TORONTO – an antipsychotic drug used as a therapy for schizophrenia appears to have an unusual property — it can neutralize some cancer stem cells that allow tumours to come back after treatment.

In lab experiments, Canadian researchers found that thioridazine can alter cancer stem cells that give rise to a type of leukemia, as well as those that generate certain breast cancer cells, without harming normal stem cells.

The discovery, reported Thursday in the journal Cell, came after researchers screened about 3,000 compounds in search of those that would selectively inhibit human cancer stem cells.

Thioridazine is one of about 20 agents winnowed down through high-tech drug screening that appear to work against cancer stem cells, said principal investigator Mick Bhatia, scientific director of the Stem Cell and Cancer Research Institute at McMaster University.

“Cancer stem cells, what they do is they keep proliferating,” Bhatia said from Hamilton. “But they don’t do what normal stem cells do, which is make other cell types. and then those cells perform their function and then die. That’s the normal life cycle.

“Cancer stem cells refuse to go through that life cycle because they never differentiate. they just keep making copies of themselves.”

That’s why, even if a patient’s malignancy appears to have been eradicated with chemotherapy and radiation, the cancer can come back, Bhatia said.

“And the feeling there is that the drugs we’re using aren’t targeting the cancer stem cells, so you’re not getting to the root. You’re just picking the dandelion off the top.

“And it looks good for a while, the lawn looks fine, but if you don’t dig at it and get the root, it just comes back.”

Bhatia admits that thioridazine’s apparent power over cancer stem cells came as a bit of a surprise.

The drug works against schizophrenia, for example, by targeting dopamine receptors in the brain. Dopamine is a chemical messenger that plays a variety of roles in the brain, including helping to regulate attention, cognition and movement.

The drug is also used to treat the symptoms of Parkinson’s disease, which develop in response to declining dopamine levels caused by the death of brain cells that make the neurotransmitter.

Thioridazine doesn’t appear to kill cancer stem cells, but rather encourages them to differentiate, thus exhausting the pool of self-renewing cells.

The researchers showed that the drug affects leukemia stem cells without harming normal blood stem cells. It turns out that unlike normal blood stems cells, leukemia cells have a dopamine receptor on their surfaces — making them vulnerable to the drug.

Dopamine receptors also appear on some breast cancer stem cells, the researchers found.

“This gives us some explanation,” Bhatia said. It also suggests that dopamine receptors might serve as a biomarker for early detection of rare, tumour-initiating cells in patients.

Michael Rudnicki, CEO and scientific director of the Stem Cell Network of Canada, described the paper as a “beautiful study, a very nice piece of work.”

“I think this paper really exemplifies the Canadian leadership in cancer stem cell research,” Rudnicki said Thursday from Ottawa. “Cancer stem cells were really discovered here in Canada, and this work takes it to a new level.”

He said the researchers’ findings strongly support the notion that already-approved, “human-ready” drugs exist in the global pharmaceutical armamentarium that could bolster doctors’ ability to defeat some cancers.

“Everything that we can do to build up the toolbox for the clinicians treating cancer will help this,” said Rudnicki, who was not involved in the study.

The McMaster-led team, which includes physicians, is planning to test thioridazine in combination with standard anti-cancer drugs in a small group of adults with acute myeloid leukemia who have not responded to chemotherapy alone.

The researchers’ next step is to investigate the effectiveness of thioridazine in other types of cancer, as well as exploring the potential of the other compounds identified so far, Bhatia said.

“We really want these drugs to be something that we can target at a clinical level.”

In the future, thousands of other compounds will be analyzed with McMaster’s robotic stem cell screening system in collaboration with other academic groups and the pharmaceutical industry.

“The goal for all of the partners,” said Bhatia, “is the same — to find unique drugs to change the way we tackle and treat cancer.”

Well, the symptoms of leukemia are broad, but there are specific symptoms of leukemia to keep an eye out for:
1) fever
2) infection
3) excessive bruising
4) fatigue
5) physical exercise intolerance
6) abdominal pain, or generally feeling fullness
7) weight loss
abnormal bleeding
9) enlargement of the lymph nodes, spleen, and/or liver
10) weakness

Leukemia usually occurs on people over 25 yrs old..but you know, when it occurs on children, it usually happens before the child is 4. So don't worry too much:)

I think I may have bone cancer in my arm. I have been having a lot of pain when I try to twist it and I can't unbend it all the way. I went to a limb specialist and she told me I may have inflammation in my arm and to take an inflammatory every day for two weeks. I have been, but nothing has changed. I don't know exactly how to tell, however my arm has been this way for a little over 6 months. it has gotten worse over that time. I read that inflammation is one of the bone cancer symptoms, but I wanted to know others. I am 15 by the way.

