Enterohemorrhagic E. coli of the serotype O157:H7 is the major cause of food borne disease, responsible for more than 76,000 cases alone in the U.S., with a potentially lethal outcome. This included the study of three widely publicized food-associated outbreaks of E. coli O157:H7 infections in 2006 that captured the attention of the U.S. Congress as well as the public health, forensic and lay communities, such as the severe 2006 multistate outbreak from ingested spinach that swept through 26 states. This information allowed the team to develop a high-resolution phylogenomic framework and follow the dynamics of pathogenome evolution at a high level of phylogenetic accuracy and resolution. Published in the prestigious international journal, Proceedings of the National Academy of Sciences, the research using phylogenetic comparison of more than 30 E. coli O157:H7 isolates from different outbreak related sources to investigate outbreak-specific genome characteristics of this food borne pathogen.Funding from the National Institute of Allergy and Infectious Diseases, National Institutes of Health, Department of Health and Human Services, contributed in part to this milestone analysis to help scientists and the medical community better trace and prepare for future outbreaks. The team included Drs. mark Eppinger and Jacques Ravel from the Institute for Genome Sciences (IGS) at the University of Maryland School of Medicine. Dr. Eppinger, Research Associate and Dr. Ravel, Associate Director, Genomics at IGS, are experts in microbial genomics and forensics. they sequenced and analyzed several bacterial E. coli O157:H7 genomes that allowed detailed insights into the outbreak during the time course of an outbreak. Just as DNA is revolutionizing criminal investigations, it can be used to understand dynamics during an outbreak through the use of evolutionary theory and genomics analysis. The rapid emergence of Escherichia coli O157:H7 from an unknown strain in 1982 to the dominant hemorrhagic E. coli serotype in the United States and the cause of widespread outbreaks of human foodborne illness highlights a need to critically evaluate the extent to which genomic plasticity of this important enteric pathogen contributes to its diseases severity. Infected patients present with a range of gastrointestinal morbidities such as severe abdominal cramping and bloody diarrhea. An estimated 15-20% of people infected with E. coli O157:H7 present with indications severe enough to require hospitalization. In such cases, symptoms may progress to hemolytic uremic syndrome renal failure (HUS), hemorrhagic colitis (HC) and central nervous system (CNS) failure with potentially lethal outcomes. yet, little is known about the genomic diversity that exists among extant E. coli O157:H7 populations or how various genotypes of this pathogen relate to development and severity of human disease. The work showed that outbreak-specific clades could be identified by unique mutations they have accumulated in their genomes, even among strains derived from the same outbreak. these genomic architecture signatures will be useful for understanding future disease events involving E. coli O157:H7, as this type of DNA fingerprinting can be used to characterize outbreaks derived strains , and ultimately for linking with high accuracy human E. coli O157:H7 infection to its source. While the current molecular assays used in public health microbiology laboratories may be adequate for routine surveillance and identification of E. coli O157:H7, but they often lack the discriminatory power needed to resolve its genetically homogenous population structure.Dr. Eppinger, the lead author on the project, said: ?The sequencing of additional E. coli O157:H7 bacteria was crucial in identifying subtle, yet important sequence and structural differences in this genetically very homogenous lineage to not only discriminate among simultaneously occurring outbreaks caused by different bacterial genotypes but also within outbreaks with a previously unprecedented level of resolution. Our data clearly suggest studying microbial outbreak populations rather than relying on single archetypal reference outbreak strain.?E. coli O157:H7 continues to be a significant public health threat and these data should prove useful for the development of a refined phylogenomic framework for forensic, diagnostic and epidemiological studies in order to better prepare for future outbreaks and better risk assessment in response to novel and emerging E. coli O157:H7 resistance and virulence phenotypes and ultimately to prevent or control future infectious disease outbreaks.

TWO STUDENTS at a Leyton school have been diagnosed with tuberculosis (TB).

The first confirmed case at Lammas School in Seymour Road, Leyton, was at the end of March, and the second in April.

According to the Health Protection Agency (HPA), which deals with infectious diseases, the cases are not thought to be linked.

The two pupils concerned, both in Year 10, are undergoing a six-month treatment programme, but their conditions are no longer contagious.

