We all have nagging worries — did I turn off the stove? did I lock the door? but for Canadians with obsessive-compulsive disorders, these thoughts get stuck, playing over and over in the mind like a broken record. And the only way to lower the volume is to perform “rituals” — behaviours such as constant checking, excessive washing or extreme hoarding to control the unwanted thoughts. Symptoms can be so severe that that some patients are unable to hold down jobs, go to school or even leave their homes.

Paul (not his real name) is a prime example. he is 14 years old and has lived with severe OCD for a decade. in his tortured world, everything must stay on his left. Paul will obsessively move, pictures, furniture, even people to his left side. he believes that if things are on his right, he will suffer extreme pain.

“I feel lots of it pain and pressure on my whole right hand side and I am very anxious and I have a hard time breathing… and I sweat a lot and shake a little… and my heart beats fast,” Paul says.

To avoid feeling the pain, he will ask his mother “Julie” to hide her hands under a blanket and order his father to keep completely still, legs uncrossed, hands placed symmetrically on both knees. His parents admit they live “like statues,” so trained by Paul to minimize their movements.

“It’s difficult to live this way, to see your son be preoccupied by everything,” says Julie.

As a toddler, Paul would ask his parents to repeat sentences over and over; threw terrifying tantrums if objects were not placed in the right order. His parents tried disciplining him. but it didn’t work. in fact, his behaviour grew more bizarre. by age five they started taking him to doctors in a desperate attempt to get help.

“You have to be a parent of a child with OCD to truly understand what is going on. Even in the first year we were wondering, what is happening to our son? What is taking over his life and ours? It really seems unbelievable,” says Julie.

“The reason we went to specialists when he was five was we wanted early treatment so he wouldn’t get entrenched and we didn’t know it was OCD. we saw many psychiatrists and psychologists. they all tried their best,” she says, trailing off shaking her head, indicating nothing worked.

Some specialists prescribed drugs that damaged Paul’s heart and liver. others put him in hospital for months at a time.

After 10 years of seeing doctors without success, Paul is trapped by distorted thinking. he can’t play outside because there are birds, trees and other houses, on his left. he is too exhausted to go to school, his life hijacked by his bizarre symptoms. he can’t even hug his parents, because they would touch him on his left, causing him pain.

“I’ve had OCD for so long I can’t even think how it will be without having some,” says Paul. “I can’t imagine myself without the OCD. I don’t know what that feels like.”

He is not alone. between 600,000 and 1.5 million Canadians will develop OCD at some point in their lives. It is the fourth most common mental disorder and among the 10 leading causes of medical disability worldwide. while some people live normally with mild symptoms, others are debilitated by the disease.

Gideon Goldman-Posluns, 23, was a normal university student and avid baseball player who developed a peanut allergy — a legitimate health problem. but his worries about the condition snowballed into an obsession and then into severe OCD.

It started innocently enough. in 2009, he ate an egg tofu wrap before heading out to a baseball game. The snack contained traces of peanut and triggered an allergic reaction and a panic attack. Physically, Gideon recovered, but mentally, he was not the same.

Gideon feared everything he ate or touched or even inhaled might have traces of peanut.

“I was in prison in my mind because everything was contaminated and I had to be very safe and clean everything,” he says.

He started washing his hands until they were raw. he wouldn’t touch doors, opening them with his feet, obsessively washing even food packages. Eventually, he was eating only apple juice, Kraft Dinner and bread alone in his bedroom. It took him four hours to eat, fearing every bite would kill him.

His friends and family thought he could simply stop these odd behaviours.

“They said I was crazy and stupid and ‘Snap out of it, it’s all in your head. It’s not the truth and logical, you can tough it out.’ well, no, I can’t,” he says.

“I was getting bounced going from psychologist to psychologist. after the third psychologist, I realized that this was not the right route and I needed specialized assistance,” says Howard.

Scouring the Internet, he found the Obsessive Compulsive Disorder Clinic at McGill University Health Centre, one of the few clinics specializing in OCD, and its director, clinical psychologist Debbie Sookman.

Gideon says that Dr. Sookman immediately recognized that his son’s case was life threatening. Paul was admitted to Royal Victoria Hospital in Montreal and stayed for six weeks.