Yeah, because the internet is a much better doctor than your specialist. stop searching up symptoms up on the internet and over reacting. keep taking the inflammatory and leukemia is very rare.

Stop worrying ask about the possibility's of cancer. Believe me if you had cancer you would have more symptoms than just inflammation because bones re produce blood cells and the solid structure in which the nervous system run's so you would expect more symtoms.

The pain is often worse at night. as the cancer grows it can also cause swelling in the affected area. If it is near a joint, it may make movement in the joint difficult.

so far a few months now I have been having horrible headaches, pain in my bones (mostly arms and legs) that sometimes makes it hard to walk. my skin is really pale and I have off and on fevers a lot, and I am always really tired. I bruise easily, too. I looked these up and got everything from leukemia to anemia, do you know what can cause these that is most likely?

It is hard to say really because of there being a lot of problems that can be initiated with many different things. Below are somethings that you should ask your doctor about.

1) what age are you? some of these conditions can be exacerbated or made worse with someone who is slightly older. There are certain elements that can indicate various ailments.

2) for the headaches, are they migraines? Or does it feel like there is a pounding sensation? if so, you might want to show the areas to the doctor when you're visiting them.

3) When you mention pain in the bones (mostly in the arms and legs), does it feel like your legs are heavy? swollen? where you could easily collapse? where you feel like there is no strength in them?

4) Do your joints lock up such as in the knees and elbows and shoulder and ankle? Do they become swollen or filled with water?

5) what type of skin color do you normally get? Are you normally fair skin? or do you normally have blotchy skin that is freckled?

6) Do you feel like you're short of breath? or able to breathe?

7) Do you tire easily or get massive cases of fatigue?

Are your muscles feeling sore?

It can be a number of conditions and to many to try to guess. maybe as simple as lack of vitamin to autoimmune disease.There are just to many to even try to go into.
Best advice would see your primary Dr. for a start and get a blood test and that may be the answer to all. if not he will lead you into the right direction.

CLYDE, Ohio — every time his kids cough, Dave Hisey’s mind starts to race. Is it cancer? Is it coming back?

His oldest daughter, diagnosed with leukemia nearly five years ago when she was 13, is in remission. His 12-year-old son has another year of chemotherapy for a different type of leukemia. and his 9-year-old daughter is scared she’ll be next.

Hisey is not alone in fearing the worst. just about every mom and dad in this rural northern Ohio town gets nervous whenever their children get a sinus infection or a stomachache lingers. It’s hard not to panic since mysterious cancers have sickened dozens of area children in recent years.

Since 1996, 35 children have been diagnosed — and three have died — of brain tumors, leukemia, lymphoma, and other forms of cancer — all within a 12-mile wide circle that includes two small towns and farmland just south of Lake Erie. with many of the diagnoses coming between 2002 and 2006, state health authorities declared it a cancer cluster, saying the number and type of diagnoses exceed what would be expected statistically for so small a population over that time.

“All you think about is what happened to these kids,” said Donna Hisey, 43, the mother whose family has been devastated by cancer. “Is it gone? or is it still here? what is it?!”

After three years of exhaustive investigation, no cause is known. Investigators have tested wells and public drinking water, sampled groundwater and air near factories and checked homes, schools and industries for radiation.

They also set up a network of air monitors across eastern Sandusky County, finding cleaner air than in most places around Ohio, the health department said.

Nothing unusual was detected. not even a hint.

“From the very beginning, we’ve said the vast majority of childhood cancer causes aren’t known,” said Robert Indian, the state health department’s chief of comprehensive cancer control. He’ll soon release yet another investigative report.

Without any answers as to what’s attacking their children, parents are left to question whether living within a known cancer cluster area is endangering their kids. perhaps surprisingly, only a handful have moved away.

“It’s in the back of everybody’s mind,” said Scott Mahler, who has two healthy young sons. “Are you going to risk your children’s lives by living here?”

Eight children were diagnosed with cancer in and near Clyde between 2002-2006, nearly four times the number that state health experts figure is normal.

Ohio health investigators converged on the town of just 6,000 people halfway between Cleveland and Toledo and home to the Whirlpool Corp.’s largest washing machine factory.

What they found was worse than anyone suspected. The cancers affecting victims age 19 and younger included neighboring townships and much of the nearby town of Fremont.

One in five of the cancer cases were related to the brain or central nervous system, matching national rates, according to the American Cancer Society.

