Aman Foundation launches second health initiative.

KARACHI: At least 40% of deaths in Pakistan are preventable, said the chief executive officer for Aman Foundation, Ahsan Jamil at the launch of the organisation’s latest initiative Aman Telehealth. “Most of these deaths occur due to three major factors – lack of transport to a healthcare facility, affordability and lack of decision makers being present at time of the emergency.”

In a previously conducted study, Dr Sadiqua N Jafery concluded that the most common of the three reasons was the lack of decision maker ie husband, mother-in-law, father or mother being present. according to Jamil, the launch of a system whereby you can receive an initial diagnosis and prescription for non-chronic symptoms on the telephone is therefore most opportune. the telehealth project has been in the works for several months now with ongoing training for the 26-strong staff to attend to a caller’s medical queries. the service will be in operation 24 hours, seven days a week on 111-113-737.

“At present we have the capacity to attend to 1,000 to 1,200 calls a day, with 10 attendants in the morning, 10 in the evening and six overnight,” said Dr Zahid Ali Faheem who is heading Aman Telehealth.

The call is answered by a registration officer who notes the caller’s name, address, gender and age. the call is then transferred to a health advice officer ie nurse or doctor where a list of your symptoms are noted in a system. if the programme is unable to generate a suitable response the call is then passed on to a medical officer who further questions the caller about their symptoms.

In case of emergency, the call is routed to the ambulance service.

According to Faheem, among other objectives they hope to achieve greater empowerment for women, early-stage intervention and reduce incidence of medical emergency through this initiative. “In three to five years we hope to increase our attendants to 150.” but the process of increasing staff as well as advertising the service needs to be gradual for a sustainable setup cautions executive director for Aman Health Dr Junaid Razzak.

“The most important thing is to fill the void of missed opportunity,” Razzak said. the most difficult part for a medical professional is not the medical life saving procedures they give but delivering the news to loved ones of the patient’s condition or demise, he added. “It is with the hope to decrease these conversations by early intervention and diagnosis that Aman Telehealth has been launched.”

The United Nations World Health Organization (WHO) has provided essential drugs to help treat an outbreak of dengue fever that has affected an estimated 1,000 people in the north-eastern Kenyan district of Mandera, which borders Ethiopia and Somalia.

There have been four unconfirmed deaths and some patients have been showing symptoms of Dengue Haemorrhagic Fever, a more severe form of the disease, the UN Office for the Coordination of Humanitarian Affairs (OCHA), reported in an update.

All neighboring districts have been alerted and people advised to seek early medical attention if they become ill.

Meanwhile, anaemia prevalence among children under the age of five in the refugee camps in Kenya’s Dadaab area is above 40 percent, according to OCHA. the camps host nearly 470,000 refugees, the majority of them from Somalia.

Three of 6 suspected cases of cholera in the Dadaab camps have been confirmed and all the patients arrived recently from Somalia. Some 82 percent of the refugee population have received measles vaccination during a recent mass inoculation campaign.

An estimated 3.75 million people in Kenya have been experiencing food shortages a result of a prolonged drought in the Horn of Africa, and relief agencies are working with the Government to scale up distributions to the affected populations through one coordinated pipeline from October.

In August, the overall number of admissions to hospital of children with severe acute malnutrition declined from 6,379 to 5,560 cases, but an increase in admissions has been reported in the districts of Marsabit, Samburu, West Pokot and Laikipia.

While no dengue fever related deaths have been confirmed, the Ministry of Health is investigating several claims of deaths as a result of the illness, according to Dr. Delon Brennen, deputy chief medical officer.

“We do know that there have been some reports and we’re investigating those on behalf of the Ministry of Health,” Dr. Brennen said.

“We’re getting samples of blood from those patients in order to send it out so that we can figure out whether it was related or not.  In addition to that, you also have to investigate some of the past medical conditions that persons had in order to really see whether dengue was a contributing factor to their death.”

There have been around 1,000 combined clinically diagnosed and laboratory confirmed cases of dengue in recent months, according to data from the Ministry of Health.

Officials have said previously that the strain of dengue fever that is seen locally is not the fatal form of the illness.

However, Dr. Brennen explained that dengue fever, like the common flu, can lead to complications in people who have existing medical issues.

“So anytime you have a complicating medical illness, one of the chronic non-communicable diseases, an immune deficiency, any of those can lead you to worse outcomes as well.  so if you are susceptible to getting infections, you too will (be vulnerable).  Influenza will do the same thing.  If you have chronic respiratory diseases, asthma, or COPD (chronic obstructive pulmonary disease), if you get influenza you will get sicker than most people will.  so it’s the same thing with dengue.  If you already have any chronic non-communicable disease, dengue just complicates that on top of it,” he said.

Yesterday online media reports suggested that one man died as a result of dengue.

Dr. Brennen said he is not surprised about the number of death claims the Ministry of Health has received.

“When people die during an outbreak such as this, lots of people claim things.  so what we need to do is investigate all of those claims,” he said.

In recent days, a large number of people have gone to public and private hospitals and clinics exhibiting symptoms of the virus.

Dr. Brennen said that up to yesterday there were 90 laboratory confirmed dengue cases.  however, there are hundreds of other clinically diagnosed cases.

“When you consider the case definition with persons who come in with symptoms close to dengue such as headache, fever, or pain behind the eye, we have more patients.  so you’re still talking about 1,000 of those patients who have been seen,” he said.

“But again those numbers are truly preliminary because without being able to confirm which of those cases are definitely dengue, it’s purely preliminary. As time goes some cases may be added and some may be taken out.”

