On Thanksgiving Day last year, Lee Sheppard ended up in the emergency room suffering from kidney failure. he had been diagnosed many years earlier with polycystic kidney disease, a hereditary condition that had killed his father and grandfather. he had lived with the disease for years with few symptoms until that day.

Doctors had told him his kidneys would only last about a year in 2010. but large, dark cysts caused severe pain for Sheppard and brought concern about cancer. Doctors removed both of his kidneys, which were functioning at less than 20 percent of normal.

Following the surgery, Sheppard began dialysis and his wife began to ponder giving him her kidney.

“It was amazing how many lovely people came out of the woodwork and volunteered their kidneys. It’s not something that’s easy to say ‘thank you’ to when somebody offers you such a great gift,” Pam Sheppard said. “I felt so privileged that I became the best match.”

“How overwhelmingly grateful I am for my wife for making such a sacrifice. we feel so liberated and energized for what the future holds, so, we’re deeply grateful,” Lee Sheppard said.

“we already enjoyed a rich love together, but this last year — which has been a journey — has reinforced it in such a way that it’s hard to put a finger on it,” Lee Sheppard said. “The transplant center team is very, very profession and does everything it can to make sure nothing goes wrong … but also (shows) real concern, really compassionate.”

Dr. James Stinson recalled the first such donation — 28 years ago — when a sister gave a kidney to her brother.

“It freed him from dialysis and gave him many years of life that he would not otherwise have had,” he said.

Today, the Intermountain Transplant Program performs more than 135 heart, kidney, liver and pancreas transplants each year.

Dr. Diane Alonso, another surgeon with the Intermountain Transplant Team, recognized the courage and generosity of the human spirit.

“It cements that living donation is a vial source of organs not only in this state but in the country, but particularly in this state,” she said. “At our institution, 50 percent of our kidney donations are from live donors.”

Hypertension can sometimes confuse both the patient and the doctor for a couple of reasons. one, the physical symptoms of high blood pressure can vary quite widely. which leads us to the second reason – these symptoms can be synonymous to other illnesses. That’s why, in dealing with any illness, a thorough examination of the patient, including checking blood pressure, is essential.

High blood pressure is a scapegoat for many a doctor, even if he/she is diagnosing a patient due to a totally unrelated condition. Many organs, conversely, are affected by hypertension if left undiagnosed, such as the kidneys, liver and the heart. It could lead to vision problems and strokes in extreme cases.

Some of the most common physical symptoms of high blood pressure include headaches, blurred vision, convulsions, clinical depression, labored walking or exertion and extreme excitement or giddiness.

Difficulty in even minor physical exertion and unusual tiredness may be signs of cardiac damage from high blood pressure. other signs that may manifest themselves include unusually deep breathing and sweating. This is a serious situation and calls for immediate medical attention.

If you experience pain while urinating or need to go more often than usual, this means your kidneys may have been damaged. a pasty complexion may be observed, caused by a lack of electrolytes and poor hydration.

High blood pressure can affect vision causing lesions in the ocular region. Lack of immediate attention could cause a total loss of vision.

But again, these symptoms are common to so many other illnesses that many doctors miss out on the main cause. Doctors need to look for underlying causes that may be related to high blood pressure. Comprehensive testing may be necessary to zero in on the real problem.

In the event of organ damage, it is common for doctors to fail to spot the real underlying cause, that being hypertension, or high blood pressure. Blood pressure checks are done for many illnesses in order to avoid false diagnosis.

Diabetes and hypertension share many common symptoms if you come to think of it. In fact, a lot of them resemble those of your average drunkard hauled in for the night after creating a ruckus! This has led to many a physician failing to notice hypertension or to treat it just because he/she jumped to the conclusion that his/her patient had one drink too many the night before.

Myriad health issues can sprout from the wide range of physical symptoms of high blood pressure being left unnoticed. So it is crystal clear that seeing a doctor once you experience the above symptoms should be your first and only priority.

Kidney is one of the most vital organs in the human body and plays a major role in filtering and removing waste products from the body. if the kidneys fail to work properly, it causes various health problems. Acute kidney failure and chronic kidney failure are the main two types of kidney failure. Chronic kidney failure, also called as chronic renal failure, allows building up waste products and fluids in the body in dangerous levels.
Generalized weakness

People, who suffer from chronic renal failure, may experience generalized weakness. In normal situation, the kidneys produce a protein hormone called erythropoietin (EPO). Its function is to stimulate bone marrow to increase red blood cell count. if the kidneys are failed, the production of erythropoietin becomes insufficient to stimulate bone marrow. it leads to the decrease in the red blood cell count resulting generalized weakness.

