On Thanksgiving Day last year, Lee Sheppard ended up in the emergency room suffering from kidney failure. he had been diagnosed many years earlier with polycystic kidney disease, a hereditary condition that had killed his father and grandfather. he had lived with the disease for years with few symptoms until that day.

Doctors had told him his kidneys would only last about a year in 2010. but large, dark cysts caused severe pain for Sheppard and brought concern about cancer. Doctors removed both of his kidneys, which were functioning at less than 20 percent of normal.

Following the surgery, Sheppard began dialysis and his wife began to ponder giving him her kidney.

“It was amazing how many lovely people came out of the woodwork and volunteered their kidneys. It’s not something that’s easy to say ‘thank you’ to when somebody offers you such a great gift,” Pam Sheppard said. “I felt so privileged that I became the best match.”

“How overwhelmingly grateful I am for my wife for making such a sacrifice. we feel so liberated and energized for what the future holds, so, we’re deeply grateful,” Lee Sheppard said.    

“we already enjoyed a rich love together, but this last year — which has been a journey — has reinforced it in such a way that it’s hard to put a finger on it,” Lee Sheppard said. “The transplant center team is very, very profession and does everything it can to make sure nothing goes wrong … but also (shows) real concern, really compassionate.”

Dr. James Stinson recalled the first such donation — 28 years ago — when a sister gave a kidney to her brother.

“It freed him from dialysis and gave him many years of life that he would not otherwise have had,” he said.

Today, the Intermountain Transplant Program performs more than 135 heart, kidney, liver and pancreas transplants each year.

Dr. Diane Alonso, another surgeon with the Intermountain Transplant Team, recognized the courage and generosity of the human spirit.

“It cements that living donation is a vial source of organs not only in this state but in the country, but particularly in this state,” she said. “At our institution, 50 percent of our kidney donations are from live donors.”

I have to do this,” said Jacob, a 25-year-old law student at Arizona State University.   a life-threatening kidney condition is a struggle for anybody, but for a young boy it poses a special challenge.   “He doesn’t like going to dialysis, and he tells me he doesn’t want to go” Cortney said.   this situation has been difficult for his sister Emma as well.   “This whole thing has been really hard on her, but she is doing remarkably well,” said Cortney.   Cortney and Ben estimate the cost of the transplant and post care are more than $50,000. Post care expenses include anti-rejection medication and regular doctor visits. in order to alleviate some of the financial burden, the Jones family has found creative ways to raise money.   “We are selling shirts, bracelets. Volunteers are having a dinner auction in New York, family and friends are doing one in Washington and people are doing car washes in Thousand Oaks,” Cortney said.   So far Cortney and Ben have raised nearly $10,000 with the help of Children’s Organ Transplant Organization, church members and friends. even though $10,000 seems like a big chunk, they still need about $40,000 more. in the near future Cortney plans to put together a local fundraiser that would take place after Austin’s transplant to further aid in the medical costs.   To contribute to Austin’s care, click here. During a regular annual physical exam, blood is usually drawn to check the health of a person’s heart, kidneys and liver. now, researchers at NewYork-Presbyterian Hospital/Weill Cornell Medical Center say a blood test that detects the early development of emphysema—well before symptoms occur—may someday also be offered.

In the March 11 online edition of the American Journal of Respiratory and Critical Care Medicine, the researchers say that because most cases of emphysema are caused by smoking, the test they are developing can warn smokers about impending development of the untreatable disease—which is currently a major cause of disability and death in the U.S.

Not all smokers develop emphysema, but those who find out they are at risk will be motivated to quit to halt progression of the disease, says the study’s lead investigator, Dr. Ronald G. Crystal, chairman and professor of genetic medicine and the Bruce Webster Professor of Internal Medicine at Weill Cornell Medical College and chief of the Division of Pulmonary and Critical Care Medicine at NewYork-Presbyterian Hospital/Weill Cornell Medical Center. 

“We know, from other studies, that smokers who learn from objective evidence that their health is in danger are much more likely to quit,” he says. “That is the only thing that will help them avoid this deadly disorder.”

Emphysema and chronic bronchitis are the twin disorders that make up chronic obstructive pulmonary disease (COPD), which is now the fourth leading cause of death in Americans. Given the aging population, COPD is soon expected to move up to third in mortality prevalence, Dr. Crystal says.