Letters were sent out to Year 10 students informing them and their parents after the first case and then to the whole school following the second diagnosis.

The agency has also identified pupils and staff who had the greatest contact with the patients and is offering them screening for TB, as a precautionary measure.

Dr Rebecca Cordery, a consultant from the HPA’s North East & North Central London Health Protection Unit, said: “Tuberculosis (TB) is a disease that typically requires close, prolonged and frequent contact before transmission occurs.

“Because of this, the greatest risk of it spreading is to people who live in the same household as a person with the infectious form of the disease.

“The risk to other contacts, including those in a school setting, is low.”

It enters the bloodstream and infect red blood cells. The parasites multiply within red blood cells, which then rupture within 48 to 72 hours, infecting red blood cells and thus result in the signs and symptoms of malaria. Some signs of malaria caused by the release of merozoites into the bloodstream, the anemia caused by destruction of red blood cells, and the problems caused by large amounts of free hemoglobin released from red blood cells in circulaion break.

Malaria is the fifth cause of death from infectious diseases in the world after respiratory infections, HIV / AIDS, tuberculosis and diarrhea, and the second in Africa after HIV / AIDS. Malaria can be transmitted at birth from a mother with her baby and blood transfusion.

What are the signs and symptoms of malaria?

Signs and symptoms of malaria are somewhat similar to flu. Common signs of malaria include fever, chills, nausea, headache and fatigue. Infection with the parasite P. Plasmodium falciparum is usually serious and can be life threatening. The severity of malaria may also vary depending on the general health and type of malaria parasites.

How the diagnosis of malaria?

Malaria Diagnosis involves the identification of malaria parasites or antigens company / product in the blood of patients. Although this seems simple, the effectiveness of malaria diagnosis is subject to many factors. Malaria diagnosis is confirmed by blood tests and can be divided into microscopic tests and microscopic.Diagnosis of malaria begins with a thorough personal and family medical history, including symptoms and travel history, complete physical examination. last trip to the subtropical and tropical world is an important indication of suspected diagnosis of malaria could increase.

Malaria diagnosis including blood tests to check that the plasmodia parasites that cause malaria. A series of tests may be needed to definitively diagnose malaria.

An increase in children’s food allergies is not just the result of hyperconscious parents but a result of the sanitary times we’re living in, doctors say.

Dr. Edward McLaughlin, an allergist with offices in DeLand and Ormond Beach, reports a “dramatic increase” in children’s food allergies among his cases. and both he and a University of Florida pediatric allergist believe it’s the result of how the human immune system has responded to the dearth of bacteria and infectious diseases compared to how it used to be.

“We live in such a hygienic society, our immune system is responding differently,” said McLaughlin, pointing to clean drinking water, bacterial wipes and immunizations as key developments in more widespread allergies.

The Centers for Disease Control and Prevention found food allergies have increased 18 percent even as recently as the period between 1997 and 2007, with milk, eggs, peanuts, tree nuts, fish, shellfish, soy and wheat accounting for 90 percent of all the food allergies. In 2007, the CDC found 3.9 percent of all children younger than 18 had food or digestive allergies in the previous 12 months.

Dr. Shih-Wen Huang, a professor of pediatrics at the University of Florida, likened the body’s immune system to two arms. as one gets weaker from disuse — the one that fights bacterial invaders — the other has gotten stronger. the one getting stronger originally developed to fight parasites that mimic healthy cells’ function.

Lacking any parasites to fight, however, this stronger arm can be triggered into action by certain substances, such as peanuts, in a life-threatening way. the allergic person has a buildup of the allergic antibody, Immunoglobulin E, in the body. Higher IgE reflects the potential escalation of allergic symptoms.

“Few (food allergies) lead to severe problems,” Huang said. “But the peanut allergy is extremely dangerous in some cases. It is currently the leading food that often caused the most severe reaction of allergy called anaphylaxis nationwide.”

Huang said the rise in allergy cases has prompted changes in the way pregnant women are counseled about what to eat.

McLaughlin said his specialty is also revising advice given to parents of allergy-prone children about when to introduce certain foods. It used to be that allergy-triggering foods were withheld until the child reached 2 years old. but that’s changing.

“Now, earlier introduction actually reduces food allergies,” McLaughlin said.