Anemia

Anemia is also caused by the lower level of erythropoietin due to a kidney failure. Regulating red blood cell production is the function of EPO. as chronic kidney failure decreases the red blood cell count, the patient becomes anemic. Loss of nutrients such as iron and folic acid that are important components of red blood cells may also cause anemia.

Loss of appetite

Chronic renal failure may cause a constant foul taste in the mouth of the patient. This can lead to the loss of appetite.

Bone disease

Chronic kidney failure symptoms include several bone diseases. Regulating the balance of substances such as calcium, vitamin D and phosphate are one of the main functions of the kidney. Failure of the kidney may result in abnormal levels of these substances that may lead to bone diseases. The bones can become thin and weak that may be associated with pain. Patients, who suffer from renal bone disease, are at a higher risk of bone fractures and osteoporosis.

Swelling

Swelling of the lower limbs and abdomen is commonly associated with chronic kidney failure symptoms. The failure of kidney causes buildup of fluid in the body. Increased fluid in the blood vessels may accumulate in various spaces of the body such as tissues in the lower limbs and abdomen with ensuring swelling.

Depression

Chronic kidney failure may affect the nervous system of the people in some cases. The patient may have a depressed mood. Toxic metabolite buildup due to kidney failure can result in general malaise, altered mood, weakness and decreased mental function.

Uremia buildup

Uremia is one of the toxins found in the blood stream. Normal functioning kidneys can remove this toxin from the body. if the kidneys fail, uremia may accumulate in the body with many effects. Uremia is toxic to the platelet cells in the blood and platelet cell dysfunction may make the body to bruise and bleed. Increased uremia can inflame the pericardium with symptoms of discomfort and chest pain. Uremia toxin also affects the brain and nervous system that may cause various symptoms such as lethargy, decreased mental acuity, loss of reflexes, impaired sensation and even coma. Constant itching also occurs due to uremia buildup.

Chronic kidney failure symptoms may often become life-threatening and it is advisable to consult a doctor when any of such symptoms are exhibited.

Dear Abby: Like many other young adults, I was too busy establishing a career during my 20s and early 30s to care much about diet and exercise. I felt healthy, so I saw no need to change my lifestyle. My doctor had told me my blood pressure was elevated during a number of my yearly physical exams, but I didn’t ask any questions and took no action.

Then one morning, I walked into my doctor’s office complaining of a severe headache and nausea. I was sent to the hospital with a dangerously high blood pressure reading. after just a few tests, I was told I had chronic kidney disease (CKD).

Even though it can be silent and cause no symptoms, high blood pressure should not be ignored. it is a leading cause of kidney disease, and because I didn’t pay attention, my kidneys began to shut down. Abby, please tell your readers who are at risk for chronic kidney disease (and that’s anyone with high blood pressure, diabetes or a family history of CKD) to check how their kidneys are functioning. I found out — too late — how important it is.

Dear Aziza: of course I will pass on your warning. according to the National Kidney Foundation, more than 26 million adults and thousands of American children have chronic kidney disease — and most of them don’t know they have it. in addition, millions of people who have diabetes, hypertension and other diseases are unaware that they too are at risk for developing it.

Dear Abby: One day, after dropping my son “Wyatt” off at day care, I looked back and saw an older kid push him and take away my son’s toy. I was furious. What happened next restored my faith and softened my heart. a little girl walked over, handed Wyatt the toy she had been playing with and patted him on the head! I was very moved that someone so young understood compassion and was willing to give up something she enjoyed so my son wouldn’t be upset.

Abby, please remind your readers that the littlest gesture can change someone’s life. I’m glad I stopped to take another look at my son that day. I can’t thank that little girl enough.

Thankful For Little Ones, Lemay, Mo.

Dear Thankful: I’m glad you stopped for that second look, too. you are absolutely right that the smallest gesture can change someone’s life — and that statement applies to people of every age and from every walk of life. there are angels among us, and you saw one of the littlest.

In the March 11 online edition of the American Journal of Respiratory and Critical Care Medicine, the researchers say that because most cases of emphysema are caused by smoking, the test they are developing can warn smokers about impending development of the untreatable disease—which is currently a major cause of disability and death in the U.S.

Not all smokers develop emphysema, but those who find out they are at risk will be motivated to quit to halt progression of the disease, says the study’s lead investigator, Dr. Ronald G. Crystal, chairman and professor of genetic medicine and the Bruce Webster Professor of Internal Medicine at Weill Cornell Medical College and chief of the Division of Pulmonary and Critical Care Medicine at NewYork-Presbyterian Hospital/Weill Cornell Medical Center.