Unfortunately, patients like Gideon and Paul are quite common, says Dr. Sookman, who is also an associate professor at McGill University’s Department of Psychiatry.

“Every day that goes by, there are sufferers who are not getting the hope they need and getting the help they need — not able to go to school not able to work, not able to care for children, families, not able to leave the house. Patients who come into their home who have to strip and spend hours in the shower until they are tolerably clean,” she says. “Untreated OCD, in most cases, is progressively worsening to the point of disability. The patient can’t function.”

OCD can be treated successfully, but too many people with OCD spend years unable to get proper help.

The problem, says Sookman, is there aren’t enough specialized OCD clinics in Canada or therapists certified in the labour intensive treatment often needed. There are often long wait lists for treatment. some provinces don’t cover the therapy, opting to send patients to U.S. clinics.

That’s why last fall, she founded the Canadian Institute for Obsessive Compulsive Disorders. its mandate to increase the number of clinicians trained and certified in specialized OCD therapy in Canada. Forty top OCD experts worldwide are involved in the project. ictoc.org/

“The Canadian Institute for Obsessive Compulsive Disorders is a major mental health development in Canada in this field. It was desperately needed,” says Sookman, who is the president of the organization.

She wants to “make sure over time, that children, adolescents and adults across Canada get the care that they need, specialized treatment, quickly and best practice because this is a disorder that requires very quick treatment to avoid unnecessary suffering, disability.”

Treatment for OCD include medications to lower their anxiety or treat their depression. but drugs alone cannot cure the disorder.

Studies show the most effective treatment comes from within the brain. It is called cognitive behaviour therapy. Patients learn strategies to manage their anxiety. And when they are ready, they can start exposure response prevention therapy, in which they must face the very things they fear the most without performing any rituals.

Over time, anxiety diminishes as patients realize their worst fears never materialize.

“We are trying to help the individual with OCD to be less scared of their own thoughts, less scared of their own anxiety,” says Sookman.

It is challenging, time-consuming therapy that often requires three-hour sessions and daily home visits over several months.

Sookman says she’s treated some 7,000 OCD patients this way. one of her success stories is Lori Merling.

Today, Merling is an accomplished, ambitious university student. but at age 10, a family illness appeared to set off a firestorm of disturbing thoughts in Lori’s young mind.

“I would lie in bed some nights when everyone was asleep (and think) ‘What if I take a knife and kill one of my parents in their sleep?’ these were not things I wanted to do. That’s why these thoughts were disturbing to me. It was scary.”

Lori also began to fear parts of her personality would “escape” her body and “stick” on cereal boxes. as difficult as this is to comprehend, the fear of losing herself left Lori distressed, literally scraping her personality off of the box fronts and making swallowing motions to make herself feel whole.

She experienced an often misunderstood OCD symptom called “magical thinking” — for example, believing stepping on a crack will literally break your mother’s back.

“It’s horrible… You realize what you’re doing is nuts. It doesn’t make sense in the real world. It takes on a life of its own and it controls you,” says Lori.

But after seeing an American documentary on OCD, Lori diagnosed herself. With her parents’ help, she was referred to the McGill OCD clinic, where she was treated with cognitive and exposure therapy.

“It was taking my greatest fears and putting them in front of me. Deal with the pain. It was scary and awful,” she says.

But the intensive cognitive and exposure therapy worked. Lori now says she is cured.

“This is something I am proud of. It’s an achievement. It’s one of the greatest things I’ve ever achieved in my life, overcoming this disorder. I fought this battle and won,” says Lori.

Lori is now studying to become a psychologist specializing in obsessive-compulsive disorder. She also volunteers with the Canadian Institute for obsessive-compulsive disorders and supports their mission of expanding specialized treatment centres for OCD.

“I want to do for other people what was done for me. I want to liberate others from this. I know it can be done. I am living proof,” says Lori.

She says there aren’t enough resources to treat all of those afflicted with OCD in Canada.

“The cost of not doing this is greater. The cost in suffering, unnecessary health care utilization, because they are not going directly to specialists,” says Sookman.

But when patients can get treatment and follow-through, the results can be liberating.