“Nut-free” classrooms. Hand-washing and mouth-rinsing routines twice a day. a peanut-sniffing dog.

Carie Starkey joined parents picketing Edgewater Public School recently to protest some measures taken to protect a first-grader whose allergy to nuts is so severe she could die if not treated quickly for a reaction. Starkey and other parents say the procedures cut into class time and interfer with their children’s education.

Tracey Bailey kept her daughter — the girl with the severe nut allergy — home the day of the demonstration to shield her from the controversy. but she and her husband are just as concerned as the protesters about what’s happening at school, even though some of the pickets’ claims were based on what school officials now say was misinformation.

“We 100 percent, hands-down want to work with the parents to work this out. We want to do what is best for all kids at the school, not just our daughter, ” Bailey said.

Bailey, who declined to confirm her daughter’s first name for publication, said touching nuts or nut products irritates the girl’s skin. Smelling them can trigger an asthma attack. Eating them carries the danger of causing hives and breathing problems that could be deadly if not treated quickly, Bailey said.

To protect their daughter, the Baileys have posted a sign on the front door of their Edgewater home barring visitors who’ve had recent contact with nuts from entering. They’ve also made sure a plan is in place to keep her safe at school, as required by federal law and regulations.

“We have nothing against this little girl being at school, but we don’t want it to take away from our children’s education,” said Starkey, whose daughter is in the same class as Bailey’s girl.

Through signs and fliers, the protesters objected to a loss of class time for their children because of a requirement they wash their hands and faces and rinse their mouths before class and after lunch.

Other issues raised by the protesters concerned a ban on class snacks and holiday celebrations, and what they believed was a plan to use Clorox wipes to clean children’s hands and faces and preferential treatment for Bailey’s daughter in the school clinic.

“The truth is we were not aware of 98 percent of what they’re protesting,” Bailey said. “If these things are truly happening, we are just as upset about them as they are.”

School officials confirmed nuts and nut products are banned from the two classrooms where Bailey’s daughter receives daily instruction. Children who bring lunches from home must leave them on a cart outside the classroom for delivery to the cafeteria, Superintendent Margaret Smith said, but they can bring peanut butter sandwiches or other nut products.

Bailey’s daughter eats her home-packed lunch in a space off the classroom in the company of a teacher’s aide so she isn’t exposed to any nut products in the cafeteria.

Clorox wipes are used to clean desks at times, school district spokeswoman Nancy Wait said, but not children’s faces and hands.

Smith said the Edgewater first-grade teachers agreed as a group not to have classroom snacks because of the children’s early lunch time. They also decided to celebrate holidays with arts-and-crafts projects and other learning activities rather than food, she said, and not to invite parent volunteers into the classroom.

Smith said those decisions were prompted by a need to keep focused on instruction without interruptions and had nothing to do with Bailey’s daughter’s allergy. she said the order in which children are treated in the school clinic is decided by the school nurse based on each child’s condition at the time.

Starkey said the twice-a-day washing and rinsing routines take at least 20 minutes out of class time.

“What if a child is struggling and they’re losing the academic time, who’s going to take up the slack?” she asked.

Wait said Area Superintendent Annette Martinson will meet soon with the Edgewater school’s administrators to see if there’s a way to cut back on the time the washing routine takes.

Starkey hopes school officials will meet with parents as well to hear and address their concerns, and Bailey is willing to be part of that. the superintendent said information will be shared with parents as the review of the situation is completed.

The education director for the Virginia-based Food Allergy & Anaphylaxis Network sees that as a good sign.

“It’s definitely helpful when everyone can work together for the best interest of all our students at school,” Eleanor Garrow said in a telephone interview. “Education is the key, and there are so many people who aren’t educated on food allergies” and the serious — sometimes fatal — complications they can cause.

Meanwhile, school officials are waiting for results from a March 11 visit to the Edgewater school by a black Labrador retriever trained to sniff out peanut residue. at no cost to the school district, the Baileys arranged for the dog to inspect the school when no children were present to see if it could pinpoint anything that’s causing what appear to be allergic reactions that have sent their daughter to the school clinic several times.

