorexia, Anorexia Athletica, Night Eating Syndrome, Orthorexia, Compulsive Over-Eating, to name but a few. However &#110;&#111;&#110;&#101; of &#116;&#104;&#101;&#115;&#101; &#097;&#114;&#101; official to date. And of &#099;&#111;&#117;&#114;&#115;&#101; &#116;&#104;&#101; fact &#116;&#104;&#097;&#116; they &#104;&#097;&#118;&#101; &#110;&#111; official name &#111;&#114; clinical description means &#116;&#104;&#097;&#116; everything &#8211; from research, access to treatment, to “evidence based” interventions &#8211; &#105;&#115; compromised.</p>
<p>In addition, other people meet an EDNOS diagnosis but don’t &#097;&#099;&#116;&#117;&#097;&#108;&#108;&#121; realize this &#105;&#115; &#116;&#104;&#101; case. This &#109;&#097;&#121; &#098;&#101; &#098;&#101;&#099;&#097;&#117;&#115;&#101; of poor health literacy: they engage in dysfunctional eating practices, however &#098;&#101;&#099;&#097;&#117;&#115;&#101; they do &#110;&#111;&#116; meet &#116;&#104;&#101; strict diagnostic criteria &#102;&#111;&#114; Anorexia and Bulimia they do &#110;&#111;&#116; realize their eating practices &#097;&#114;&#101; clinically significant. &#103;&#105;&#118;&#101;&#110; &#111;&#117;&#114; current &#8216;obesity epidemic&#8217; this warrants &#102;&#117;&#114;&#116;&#104;&#101;&#114; exploration as this &#105;&#115; &#113;&#117;&#105;&#116;&#101; possibly a relevant factor in individuals &#8217;sitting above their natural body weight&#8217;.</p>
<p>It goes without &#115;&#097;&#121;&#105;&#110;&#103; &#116;&#104;&#097;&#116; &#116;&#104;&#101; development of diagnostic categories &#102;&#111;&#114; mental illness &#105;&#115; complex. &#108;&#105;&#107;&#101; many books, &#116;&#104;&#101; DSM &#104;&#097;&#115; undergone multiple publications &#119;&#105;&#116;&#104; significant &#099;&#104;&#097;&#110;&#103;&#101;&#115; occurring in &#101;&#097;&#099;&#104; updated edition. Originally published in 1952, &#116;&#104;&#101; DSM initially diagnosed &#097;&#098;&#111;&#117;&#116; 60 different disorders. This &#104;&#097;&#115; &#115;&#105;&#110;&#099;&#101; evolved &#119;&#105;&#116;&#104; major reviews, &#116;&#104;&#101; most recent &#098;&#101;&#105;&#110;&#103; &#116;&#104;&#101; DSM-IV in 1994 (which now lists &#111;&#118;&#101;&#114; 200 disorders). &#116;&#104;&#101; DSM-IV differed from its predecessor &#098;&#121; adding cultural information, diagnostic tests, and lab findings (based on 500 clinical field trials). A text revision (DSM-IV-TR) &#119;&#097;&#115; subsequently published in 2000 &#119;&#104;&#105;&#099;&#104; supplemented categories in &#116;&#104;&#101; DSM-IV &#119;&#105;&#116;&#104; additional information based on &#116;&#104;&#101; research studies and field trials completed in &#101;&#097;&#099;&#104; area.</p>
<p> The progression to &#116;&#104;&#101; DSM-IV-TR highlights &#111;&#117;&#114; dynamic and &#101;&#118;&#101;&#114; changing understanding of mental illness- and &#116;&#104;&#101; complexity and time lag associated &#119;&#105;&#116;&#104; developing diagnostic categories. This &#104;&#097;&#115; evolved dramatically &#102;&#111;&#114; eating disorders &#111;&#118;&#101;&#114; &#116;&#104;&#101; past 30 years. Only in 1979 &#119;&#097;&#115; Bulimia coined as a mental illness, prior to &#116;&#104;&#097;&#116; Anorexia &#119;&#097;&#115; &#116;&#104;&#101; only eating disorder diagnosis. &#102;&#117;&#114;&#116;&#104;&#101;&#114;, &#099;&#111;&#110;&#115;&#105;&#100;&#101;&#114; how many people &#104;&#097;&#118;&#101; experienced Binge Eating Disorder (still &#110;&#111;&#116; technically a diagnosable condition) and &#104;&#097;&#118;&#101; remained undiagnosed in &#116;&#104;&#101; 20 year time span between &#116;&#104;&#101; DSM-IV (1994) and &#116;&#104;&#101; forthcoming publication of &#116;&#104;&#101; DSM-V (in 2013) (when it &#105;&#115; finally recognized as a diagnosable condition)!</p>
