Heartburn-like pain is a common symptom of gastroesophageal reflux disease (GERD). But several other conditions can cause a burning feeling in your chest.

Most of the time, your doctor will be able to identify whether you have heartburn or GERD by doing tests.

Here are nine other conditions that can cause heartburn-like pain.

1. Angina Angina, or chest pain caused by lack of blood flow to the heart, can feel a lot like heartburn. “The major key is if you’re getting heartburn when you’re doing strenuous or moderate activity,” says Dr. Ryan Madanick, a gastroenterologist and assistant professor of medicine at the University of North Carolina School of Medicine, in Chapel Hill.

If you’re 50 or older and getting heartburn—especially if you haven’t had this kind of pain before—it can raise suspicion of angina. Suspicions can also be raised if you’re younger but have heart risk factors such as hypertension, diabetes, or a family history of heart disease. 

2. Gallstones Although gallstones don’t always cause symptoms, a stone blocking your bile duct can hurt, usually in the middle or upper-right side of the abdomen.

Pain may be cramping, dull, or sharp, and often strikes minutes after you eat.

If you’re experiencing stomach pain after meals that doesn’t improve after you take an over-the-counter acid-suppressing medication, gallstones should be suspected, says Dr. Joel Richter, a gastroenterologist and chairman of the department of medicine at Temple University School of Medicine, in Philadelphia.

3. Stomach Ulcer Ulcers can cause a gnawing, burning sensation, usually felt in the upper abdomen. the pain can find its way up to the chest, Madanick says.

Acid-suppressing medications may relieve ulcer pain. But ulcers are usually caused by Helicobacter pylori, a type of bacteria that inflames the stomach lining, so you will need to take antibiotics to clear the infection.

Certain anti-inflammatory drugs (such as aspirin, ibuprofen, and naproxen), and osteoporosis drugs called bisphosphonates, can also cause stomach ulcers.

4. Hiatal Hernia A hiatal hernia occurs when a portion of the upper stomach pokes through the diaphragm into the chest cavity, rather than staying in the abdominal cavity where it belongs.

This can push food and stomach acid up into the esophagus, causing heartburn. Other signs of hiatal hernia include chest pain, belching, and nausea. If you have heartburn due to hiatal hernia, your doctor will typically prescribe acid-suppressing drugs, and recommend lifestyle changes like eating smaller meals, avoiding alcohol, and not eating right before bed. in rare cases, surgical repair may be warranted.

5. Esophageal Cancer Esophageal cancer is rare, but its incidence is rising rapidly in the United States, Madanick says.

“If you have heartburn, it might be a sign of esophageal cancer, but it’s highly unlikely,” he says. your doctor may decide to order an upper endoscopy to examine your esophagus if you’ve got long-standing heartburn, especially if you smoke or drink heavily, both of which are risk factors for esophageal cancer.

This test involves passing a tube with a light and a camera at one end down your throat into your esophagus. During the test, your doctor can look for abnormal areas as well as collect tissue samples to test for cancer.

6. Gastroparesis Poor control of diabetes can lead to nerve damage, which can affect the workings of your digestive tract.

This is called gastroparesis, and it dramatically slows the movement of food through the stomach and can cause heartburn.

Treatment can include dietary changes such as eating smaller meals, avoiding fat and fiber, medications, and, for people with very severe symptoms, inserting a feeding tube or an implanted device that emits electrical pulses mimicking stomach contractions.

7. Esophagitis Esophagitis, or inflammation of the esophagus, can result from frequent acid reflux. This condition can in turn lead to more heartburn, as well as difficulty swallowing.

The esophagus can also become inflamed from taking certain painkillers and osteoporosis medications, particularly if the pills are taken without water, allowing them to remain in the esophagus.

A third type of esophagitis, called eosinophilic esophagitis, occurs when white blood cells known as eosinophils invade the esophagus. the condition is often allergy-related, so treatment requires identifying and avoiding the offending foods. Doctors may also prescribe steroid medications to ease inflammation.

8. Pleuritis or Costochondritis Inflammation of the lining of the lungs and chest cavity, known as pleuritis or pleurisy, can cause heartburn-like chest pain. Pleuritis should be suspected “if the pain or burning gets worse when you take deep breaths or move around,” Madanick says.

Pleuritis is most commonly caused by a viral or bacterial infection, and resolves when the infection does.

Costochondritis—an inflammation of the cartilage anchoring ribs to the breastbone—can cause sharp pain along the breastbone or sternum. It can be related to injury or infection, and typically is treated with anti-inflammatory medicines, pain relievers, and rest.