Dengue fever is transmitted through the sting of the aedes aegypti mosquito. The Department of Environmental Health Services is fogging throughout New Providence and some of the Family Islands in order to reduce the mosquito population in the country.

Health officials say individuals should take steps to eliminate mosquito breeding grounds around their homes by removing all standing water in discarded cans, bottles, and tires and draining water from the base of planters.

People with mild symptoms are being asked to treat themselves at home by getting rest, drinking fluids and taking painkillers such as Panadol or Tylenol for fever, headache and joint and muscle pain.

However, if symptoms persists infected people should seek medical attention.

Dengue hemorrhagic fever is the most deadly form of the disease and it is not common in The Bahamas.

KNOXVILLE, Tenn. (WVLT) — the heat has been unbearable here in East Tennessee, but in other parts of the state it’s been deadly.

There have been two heat related deaths in West Tennessee. in Shelby County, a 75-year-old woman, with health issues, died in her home, when her air conditioner wasn’t on. And, in Tipton County, a 60-year-old man died after falling in his driveway, and lying in the heat for hours.

The heat wave continues, so there are things you need to know to make sure your family, of all ages, is safe outside.

David Farmer took his three daughters to the Carl Cowan Park in Farragut too cool off Wednesday. “Unfortunately we’ve had to stay inside a lot just because it’s so hot, and then you’ve got to find water sources just to cool the body off,” he said.

He is trying to find ways to have fun and be safe in the sun. He said, “They do get heated and flushed, but they don’t want to stop, so you’ve got to find cool sources.”

UT Medical Center’s emergency department sees an increase in heat related illnesses about this time every year, and Nurse Manager Christian Lawson says you can get sick from the sun, quicker than you might realize.

He said, “The extreme heat setting, symptoms could develop with in 15 to 20 minutes if you’re not properly hydrating.”

He said you need to keep an eye out for certain signs and symptoms, starting with Heat Exhaustion, when you’re sweating a lot, and feeling dizzy. Cramps are also possible, and a more common symptom for kids.

The worst is Heat Stroke, he said, “Your body has exhausted all of it’s compensatory mechanisms. You’re not sweating anymore, your skin is very hot, your actual internal body temperature gets above normal, and that is a true medical emergency.”

Lawson said children and the elderly are the most common victims of heat illnesses, and “If you have diabetes high blood pressure, other illnesses like that, that predisposes you to develop heat related signs and symptoms and illnesses more quickly,” he said.

You can prepare for extreme heat by drinking plenty of water before you go out, and avoiding direct sunlight.

David pointed out he purposely sat on the shaded bench, while his girls played in the water.

Lawson said to also check your clothing labels, “There’s kind of a term in the emergency setting world that ‘cotton kills.’ It’s not the best to be wearing in the heat.” He says synthetics will help get the sweat away from your body.

Lawson said everyone should get a minimum of 64 ounces of water a day, but if you’re going to be outside or exerting yourself, then you need to drink even more. Sports drinks are a good alternative to water, but he says sodas are the worst when you’re dehydrated.

Free prostate cancer screenings are being offered to men ages 40 and offer from 10 a.m. to 2 p.m. Saturday at the Pike County Health Department.

The screenings will be conducted by Urology Centers of Alabama, which offered a similar screening in April 2010. at that time, 87 men took advantage of the service. Of those, 16 percent had some abnormality and at least two had positive biopsies, said Dr. Tom Moody.

“Screening for prostate cancer is very important because, like other cancers, when detected early, it is curable. if prostrate cancer is not detected early, it is not curable,” Moody said. “Prostate cancer continues to be the second leading cause of deaths among men. In 2010, 600 men in Alabama died from prostate cancer and many of those deaths could have been prevented.”

Moody said early prostate cancer has no symptoms and, when the symptoms appear, it is often too late.

“Our goal is to reduce the number of deaths from prostate cancer through early detection,” Moody said. “We can best do that by prostate screenings for men ages 40 and over. we are trying to create the same mindset in men that women have regarding their annual checkups.”

The prostate screenings are free so there are no insurance forms to fill up or financial disclosures required.

“If the screenings find an abnormality and the individual doesn’t have the ability to pay, we will do whatever follow up is necessary,” Moody said. “We have done a lot of biopsies at the Urology Centers of Alabama and surgeries, too. we aren’t going to screen an individual and find a problem and say, ‘good luck.’ We’re going to do whatever needs to be done.”

The prostate screening is a simple procedure that includes a blood screening and a rectal examination.

“The screening only takes 10 minutes and it saves lives,” Moody said.

Urology Centers of Alabama has been conducting free prostate screenings since 2006 and to date 2,582 men in 14 counties have been screened.

“And, what is so gratifying is that many of those who were screened for the first time are returning and bringing friends with them,” Moody said. “That give us an opportunity to provide a continuity of care as far as prostate cancer is concerned.”

In 2006, Alabama received an “F” when the National Prostrate Council Coalition graded the states on how they took care of prostate cancer.

“That really bugged me that we got an ‘F,’” Moody said. “At that time, Alabama didn’t have an insurance bill that required insurance companies to pay for prostate screenings. we got an insurance bill passed and also had an opportunity to talk to legislators and got some money flowing to pay for some of the free screenings.”

Moody expressed appreciation to the Pike County Health Department for its partnership in the free prostate screening program.

“Alabama has an outstanding health department and it has worked out to be a great partnership between us and the health departments,” he said. “We like to believe that together we are doing a little good.”