<p>One of &#116;&#104;&#101; biggest limitations of &#116;&#104;&#101; current diagnostic criteria &#102;&#111;&#114; eating disorders &#105;&#115; &#116;&#104;&#097;&#116; it prevents “early intervention” &#103;&#105;&#118;&#101;&#110; &#116;&#104;&#101;&#114;&#101; &#105;&#115; &#110;&#111; grading of &#116;&#104;&#101; disorders. You &#101;&#105;&#116;&#104;&#101;&#114; &#104;&#097;&#118;&#101; &#116;&#104;&#101; illness- &#111;&#114; you don’t. &#102;&#111;&#114; &#101;&#120;&#097;&#109;&#112;&#108;&#101;, many patients &#104;&#097;&#118;&#101; &#100;&#101;&#115;&#099;&#114;&#105;&#098;&#101;&#100; having their condition dismissed &#098;&#121; their doctor simply &#098;&#101;&#099;&#097;&#117;&#115;&#101; they &#100;&#105;&#100; &#110;&#111;&#116; meet &#116;&#104;&#101; strict weight requirement as necessitated &#098;&#121; &#116;&#104;&#101; DSM-IV-TR. As such, a common complaint of people seeking treatment &#105;&#115; &#116;&#104;&#097;&#116; they &#097;&#114;&#101; sent &#116;&#104;&#101; message they &#110;&#101;&#101;&#100; to “get sicker” to meet diagnostic criteria- and &#116;&#104;&#117;&#115; warrant treatment. Needless to &#115;&#097;&#121;, this poses strong barriers to &#116;&#104;&#101; accessing of treatment. This &#105;&#115; particularly &#099;&#111;&#110;&#099;&#101;&#114;&#110;&#105;&#110;&#103; as treatment outcomes generally improve &#119;&#105;&#116;&#104; early intervention. &#119;&#111;&#117;&#108;&#100; it &#098;&#101; more helpful to classify eating disorders along a spectrum, &#108;&#105;&#107;&#101; autism? &#111;&#114; to &#104;&#097;&#118;&#101; a grading &#115;&#121;&#115;&#116;&#101;&#109; &#108;&#105;&#107;&#101; other medical conditions such as burn severity &#111;&#114; cancer?</p>
<p>Another significant limitation of &#116;&#104;&#101; current diagnostic criteria &#105;&#115; &#116;&#104;&#097;&#116; it &#105;&#115; common &#102;&#111;&#114; people to &#109;&#111;&#118;&#101; between diagnoses. &#102;&#111;&#114; &#101;&#120;&#097;&#109;&#112;&#108;&#101;, 40% of people &#119;&#105;&#116;&#104; Anorexia &#097;&#114;&#101; &#097;&#116; risk of developing Bulimia- &#097;&#116; &#116;&#104;&#101; &#115;&#097;&#109;&#101; time, behaviours associated &#119;&#105;&#116;&#104; Bulimia tend to &#098;&#101; a common pathway out of Anorexia. &#115;&#111;&#109;&#101; clinicians &#119;&#111;&#117;&#108;&#100; argue &#116;&#104;&#097;&#116; &#116;&#104;&#101; symptoms (and treatment approaches) &#102;&#111;&#114; an original diagnosis of Bulimia differ from &#116;&#104;&#111;&#115;&#101; &#119;&#104;&#111; exhibit Bulimic symptoms &#102;&#111;&#108;&#108;&#111;&#119;&#105;&#110;&#103; an original diagnosis of Anorexia. Other clinicians adopt a “transdiagnostic” &#097;&#112;&#112;&#114;&#111;&#097;&#099;&#104; whereby specific diagnosis &#105;&#115; &#110;&#111;&#116; &#105;&#109;&#112;&#111;&#114;&#116;&#097;&#110;&#116;.</p>
<p>Much hope &#104;&#097;&#115; been pinned on &#116;&#104;&#101; release of &#116;&#104;&#101; DSM-V, in &#116;&#104;&#097;&#116; it &#119;&#111;&#117;&#108;&#100; capture a broader range of dysfunctional eating patterns, thereby ensuring &#116;&#104;&#097;&#116; &#116;&#104;&#101; bulk of sufferers meet a clearer diagnostic category than EDNOS. <p>We &#104;&#097;&#118;&#101; a desperate &#110;&#101;&#101;&#100; &#102;&#111;&#114; more funding and research into eating disorders, to ensure &#116;&#104;&#097;&#116; diagnostic categories accurately do capture &#119;&#104;&#097;&#116; &#105;&#115; presenting clinically. &#119;&#101; &#097;&#114;&#101; &#112;&#108;&#101;&#097;&#115;&#101;&#100; to note &#116;&#104;&#097;&#116; The Butterfly Foundation has done a wonderful job in this recently- however, this &#105;&#115; really only a start.</p>
<p> The urgency &#105;&#115; increased &#098;&#121; &#116;&#104;&#101; current &#8216;obesity epidemic&#8217;. It &#105;&#115; paramount &#116;&#104;&#097;&#116; more resources &#097;&#114;&#101; invested into understanding &#116;&#104;&#101; extent to &#119;&#104;&#105;&#099;&#104; disordered eating &#105;&#115; contributing to people sitting above their natural body weight. As &#119;&#101; &#104;&#097;&#118;&#101; written &#097;&#098;&#111;&#117;&#116; &#101;&#108;&#115;&#101;&#119;&#104;&#101;&#114;&#101;, it &#109;&#097;&#121; &#098;&#101; &#116;&#104;&#097;&#116; &#116;&#104;&#101; epidemic &#119;&#101; &#097;&#114;&#101; really facing &#105;&#115; one of “disordered eating”- it &#119;&#111;&#117;&#108;&#100; &#098;&#101; helpful &#105;&#102; &#116;&#104;&#101; diagnostic criteria &#119;&#097;&#115; adjusted to assist us in determining this.</p>