Gideon is 18 months into his treatment. while he is taking some medication to control his anxiety, the therapy is helping him realize that he can open doors with his hands, and eat normally, as long as he avoids eating foods with peanuts. His therapy is far from over but he feels freed.

“I am able to eat. to do everything because of the medication and the specialized therapy. yes, it saved my life,” he says.

Paul is a little more than three months into his therapy. His parents say they can already see glimpses of the normal Paul within. they can move about the house freely, without worrying that if they are on his left, he will become panicked.

“He is less afraid of the fear the OCD creates in him. says his father David. “We’re even starting to be a bit more relaxed,” says his mother Julie.

“I was convinced nobody could help me. Nobody I saw before no one could help me,” says Paul, who hopes to return to school in September.

“I think if I keep doing all my exposures and homework, I will keep getting better and better… I want to enjoy life.”

Pat Duckett deals with death every day. she works for an NHS trust in which 1,600 people die every year, and she has seen many things. One of her more extreme encounters was when one man asked her to hold a stethoscope over the chest of his dead father while he chanted prayers. “He thought he would wake up and they could take him home in the car. the son said: ‘We’ve got the engine running outside. Wake up, Daddy. It’s time to go.’ I had to say: ‘I’m afraid his heart isn’t going to start beating, because he died last week.”

As anyone who has ever been suddenly bereaved knows, the death of a loved one can have an explosive effect – following the immediate trauma of loss comes a prolonged period of shock, and questions: “When did this illness start?”, “How could someone die so suddenly?”, or just “Why?”

Pat is a bereavement co-ordinator for the North West London hospitals NHS trust, a major acute trust. in the majority of cases, relatives find a way of coming to terms with their loss. but for a significant minority, making sense of what has happened is extremely difficult. “Most people will grieve and be perfectly OK, with support from family,” she says. “They get over it. Grief is, after all, a natural process. but sometimes when someone dies, it is literally incomprehensible to their loved ones.”

Her role is to help the bereaved edge closer to some form of closure. she is not a counsellor, but rather a liaison officer between a hospital and families in mourning. more often than not, her work involves guiding them through medical notes, enabling them to have a better medical understanding of the events that led to the death.

“If a patient is in intensive care, then typically the family is bombarded with a lot of medical information at the time,” she explains. “But then, when the person dies, that’s it.”

I am speaking to Pat in one of her consulting rooms at Northwick Park hospital (whose trust has one of the lowest mortality rates in the country). the room is designed to exude serenity – sunshine cascades through the venetian blinds; a pot of brightly coloured synthetic flowers sits in one corner, and there is a box of tissues on a coffee table. in this room she arranges 40 meetings a month with bereaved relatives, taking them through folder after folder of medical notes, jargon-heavy texts that provide a medical synopsis of the final chapter in someone’s life.

“I’ll explain to them, ‘So at this point, there was an X-ray, which indicated this. then, here we can see that they took a blood test.’” a frequent reaction to revisiting the past in this way is that family members will suddenly realise just how ill their relative was, she says. “In hospital, when you are losing a loved one, it’s very intense. You don’t always listen to what medical people are telling you.”

While guiding relatives through medical notes is a central part of what she does, her role as bereavement co-ordinator involves other strands: she advises families of other services, including counselling, and helps organise funerals when, for instance, families are too overwhelmed to cope, or when elderly patients die in hospital with no apparent family.

Not all bereaved families wish to take up Pat’s services: the majority navigate their own way through grief. but in some cases, that is too hard a path to travel alone. a lot of Pat’s time is spent talking to couples whose children have died unexpectedly, or to families who have suffered a traumatic death, or to those who find it too difficult to accept that someone has died.

“Through this job, I’ve learned the true meaning of being ‘grief-stricken’,” she says. “I’ve encountered a couple who have broken up after losing their child. Grief tore them apart. I’ve dealt with a wife who insisted on seeing her husband in the mortuary for several consecutive days. she arrived each day to dress him ritualistically. she even shaved him.”