9. Anxiety Although anxiety won’t cause GERD, it can cause heartburn and make GERD symptoms worse, Madanick says. a person can have both anxiety-related heartburn and GERD-related heartburn. 

“One of the signs that it’s not just reflux is that treating the reflux problem doesn’t make it any better,” he says.

Reducing anxiety and stress through, for example, exercise, relaxation, and therapy, can also ease heartburn.

Madanick says that, as with other conditions that masquerade as GERD, “many times the only times we will see a patient…is when the over-the-counter medications haven’t worked, because the medications tend to be very effective for treating simple heartburn.”

Liver Cancer ? Causes, Symptoms and Treatment

Your liver is the largest organ inside your body. It filters harmful substances from the blood, digests fats from food and stores the sugar that your body uses for energy. Primary liver cancer starts in the liver. Metastatic liver cancer starts somewhere else and spreads to your liver.

What are the symptoms of liver cancer?

The initial symptoms (the clinical presentations) of liver cancer are variable. in countries where liver cancer is very common, the cancer generally is discovered at a very advanced stage of disease for several reasons. for one thing, areas where there is a high frequency of liver cancer are generally developing countries where access to healthcare is limited. for another, screening examinations for patients at risk for developing liver cancer are not available in these areas. in addition, patients from these regions actually have more aggressive liver cancer disease.

Jaundice if the bile duct becomes blocked, bile produced by the liver will flow back into the bloodstream, causing jaundice. this will cause the skin and whites of the eyes to go yellow and may make the skin very itchy. the itching may sometimes be relieved by antihistamine tablets or other drugs, which your doctor can prescribe. Sometimes the jaundice itself can be relieved.

Causes of Liver Cancer

Your liver is a football-sized organ that sits in the upper right portion of your abdomen, beneath your diaphragm and above your stomach. your liver processes most of the nutrients absorbed from your small intestine and determines how much sugar (glucose), protein and fat enter your bloodstream. It also manufactures blood-clotting substances and certain proteins. your liver performs a vital detoxifying function by removing drugs, alcohol and other harmful substances from your bloodstream.

Many factors may play a role in the development of cancer. Because the liver filters blood from all parts of the body, cancer cells from elsewhere can lodge in the liver and start to grow. Cancers that begin in the gut often spread to the liver. the ability of the liver to regenerate may also be linked to the development of liver cancers.

How is liver cancer diagnosed?

Liver cancer may be discovered in a routine checkup if the doctor feels hard lumps in the abdomen, or incidentally by imaging studies. To confirm a diagnosis of liver cancer, doctors would use blood tests; ultrasound; computer tomography (CT) scans; and magnetic resonance imaging, or MRI. Doctors may find it necessary to do a biopsy, where a small sample of liver tissue is removed with a needle and then examined for cancer cells.

Liver Cancer Treatment

Systemic chemotherapy for hepatocellular carcinoma has not been very successful. Doxorubicin, cisplatin and 5-FU have been the most successful drugs, however response rates have been very low (less than 10%) and treatment has had no impact on overall survival. the use of combination chemotherapy has not shown to have any advantage over single-agent chemotherapy. Intraarterail chemotherapy (mitomycin, doxorubicin, cisplatin) can be given into the hepatic artery that is providing blood supply to the tumor.

In situations in which spread is limited to the liver, systemic chemotherapy may be used, although other treatment methods may be effective. when the tumor is localized to only a few areas of the liver, the cancer may be removed surgically.

Use of radiofrequency waves or injection of toxic substances may also be used to kill tumors. when larger areas of the liver are involved, liver-directed chemotherapy (infusing chemotherapy directly into the liver), or embolization (blockage of blood flow to parts of the liver) may be used.

Patients with such limited cancer may benefit from chemotherapy infused directly into the liver. Delivered by an implanted pump connected to the hepatic artery, this therapy takes advantage of the liver’s ability to metabolize some drugs, meaning that the tumor may be exposed to high concentrations of chemotherapy while the rest of the body is spared the side effects.

Acute pancreatitis is usually caused by excess alcohol consumption or a disease of the gall bladder or the bile duct. Acute pancreatitis symptoms in pregnant women can be because of two reasons, viz. increased estrogen effect and blockage of the pancreas by gallstones. At least 1 out of 3,333 pregnant women are known to pick up this syndrome and the symptoms are usually present throughout the pregnancy.Acute Pancreatitis Symptoms in Pregnant Women: As you can see, the incidence of acute pancreatitis in women is rare. But a pregnant woman who develops this disease can have the following symptoms-

• mid epigastric pain, pain in the central abdomen due to the presence of gallstones
• radiating pain in the left flank, usually towards the top
• nausea
• vomiting
• low temperatures and symptoms of fever
• anorexia (an eating disorder)
• pulmonary issues which can also lead to respiratory distress syndrome with the symptoms being jaundice, tenderness in the abdominal region, hypercalcemia (presence of excess calcium in the blood) and muscle rigidity.