“We know, from other studies, that smokers who learn from objective evidence that their health is in danger are much more likely to quit,” he says. “That is the only thing that will help them avoid this deadly disorder.”

Emphysema and chronic bronchitis are the twin disorders that make up chronic obstructive pulmonary disease (COPD), which is now the fourth leading cause of death in Americans. Given the aging population, COPD is soon expected to move up to third in mortality prevalence, Dr. Crystal says.

The new test measures particles that are shed by tiny blood vessels known as capillaries that surround air sacs (alveoli) in lungs. These particles are debris shed by ongoing injury to the air sacs—damage that eventually results in devastation of the sacs and the “Swiss cheese” appearance of the lungs. the alveoli are where critical gas exchanges occur: blood in the capillaries brings carbon dioxide from the rest of the body for release into the air sacs, and the oxygen in the sacs (taken in from breathing) is taken up by the blood and transported to the rest of the body.

As the sacs are destroyed, people develop shortness of breath because they cannot take in enough oxygen to feed the body and eventually cannot remove carbon dioxide from the blood.

Dr. Crystal and his colleagues reasoned that as capillaries surrounding the air sacs are being injured, the debris would be carried out by the blood supply and could potentially be quantified as a disease biomarker. so they began to look for evidence of what they called endothelial microparticles (EMP).

“Our blood vessels are always being replenished, so we all have some level of EMPs in our blood,” he says. “What we are looking for are elevated levels of EMPs. for smokers, this is the equivalent of a smoke detector sounding its alarm; elevated levels of EMPs suggest their air sacs are being injured and it is time to act.”

To do this, the researchers enrolled three groups of people—healthy nonsmokers, healthy smokers, and smokers with early evidence of lung destruction. Study participants had their medical histories taken, and to gauge lung function in these participants, all underwent two pulmonary function tests. one is spirometry, which measures the volume and speed of air as it is inhaled and exhaled from the lungs. the other, known as DLCO, is the only lung function test available today that can detect emphysema in patients. it uses a machine that measures the ability of gases to diffuse across the alveolar-capillary membrane.

The researchers found a 95 percent positive correlation between elevated EMPs in the blood and an abnormal DLCO test result, meaning that it detected nearly all verified cases of early emphysema in participants.

Two other independent groups of participants were then given the same group of tests—spirometry, DLCO and the EMP blood test—and, once again, a positive EMP finding correlated with an abnormal DLCO 95 percent of the time. Differences in the spirometry findings had no bearing on results of DLCO or EMP.

DLCO, which must be administered by a pulmonologist, is most often used to confirm a suspicion of emphysema, Dr. Crystal says. By contrast, the EMP blood test is designed to be a simple, low-cost screening tool that can pick up development of emphysema in individuals who show no signs of the disorder.

“We need a blood test that can be administered to the 20 percent of American adults who smoke as well as nonsmokers exposed to secondhand smoke—all who may not understand their risk of developing this progressive lung disease,” says Dr. Crystal.

The researchers are conducting further studies of the EMP test in larger groups of participants in order to validate these initial findings.

The study was funded by grants from the National Institutes of Health.

Detecting kidney disorders will now become as easy as detecting pregnancy. ?At home? kits for kidney function tests are now available over the counter, providing a much-needed way to diagnose kidney problems in time.

According to doctors, groups susceptible to kidney disorders, especially diabetics and patients with high blood pressure, should take conduct home tests at least once a month. Others can do it twice a year as a preventive measure.

Medically termed urine microalbuminuria tests, these kits have labelled colour code strips.

Dr Sanjeev Gulati, Associate Director of Nephrology at Fortis Hospital, said, ?The first morning urine sample should preferably be used for the test. Like pregnancy tests, the urine sample is passed on the kit, and the change in colour indicates the result.?

The greatest advantage of taking this test is the detection of disorders without having to wait for symptoms. ?The problem with kidney diseases is that patients start showing symptoms only after 20-30 per cent of the organ functionality is lost. hence, the treatment starts late ? at a time when the kidneys have already deteriorated,? said Dr Sandeep Manchanda, assistant professor of Nephrology at AIIMS.

Doctors say that the Calculated Glomerular Filtration Rate (CGFR) test, arguably the most appropriate way to detect kidney disorders, does not serve as a suitable screening method. ?The dip test kits can serve as a convenient screening methods for kidney diseases at home. If the tests show positive, we make the patient undergo confirmatory results before starting treatment,? said Dr Sanjiv Jasuja, senior consultant, Nephrology, Apollo Hospital.