The diagnoses peaked in 2006, when nine children were told they, too, had cancer. Since then, there have been four new cases. The most recent came in the spring this year, when a 7-year-old girl was diagnosed with rhabdomyosarcoma, a rare cancer of the body’s connective tissues.

At first, investigators focused just on Clyde, where social calendars revolve around school, sports and church. Most families have been here for generations. It’s the kind of place where teens can’t wait to leave — only to find they can’t wait to come back to start a family.

Seeing their children afflicted by unexplained illnesses has strengthened the bond among parents and neighbors instead of scaring them away.

“Even if it would’ve happened to my family, I can’t imagine where else I would go to get the support I needed,” said Melanie Overmyer, an English and journalism teacher at Clyde High School.

“People in neighboring towns say ‘I can’t believe you still live there,’” said the mother of two. “You can’t pick up your life and move every time there’s something that scares you.”

Enrollment numbers at area schools haven’t dropped and real estate agents say they haven’t encountered anyone who doesn’t want to look for homes in the area or is desperate to get out.

“Clyde is small enough that we would really know if that was happening,” said City Manager Paul Fiser.

Ohio health and environmental regulators have speculated the cause was environmental and may have come and gone — maybe a chemical from a factory or a dump that polluted the air or water.

Air and water samples have not revealed any concerns around the Whirlpool plant or the Vickery Environmental waste site just outside town, where hazardous chemicals are injected into rock a half-mile below ground.

And in September, investigators said they found no radiation from homes, schools, or industries to link to the illnesses, ruling out the Davis-Besse nuclear plant, about 20 miles from Clyde, and NASA’s former nuclear reactor near Sandusky as a possible source.

Please advise.

Someone in my family has been diagnosed with Leukemia, 3 years ago. she has undergone chemo and is now ok (thank God!). but, starting abt 4-6 months ago, she started to experience sharp, almost constant pain in what seems to be the leg and hip bones. she also says she feels 1000 needles in her heels/soles.

Now the doctor didn't allow any x-ray or CAT scan (for fear of radiation exposure). Her osteoporosis test from a year ago was fine.

Definition of Bone Cancer
Cancer that begins in the bone is called primary bone cancer. Primary bone cancer is relatively uncommon in comparison with secondary or metastatic cancer (cancer that occurs initially in another organ and then spreads to bone tissue).

Description of Bone Cancer
The bones in the body serve several purposes. They support and protect internal organs (for example, the skull protects the brain and the ribs protect the lungs). Muscles pull against the bones to make the body move. Bone marrow (the soft, spongy tissue in the center of many bones) makes and stores blood cells.

Cancer that begins is the bone is called primary bone cancer. it is found most often in the arms and legs, but it can occur in any bone in the body. Children and young people are more likely than adults to have bone cancer.

Primary bone cancers are called often called sarcomas. there are several types of sarcoma. Each type begins in a different kind of bone tissue. The most common are osteosarcoma, Ewing's sarcoma and chondrosarcoma.

Osteosarcoma is the most common type of bone cancer in young people. it usually occurs between the ages of 10 and 25. Males are affected more often than females. Osteosarcoma often starts in the ends of bones, where new bone tissue forms as a young person grows. it usually affects the long bones of the arms or legs.

Chondrosarcoma is found mainly in adults. This type of tumor forms in cartilage, the rubbery tissue around joints.

Cancers that begin in the bone are quite rare. on the other hand, it is not unusual for cancers to spread to the bones from other parts of the body. When this happens, the disease is not called primary bone cancer. Each type of cancer is named for the organ or the tissue in which it begins. Treatment of Bone Cancer
The treatment of cancer of the bone, especially metastatic cancer, has two goals: management of the neoplasm and management of the symptoms produced by the local lesion. Prognosis is affected by a patient's age, the size of the primary tumor, grade and stage, degree of lymphatic and blood vessel invasion, the duration of symptoms and the location of the tumor on the arm, leg or trunk.
There are two ways bone metastasis is treated. Systemic therapy, aimed at cancer cells that have spread throughout the body, includes chemotherapy, hormone therapy, and immunotherapy. Local therapy, aimed at killing cancer cells in one specific part of the body, includes radiation therapy and surgery.

Surgery often has to be extensive, with a wide margin of tissue around the tumor being removed. Sarcomas involving muscles require removal of the entire affected muscle group.

Radiation therapy is used to prevent local recurrences of radiosensitive tumors and may be given either before or after surgery.