Everyone responds differently to death. One of Pat’s mantras is “nothing is wrong in grief”. she almost always honours requests from bereaved relatives, however unusual. a common wish is to touch the body of a loved one: hold their hands, or kiss their foreheads or even wash their face. One woman asked Pat if she could help her retrace the journey her 15-year-old daughter’s body made from the hospital to the mortuary, after she died from a very protracted illness. she then wanted to see where she had been blessed in the mortuary. “And there’s nothing wrong with that,” she says.

While a lot of NHS trusts can provide some form of bereavement advice, the services on offer can vary greatly and there is no standard model, she says. Her role as bereavement co-ordinator was conceived two years ago, the idea behind it partly stemming from the fact that a significant number of complaints to any hospital trust are to do with deaths: according to Pat, 60% of deaths take place in hospital, and 60% of complaints to the Care Quality Commission, the organisation that monitors hospital care, are about bereavement.

The thought was that if bereaved people could be empowered with facts about someone’s death, they might be less inclined to make a formal complaint, and be able to find closure more quickly. Neonatal deaths are often a focus of her work: she will sit down with parents and discuss the, often not wholly knowable reasons why a baby has died before even leaving hospital.

“If people get stuck into protracted correspondence with a hospital, it doesn’t help with their grief,” she says. “What helps is having knowledge about the events that led to that death, so they get a better understanding and proper support, bringing them out of the loop so their grief can progress.” (In the past two years, the trust has seen a drop in bereavement complaints of 48%.)

Pat’s theory is that grief itself has evolved in the last century. “We have lost this pattern of sharing the death in the family. in Victorian times, when someone died, the neighbours would all come in and see the dead person in the parlour. And the family would pay their respects up close with the body.”

She experienced her first sense of true bereavement when her grandmother died. but this was infused with a sense of happiness: “She died at home, after falling asleep with her hairnet on, her brandy glass empty, a smile on her face. it was a kind death.”

If death had dealt a kind hand to her grandmother, it was much crueller to Pat’s father seven years ago. a serious bowel condition meant his gut used to “twist”, resulting in emergency procedures. Finally, he came to Northwick Park hospital (where Pat is now based) to have most of his colon removed. He lost six stone, and was a shadow of his former self. When he succumbed to organ failure it was a release for everyone, she says.

Pat was keen that her own daughter, then aged six, was involved in his death. she encouraged her to “do her homework on her grandpa’s deathbed” then attend his funeral. “I wanted her to see that death is a process that is part of life,” she says.

How has her job changed her view of death? How does she feel about her own death now? “I’m not afraid of it. In fact, I revere death. that might sound odd, but I see it as something phenomenal. the effects of it on families are so powerful, how it casts such a long shadow over so many people. Being involved in that, I feel quite honoured.”

When she started her job, Pat admits she brimmed with enthusiasm, but the work took its toll. she initially saw up to three families a day, but got exhausted – physically not emotionally. she developed intense headaches and decided on a strategy of limiting her meetings to just one or two a day.

The overriding feeling that she has about her job, however, is that she is privileged. every day she finds herself embroiled in the most emotionally profound experiences of people’s lives and for that, she says, she feels deeply honoured.

“I always acknowledge the intimacy of strangers. the people I meet tell me the most intimate things and I’ve never met them before in their life.”

My husband James was 59 when he was admitted to hospital on 10 December last year. He had been feeling lethargic but was not in pain. He was working on the Thursday when he told me when he went to the toilet that he found he had passed black motions.

Doctors said internal bleeding and an enlarged liver could be symptoms of alcohol-related liver disease. I knew that could not be the case with James: he drank four or five times a week, but was certainly not an alcoholic.

He was given blood transfusions and heavily sedated. He really didn’t know what was going on. As the days proceeded, he seemed increasingly agitated, but the doctors put this down to withdrawal from cigarettes and alcohol. Ten days after he was admitted, we received the phone call: he had had a cardiac arrest. By the time we arrived, he had died.

The shock for me, and my son and daughter, was immense. He had died so suddenly. we all wanted to get to the bottom of what had happened. that was when we met Pat. we went through the coroner’s report and began to get some answers.

Pat explained that while James had symptoms exactly like those of liver disease, the breakdown of his liver had been secondary to the main cause of his illness: lung cancer. He’d had the worst form, “small cell” lung cancer, which had spread rapidly from the lungs to his liver and renal glands.