It is very important to diagnose the underlying pancreatitis disease in women especially in the first trimester as a mis-diagnosis can cause death or result in fetal loss.

Reasons for the Disease: The most common cause of the disease in pregnant women is the presence of gall stones which obstruct the pancreatic duct. This condition is known as cholelithiasis. another very common reason is the hormonal imbalance of estrogen which can lead to high triglyceride (fat) levels. High triglyceride levels block oxygen from reaching the pancreas, thus causing pancreatitis.

Apart from the above two mentioned reasons, alcohol consumption during pregnancy, reaction to medications, and pancreatic duct trauma can also cause pancreatitis.

Treatment: The treatment of this condition in pregnant women is similar to that in non-pregnant women.

• Eating should be strictly avoided in order to rest the digestive tract which will also reduce the pain. instead, fluids would be given intravenously.
• If it is the gallbladder that is causing the problem, it needs to be removed, but only after the pregnancy. during this time, a stent is placed in the bile duct for temporary relief.
• If it is the triglycerides that are causing the problem, then diet restrictions are necessary to help prevent further attacks.

If a pregnant woman has to exhibit any of the above mentioned acute pancreatitis symptoms, then it is important to diagnose the condition and take the necessary precautions. Though very rare, maternal death can be a certainty if the condition is misdiagnosed.

Uphill struggle: Dr Scurr says you should not listen to people who claim they can get rid of gallstones with natural remedies

Yi Dan is a form of herbal medicine and, as regular readers will know, I am no fan of alternative therapies. But before I explain why, first to the gallstones.

About 20 per cent of people over the age of 65 have them, and in most cases they’re not a problem. however, in some cases they cause pain, infection in the gallbladder (cholecystitis) or bile-duct system (cholangitis), or even an inflamed pancreas (pancreatitis), a common and potentially very serious complication.

There are different types of gallstones. The most common are formed of cholesterol; there are also black or brown ‘pigment’ stones, made up of waste products excreted by the liver. Stones grow about 1mm to 2mm each year, taking some years to become big enough to cause problems.

Why do gallstones form? The process starts with bile, which is made by the liver and carries away unwanted waste such as cholesterol. Bile travels from the liver via the common bile duct. some is then diverted up a short tube, the cystic duct, into the gallbladder, which acts as a storage reservoir.

The wall of the gallbladder absorbs 90 per cent of the water from the bile, concentrating it. When you eat, the gallbladder empties the stored bile back into the common bile duct.

But if the gallbladder is diseased — common risk factors include obesity, genetics and a rich, fatty, over-calorific diet — waste products such as the pigments formed by the breakdown of old blood cells collect into small stones.

Gallstones can be dissolved by taking bile acids orally for many months, though this is only effective for cholesterol stones. And the success rate with large stones is poor; even in the best cases, less than half are dissolved after two years of treatment.

The stones can also recur — around 50 per cent of patients suffer again within five years.

A bigger worry is that stones made smaller by dissolution can become small enough to be squeezed down the cystic duct into the common bile duct and cause a blockage — a considerable problem. in more than 30 years as a GP, I’ve never had a patient deal successfully with gallstones in this way.

Alternative treatments: Dr Scurr has never met anyone who cured their gallstones with olive oil or lemon juice

The gold-standard treatment is cholecystectomy — removal of the gallbladder. this is offered once symptoms occur and is usually a keyhole procedure. it does not affect digestion, although some patients develop chronic diarrhoea.

My patients often mention the alternative method you describe and always seem to know somebody who knows somebody for whom it worked. But I’ve never managed to speak to an actual patient who’s had it, and frankly I think it’s a fairy story, perpetuated by the internet.

In any case, it’s difficult to imagine how Yi Dan would help flush your 2cm stone down the common bile duct, which is just 5mm in diameter.

I receive many readers’ letters seeking advice on alternative treatments. I do sympathise, as I think it’s essentially a quest for being listened to with patience, kindness and understanding: these can be in short supply in the hard-pressed world of high-tech medical care.

However, don’t confuse this with efficacy: alternative treatments are ‘backed’ only by anecdote.

But it works, people say. Well, that’s the magic of the placebo effect.