The new test measures particles that are shed by tiny blood vessels known as capillaries that surround air sacs (alveoli) in lungs. These particles are debris shed by ongoing injury to the air sacs—damage that eventually results in devastation of the sacs and the “Swiss cheese” appearance of the lungs. the alveoli are where critical gas exchanges occur: blood in the capillaries brings carbon dioxide from the rest of the body for release into the air sacs, and the oxygen in the sacs (taken in from breathing) is taken up by the blood and transported to the rest of the body.

As the sacs are destroyed, people develop shortness of breath because they cannot take in enough oxygen to feed the body and eventually cannot remove carbon dioxide from the blood.

Dr. Crystal and his colleagues reasoned that as capillaries surrounding the air sacs are being injured, the debris would be carried out by the blood supply and could potentially be quantified as a disease biomarker. so they began to look for evidence of what they called endothelial microparticles (EMP).

“Our blood vessels are always being replenished, so we all have some level of EMPs in our blood,” he says. “What we are looking for are elevated levels of EMPs. for smokers, this is the equivalent of a smoke detector sounding its alarm; elevated levels of EMPs suggest their air sacs are being injured and it is time to act.”

To do this, the researchers enrolled three groups of people—healthy nonsmokers, healthy smokers, and smokers with early evidence of lung destruction. Study participants had their medical histories taken, and to gauge lung function in these participants, all underwent two pulmonary function tests. one is spirometry, which measures the volume and speed of air as it is inhaled and exhaled from the lungs. the other, known as DLCO, is the only lung function test available today that can detect emphysema in patients. it uses a machine that measures the ability of gases to diffuse across the alveolar-capillary membrane.

The researchers found a 95 percent positive correlation between elevated EMPs in the blood and an abnormal DLCO test result, meaning that it detected nearly all verified cases of early emphysema in participants.

Two other independent groups of participants were then given the same group of tests—spirometry, DLCO and the EMP blood test—and, once again, a positive EMP finding correlated with an abnormal DLCO 95 percent of the time. Differences in the spirometry findings had no bearing on results of DLCO or EMP.

DLCO, which must be administered by a pulmonologist, is most often used to confirm a suspicion of emphysema, Dr. Crystal says. By contrast, the EMP blood test is designed to be a simple, low-cost screening tool that can pick up development of emphysema in individuals who show no signs of the disorder.

“We need a blood test that can be administered to the 20 percent of American adults who smoke as well as nonsmokers exposed to secondhand smoke—all who may not understand their risk of developing this progressive lung disease,” says Dr. Crystal.

The researchers are conducting further studies of the EMP test in larger groups of participants in order to validate these initial findings.

The study was funded by grants from the National Institutes of Health.

Detecting kidney disorders will now become as easy as detecting pregnancy. ?At home? kits for kidney function tests are now available over the counter, providing a much-needed way to diagnose kidney problems in time.

According to doctors, groups susceptible to kidney disorders, especially diabetics and patients with high blood pressure, should take conduct home tests at least once a month. Others can do it twice a year as a preventive measure.

Medically termed urine microalbuminuria tests, these kits have labelled colour code strips.

Dr Sanjeev Gulati, Associate Director of Nephrology at Fortis Hospital, said, ?The first morning urine sample should preferably be used for the test. Like pregnancy tests, the urine sample is passed on the kit, and the change in colour indicates the result.?

The greatest advantage of taking this test is the detection of disorders without having to wait for symptoms. ?The problem with kidney diseases is that patients start showing symptoms only after 20-30 per cent of the organ functionality is lost. hence, the treatment starts late ? at a time when the kidneys have already deteriorated,? said Dr Sandeep Manchanda, assistant professor of Nephrology at AIIMS.

Doctors say that the Calculated Glomerular Filtration Rate (CGFR) test, arguably the most appropriate way to detect kidney disorders, does not serve as a suitable screening method. ?The dip test kits can serve as a convenient screening methods for kidney diseases at home. If the tests show positive, we make the patient undergo confirmatory results before starting treatment,? said Dr Sanjiv Jasuja, senior consultant, Nephrology, Apollo Hospital.