Pat explained the detail calmly and with sympathy. we had so many questions but the doctors were often too busy to answer. I had three more sessions with Pat, which were more informal, less medical. Meeting her helped enormously. I had been angry about the misdiagnosis of alcohol-related disease, but came to understand why it was made, and was more accepting of James’s death.

Even i if you are aware of the symptoms of high blood pressure, it can be hard to diagnose without regular checks of your blood pressure. In many cases, there are no warnings signs of hypertension until it has reached a dangerous level, and untreated it can be a serious problem. Don’t loose heart though, there are recognizable symptoms and below we discuss a few of these.

Your doctor might tell you that you have prehypertension, a condition that does not usually have any specific symptoms. this simply means that your blood pressure is slightly higher than normal but is not at the level of hypertension. while no one likes to hear bad news from the doctor, this can actually be considered good news, because if you are diagnosed with prehypertension, you are still able to do something about it before it becomes more serious. your physician will probably recommend medicine or lifestyle changes and there are quite a few different natural methods that you can use to reduce your blood pressure before you begin to show more symptoms.

Serious consequences such as kidney disease and diabetes can occur for those who do not treat their high blood pressure. unfortunately by the time someone experiences these symptoms they have developed some of these serious conditions, since they didn’t get their blood pressure checked regularly. it is possible that your first symptoms of high blood pressure are in fact signs of kidney disease or diabetes.

If you are experiencing problems with your kidneys you may feel the frequent need to urinate or blood in your urine. Sudden weight loss and being more thirsty than normal are both signs of diabetes. be sure to have any of these symptoms checked out since they can be a sign of something serious regardless if it is high blood pressure or not. High Blood pressure can cause ED that can lead to mental stress and should be treated.

Another symptom of high blood pressure can sometimes be anemia. When your body stops producing a sufficient amount of red blood cells is the condition referred to as anemia. In some cases, a disorder of kidneys related to anemia, is brought on by high blood pressure. Shortness of breath, depression or unusual fatigue are all symptoms of anemia. Medical tests are required to diagnose mild anemia. this is one problem that can develop if you have high blood pressure, so if you have any symptoms that might indicate anemia you should see your doctor.

While, hypertension or blood pressure is a common yet serious disease, it is even more so since it is hard to identify in its early stages. of course, if you take the time to get your blood pressure checked, it’s very easy to find out if you have hypertension, but too many people neglect to do this. whether or not you have the type of symptoms mentioned above, getting your blood pressure checked is a simple way to safeguard your health.

Hello, I've been having some small problem with my stomach lately, I think it's my stomach because I feel discomfort in that area, but it could be related to the esophagus or the pancreas.

The symptoms are strange, they are not really obvious. the most noticeable one it discomfort in the stomach area, as if slightly bloated, a little nervous. I also burp a lot, more than normally.

Sometimes I have chest pain on the left side in random areas close to the surface, it's not the heart I've checked it out and all looked good, but it's pain radiating strangely from the esophagus to the chest (this im sure of ill spare you the details).

And then there's a psychological effect I feel, I sometimes feel dull minded, sometimes I catch myself making slight misconceptions (for instance catch myself thinking it's sunday when it's tuesday, or im in a group of people seating and talking I feel like someones missing and then I tell myself no everyone's here) not big problem but I feel sometimes like im not totally there not fully conscious and sometimes anxious.

I also feel as if I'm lacking in energy but I can go about doing exercise with no prob.

What could this be? Hypoglycemia? Ive had blood tests, all looks good except the antibodies of the chest area were quite elevated.

In summary, discomfort in the stomach area, burping, some chest pains and slight mental effect.

Appreciate any guidance/help. I'm already consulting my MD. perhaps it's an allergic reaction to something?