Nowhere is this more true than with homeopathy, which James Delingpole recently wrote about in these pages.

Homeopathy is based on the joke principle that like treats like — giving minute doses of a poison which, in larger amounts, apparently produces the symptoms from which the patient suffers.

This is not the same as herbal medicine (heroin, from the opium poppy, is a herbal medicine and really works for pain). in homeopathy, the substance used is so diluted there’s none left.

Put a sugar lump in the sea at Portsmouth and take a spoonful of seawater at Penzance and you’ll understand the sort of dilutions being employed. Sure, homeopathy is well meaning, but it’s based on wishful thinking and, as such, is dishonest.

Just like the loony Yi Dan treatment proposed for your gallstones. Disappointing I know, but please don’t waste your time. as for the olive oil and lemon juice, I’m afraid that won’t help either.

The phrase ‘hardening of the arteries’ refers to the process of atherosclerosis, when deposits of fatty cholesterol material build up inan artery. Risk factors include high cholesterol, raised blood pressure, smoking, diabetes and obesity. Any artery in the body can be affected: in your case, it was the carotid artery.

The two carotid arteries, one on each side of the neck, are the most important arteries supplying blood to your brain.

The risk is that a clot forms on a section narrowed by a fatty build-up. The clot then detaches and is carried up into the brain, whereit eventually blocks a smaller artery, reducing blood supply and damaging the area of brain cells the vessel was supplying — causing an ischaemic stroke.

In patients with at least 50 per cent narrowing of the artery, surgery is used to remove the fatty build-up. however, studies suggest there’s no benefit in operating on those with 30 to 49 per cent narrowing; and surgery is actually harmful in those with less than 30 per cent.

That’s why you’ll have been advised to take a small daily dose of aspirin, as this makes the blood less sticky and reduces the chances of aclot. It’s also advisable to control blood pressure meticulously; buy your own machine and use it once a week.

Most important, taking a statin is essential, preventing further cholesterol build-up and even shrinking the blockage.

In your letter, you also mention suffering neck pains with small amounts of alcohol. this, I suspect, is unrelated to your arteries and may be due to a creaky old neck, which blights many of us.

Save your pennies: Private health insurance doesn’t cover primary care

Just at the moment financial cutbacks and a major re-organisation put the NHS under huge strain, up pop those seductive ads for private healthcare. they imply any illness will be paid for, and you can bypass the NHS. Ah, such peace of mind.

But there’s not one policy anywhere that covers primary care (GP care), and yet 90 per cent of anything that might happen to you is treated in general practice. Your attention is craftily distracted by the promise of cover for osteopathy, physiotherapy and other more peripheral treatments — but before anything can be treated, you need a diagnosis!

My other concern is insurers will attempt to hijack your medical care by ‘helping you find a consultant’. since when have clerks at the big insurance providers had the training in medical care to select specialists?

Furthermore, and even more insidious, is that insurers tell the specialists what fee they may charge: anyone who doesn’t sign up to this is not registered by the insurer. not only does this improperly suggest to the layman that ‘unregistered’ consultants have not passed muster in some way, but patients see specialists chosen on the basis of cost, not excellence.

Meanwhile, the specialists themselves have to do more work in less time; the only way to cover their overheads is to squeeze in more consultations. why go privately if you don’t then get to spend time with the specialist and receive a detailed opinion?

My approach is to opt for in-patient cover only. this means the insurer re-imburses the fees when the problem is so serious you must be in hospital, but keeps the premiums down. otherwise it’s just a rip off.

Cats with only a mild case of pancreatitis usually have a good prognosis. However, sometimes the condition recurs more severely after an apparent successful treatment. Also, cats suffering from other conditions have a worse prognosis. Some of these other conditions include diabetes and liver or small intestine diseases.

The liver is the largest organ in the body, and the main heatproducing organ.  it is surrounded by a fibrous capsule and is divided into sections called lobes.  it is situated in the upper part of the abdomen on the right-hand side of the body and is surrounded and protected from injury by the lower ribs.

The liver is connected to the small intestine (duodenum) by a tube called the bile duct. this duct takes the bile produced by the liver to the intestine.The liver has an amazing ability to repair itself. it can function normally with only a small part of it in working order.

Primary liver cancer which starts in the liver itself, is one of the most common cancers in some parts of Asia, including the coastal area of China, for example Hong Kong, Taiwan, Guangxi and Vietnam.