The superintendent said the school was cleaned carefully after the dog left to ensure children with animal allergies won’t be affected when they return to classes Monday after Spring Break.

ScienceDaily (Feb. 1, 2011) — Hepatitis C is a leading cause of illness and death for individuals infected with both HIV and hepatitis C. Recent reports from around the world demonstrate that hepatitis C is emerging as a sexually transmitted infection among HIV-infected men who do not inject drugs. however, many HIV-infected men do not receive continued screening for hepatitis C throughout their HIV care. Hepatitis C symptoms often do not manifest themselves until the later stages of the illness, so people are not as likely to know that they have become infected and hence need further testing and treatment.

Researchers examined the role of later acquisition of hepatitis C in HIV patients in a new study published in Clinical Infectious Diseases, which is currently available online.

In the study, 1,800 HIV-infected men had an initial negative hepatitis C blood test result, with at least one subsequent test. At the time of their initial negative hepatitis C results, 94 percent were receiving antiretroviral therapy for HIV and 6 percent reported current or prior injection drug use.

Ultimately, 36 patients were subsequently diagnosed with hepatitis C. of those, 25 percent reported an injection drug use history, although 75 percent reported no current or previous injection drug use.

"Screening HIV-infected patients for hepatitis C only once upon entry into HIV care is not sufficient," according to study author Lynn E. Taylor, MD, of Brown University in Providence, R. I. "The standard of care needs to change. HIV-infected persons should have access to ongoing screening for hepatitis C. Doctors and patients may not be aware of or freely discuss all risk behaviors that may lead to hepatitis C infection. these behaviors are often stigmatized. Patients may not feel comfortable discussing these risk factors nor may they be aware of all the ways in which hepatitis C may be transmitted via blood."

GREEN VALLEY, AZ – Experts say Valley Fever is not only often misunderstood, it’s also misdiagnosed. A study by the University of Arizona shows that two-thirds of patients with the infection had it misdiagnosed in Arizona.

Part of the reason is the symptoms can be confused with influenza.

Fatigue, coughing, chest pains, fever, rashes, headaches and joint aches are all signs a patient may have Valley Fever.

While the flu typically lasts for a week, Valley fever can last for weeks, months, even a year, said Clarisse Tsang, an epidemiologist who works with infectious diseases.

Tsang said if the above-mentioned symptoms are persistent, ask a doctor to test you for Valley fever.

“Doctors, especially ones who didn’t get their training in Arizona, may not be familiar with the disease,” Tsang said.

A simple blood test will show if the fungus is in the person’s blood stream, said Tsang, who specializes in Valley fever.

Anti fungal medication, not antibiotics, will treat the disease, she added.

Valley fever peaks between the months of June and August and between the mild winter months of October and December.

In 2010, more than 10,000 cases were reported in Arizona.

Valley fever can not be transmitted from person to person.

The Arizona Department of Health Services produced a video , where you can learn more about Valley Fever.

The trials indicate that treatment helps “when the diagnosis is certain,” said Dr. Wilbert Mason, an infectious diseases specialist at Children’s Hospital Los Angeles who was not involved in the study. though the reduction of symptoms was marginal, he said, treated children “were less likely to have a relapse or to have persistent signs of infection at the end of therapy.”

The findings “confirm the current guidelines” for treating children under the age of 2, said Dr. Al Lieberthal of the University of Southern California’s Keck School of Medicine, who is in charge of the American Academy of Pediatrics committee that prepared the guidelines. they will be revised in light of the new studies, however, to tighten the definition of ear infections, known formally as acute otitis media, he added.

At least a dozen previous clinical trials have studied the use of antibiotics in ear infections, but these have given mixed results because of poor study design and poor diagnosis of the subjects’ conditions, according to Dr. Anthony S. Fauci, director of the National Institute of Allergy and Infectious Diseases.

“In many cases, (the children) had something that looked like otitis media but really wasn’t,” he said. the new studies “had really precise criteria to be eligible for enrollment,” which makes them “substantial studies.”

In one of the studies, Dr. Alejandro Hoberman of the University of Pittsburgh School of Medicine and his colleagues studied 291 children ages 6 months to 23 months with stringently defined acute otitis media. half were given the antibiotic amoxicillin-clavulanate for 10 days and half a placebo.