These women. these women I love. Fiercely. I have never laid hands on any of these women. Well, about 95% of them. they are my sisters. I only need utter type a few words and they understand EXACTLY what I’m trying to convey. they are passionate, fierce, hilarious. they fight, rally, grieve. they are my fellow Momcologists. We have all have a sick and twisted sense of humor. it is a must have in this world. Never, would we have crossed paths, had it not been for Childhood Cancer. while NOTHING is worth what Gregory and our family have been/are going through, these women make it a safe place to fall. a safe place to show vulnerability. a safe place to share fears and realities. there are moments when my heart is overflowing with the emotions I feel for these women. Every. Last. One. Of. Them. I dream of a time in my life where I can travel, in a luxurious RV and visit these women. See them in the flesh, *fiercely squeeze* them, sit silently in their presence while holding their hand. Knowing we don’t need to speak a word. Knowing that THEY know my heart. I love you, ladies. So much, it often brings me to tears. Thank you for being. Things have been crazy around here. Day in and day out. Anxiously waiting for cold/flu season to be DONE! Anxiously waiting to let my guard down. then…….. We have a goal to get Gregory off his Prednisone by our annual follow-up in June. He is currently at 5mg every other day. in two weeks we go to 4mg. Decreasing by 1mg every two weeks. the thought is that IF he should show any treatable GVH symptoms, it will coincide with his annual follow-up and SCCA will be there to advise us on how to proceed. He has done so well since we upped his Prednisone and switched from Tacrolimus to Sirolimus. I must admit to being nervous about tapering his Sirolimus. I’ve believe, for quite some time now, that the Sirolimus is what has done the trick for Gregory. only time will tell and we will cross that bridge, when we get there. He spent three weeks in March spiking intermittent fevers. Daily. at least twice a day. He had little to no other symptoms. three ER visits and a two night admit. All testing came back negative. my gut tells me it was GVH. His doc also mentioned that it could have been related to his adrenal insufficiency. what ever the cause, it does not alleviate the stress and worry that comes with the symptoms. my mind went crazy trying to figure out what was going on. two of my bigger fears kept racing through my mind. Epstein Bar Virus (Mono) and Cytomegalovirus (CMV). He is, and will be, extremely susceptible to these nasty illnesses. the best plan of action for Gregory, is to catch them early and treat them aggressively, when they can be treated. I’ve heard about Chicken Pox running around town. Another deadly virus for Gregory. Which has resulted in keeping him and I extremely isolated. Yes, you could argue that it is better for him to be as “normal” as can be. Yet, here’s my argument: He is still pretty young. I think it is closer to “normal” to be isolated at home, than exposed to whoknowswhat and wind up being admitted for days on end. I also DO NOT want to overly stress his body. We have been decreasing his Prednisone and I want his body to accept these changes in the best possible circumstances. We have the luxury of being able to make these decisions. in the big picture, Gregory has done amazingly well. He’s managed to escape some pretty nasty illnesses that are common in post BMT patients. No big respiratory illnesses and not even a whiff of pneumonia. THAT is my take home. THAT is my “proof” to keep doing what we are doing. We are getting ready to enter the crazy time of the school year. Once Spring break is over the balance of the school year flies by in a flurry of activity. Before you know it, we will be slathering on the sunscreen and sipping beverages, water side. CANNOT WAIT! Gregory returns for his SECOND annual post-BMT follow-up on June 20. We meet with the endocrinologist in July to seriously discuss the addition of Growth Hormone. Dreading THAT appointment. We return to SCCA this year, knowing that Gregory will not have a bone marrow aspirate. while I’m thrilled that he won’t need to add another scar to his backside, the security of KNOWING the condition of his marrow will not be there. He will only have an aspirate if his blood work indicates something wonky is going on. *sigh* Big leap of faith. Gregory’s two lower front teeth are incredibly loose. the new adult teeth are slowly protruding behind them. it takes GREAT restraint to not reach in there and yank those teeth out. He is nervous to lose them and I cannot wait to see a toothless grin. it is evidence that something NORMAL is going on in his body. We have no clue what those adult teeth will look like. Yet, they are making their debut ON TIME! Gregory will be six at the end of June. Oh yeah. GREGORY IS DAY TIME POTTY TRAINED! 100% He was reluctant to begin with. after all, who wants to be bothered with having to STOP what you are doing and use a toilet, when we have a very serviceable alternative called a diaper? He got over that in a couple days and has been rock solid. even at his first day of school with skivies.