Two types of primary liver cancer:

-Hepatoma or Hepatocellular Carcinoma (HCC): is the most common kind and arises from the main cells of the liver (the hepatocytes).this type is usually confined to the liver, although occasionally it spreads to other organs and occurs mostly in people with a liver disease called cirrhosis.  there is also a rarer sub-type of hepatoma called Fibrolamellar hepatoma, which may occur in younger people and is not related to previous liver disease.-Cholangio carcinoma or bile duct cancer: starts in the cells lining the bile ducts.Some primary tumours in the liver are non-cancerous  (benign) and do not spread to other parts of the body.  they are usually small and may cause no symptoms, and are often discovered by chance during operations or investigations for other conditions. Unless they are causing symptoms they do not usually need to be removed.The causes of primary liver cancer

1. Hepatoma (cancer of the liver cells):

a. Lots of people who develop hepatoma usually have a condition called cirrhosis of the liver. this is a fine scarring throughout the liver which is due to a variety of causes including infection and heavy alcohol drinking over a long period of time. However, only  a small proportion of people who have cirrhosis of the liver develop primary liver cancer.b. Infection with either the hepatitis B or hepatitis C virus can lead to liver cancer, and can also be the cause of cirrhosis, which increases the risk of developing hepatoma.c. People who have a rare condition called haemochromatosis, which causes excess deposits of iron in the body, have a higher chance of developing hepatoma.d. In Asia a poison called aflatoxin, found in mouldy peanuts and grain, is an important cause of hepatoma.

2. Bile duct cancers (cholangio carcinomas) are less common than hepatomas. The cause of most bile duct cancers is unknown, but they are slightly more likely to occur in people:a. with conditions which cause inflammation of the bowel, such as ulcerative colitis.b. In Asia, infection with a parasite known as the liver fluke is thought to cause many cholangiocarcinomas.

The symptoms of primary liver cancer

In the early stages of primary liver cancer, there are often no symptoms.People sometimes notice a vague discomfort in the upper abdomen that may become painful. this is due to enlargement of the liver.Pain can sometimes also be felt in the right shoulder. Thisis known as referred pain and is due to an enlarged liverstimulating the nerves beneath the diaphragm (the sheet of uscle under the lungs) which are connected to nerves in the right shoulder.

1. Common symptoms:a. Loss of appetiteb. Weight lossc. Feeling sick (nausea)d. Weaknesse. tiredness (lethargy)f. it may also develop a hightemperature and feel shivery.2. Other symptoms:a. Jaundice- The itching may sometimes be relieved by antihistamine tablets or other drugs which your doctor can prescribe. Sometimes the jaundice itself can be relieved.- this is done by inserting a narrow tube called a stent into the bile duct to keep it open and allow the bile to flow normally into the small intestine.- Other signs of jaundice are dark-coloured urine and pale stools (bowel motions).b. AscitesSometimes fluid builds up in the abdomen and causes swelling known as ascites. there may be several possible reasons for this:- if cancer cells have spread to the lining of the abdomen, they can irritate it and cause fluid to build up- if the liver itself is affected by cancer cells, this causes an increase in pressure in the veins which lead into the liver. Fluid from the abdomen cannot then pass quickly enough through the liver, so it starts to collect in the abdomen- if the liver is damaged, it may produce less blood protein..  this may upset the body’s fluid balance, which causes fluid to build up in the body tissues, including the abdomen- if the cancer cells blocking the lymphatic system. The lymphatic system is a network of fine channels which runs throughout the body. one of its functions is to drain off excess fluid, which is eventually passed out of the body in the urine. If some of these channels are blocked, the system cannot drain efficiently and fluid may build up.- if ascites does develop, a tube can be put through the wall of the abdomen to drain the fluid away.  whatever the cause, jaundice or ascites will always indicate  a condition that needs medical attention and so should not be ignored. Always have these symptoms checked by your doctor.Secondary or metastatic liver cancer

This is a cancer which has started somewhere else in the body and has spread to the liver.  almost any cancer can spread to the liver, but the most common ones include bowel, pancreas, stomach, lung and breast cancer

The causes of secondary liver cancer

The cause of secondary liver cancer is always a primary cancer situated elsewhere in the body that has spread to the liver. If cancer cells from the primary cancer have escaped into the bloodstream, the liver is a likely place for them to settle as all the blood in the body passes through the liver.

The symptoms of secondary liver cancer

The symptoms are similar for both primary and secondary liver cancer. In the early stages of both these types of liver cancer there are often no symptoms.  Although you should always have these symptoms checked by your doctor, it is important to remember that  they are common to many conditions other than cancer.whatever the cause, jaundice or ascites will always indicate a condition that needs medical attention and so should not be ignored.