Chemotherapy – a number of drugs have proven to be effective in treating bone and soft tissue sarcomas. The dosages required to provide a good chance for cure often produce significant side effects. Effective single agents may include doxorubicin (Adriamycin), cyclophosphamide, high-dose methotrexate (with leucovorin rescue), ifosfamide, dacarbazine, vincristine, dactinomycin (Actinomycin D), etoposide and investigational agents. Combinations of these drugs are often used.

Hormone therapy is either the removal of the organs which produce hormones which can promote the growth of certain types of cancer (such as testosterone in males and estrogen in females), or drug therapy to keep the hormones from promoting cancer growth.

Biphosphonates are drugs that can be used to reduce bone pain and slow down bone damage in people who have cancer that has spread to their bones.

Even if a bone or soft tissue sarcoma is appears to be localized and could apparently be completely removed, there is still significant risk that tumor cells too small to detect have already spread to other places in the body. Additional treatment with chemotherapy (adjuvant chemotherapy) attempts to eliminate these tumor deposits.

There are also safe and effective ways to treat pain. Medications can allow people to be free of pain so that they can continue the activities that are important to them.

When appropriate, surgeons avoid amputation by removing only the cancerous section of the bone and replacing it with an artificial device called a prosthesis.

Chemotherapy and radiation may also be used alone or in combination. because of the tendency for Ewings sarcoma to metastasize rapidly, multidrug chemotherapy is often used, in addition to radiation therapy or surgery on the primary tumor. Radiation therapy is used to treat bone pain in a person who has bone metastasis. Radiation to the affected areas can also prevent further weakening of the bones. The total dose of radiation that can be given is limited, however. Chemotherapy is sometimes used to help control the underlying cancer.

GOLDEN GATE ESTATES —Chase Hoover is, in many ways, a typical 18-year-old.

He loves hunting, camping and fishing. He loves fast food. He loves a girl named Brittney, whom he text messages frequently.

But while Chase’s friends are sitting in class on a Thursday afternoon, Chase is lying in a hospital bed at Lee Memorial Children’s Hospital, a needle in his back, spinal fluid dripping into glass vials collected by his doctor.

The vials will be tested to make sure the cancer coursing through Chase’s bone marrow hasn’t made it into his spine.

Chase has acute lymphoblastic leukemia, which is a form of cancer of the white blood cells. with this type of leukemia, malignant white blood cells continuously multiply and are overproduced in the bone marrow. it causes damage and death by crowding out normal cells in the bone marrow, and by spreading into other organs.

Chase was diagnosed with leukemia when he was 15.

“It was April 30, 2007,” Chase’s mother, Dodi Hoover, remembers. “We had a huge party for (oldest son Austin) because he was home from this first deployment in Iraq. but Chase had been showing symptoms three months before. He had aches and pains, bruising. He had jaw pain, but we thought that came from getting his braces tightened.”

Hoover said she was cleaning the hot tub on that spring evening when she heard screaming from inside the house.

“Chase said it felt like his bones were being crushed,” she said.

She drove him to the hospital where he underwent bone scans, ultrasounds and other tests to determine what it was. When the Hoovers were told it was leukemia, Hoover said she couldn’t muster a positive attitude.

“I thought he was going to die,” Dodi Hoover said. “I was scared to death. I thought, ‘Oh my God, I am going to lose my baby.’”

But doctors quickly alleviated her fears. This type of leukemia is 80 percent curable in children. All Chase needed was to receive chemotherapy, which he did through a port in his chest once a week and then, once every two weeks. He took chemotherapy every night in pill form, too.

“It’s the most common cancer we see in children,” said Craig Macarthur, Chase’s pediatric hematologist and oncologist at Lee Memorial. “And it is very treatable.”

After almost three years of treatment, Chase was feeling good. He was a senior at Palmetto Ridge High School in Golden Gate Estates. He was healthy.

A large green ribbon with the words “Chase Life” were tattooed on his leg, something he got when he turned 17 and entered the second and final year of his treatment.

“It’s my trademark,” he said. “The ribbon is supposed to be orange, but green is my favorite color.”

But it wouldn’t last. Chase was at a party several weeks ago when his back “spazzed out.”

“I could not walk or talk, I was in so much pain,” he said.

The next morning, still in pain, Chase went to the emergency room. there, the family received the bad news.

The cancer was back and this time, it affected 85 percent of his bone marrow and, without a bone marrow transplant, Chase’s chance of survival was slim.

“An early relapse is not good because it means the cancer is becoming chemo resistant. That means that it is pretty impossible to cure with chemo alone,” oncologist Craig Macarthur said.

Chase is pragmatic about the new diagnosis.

“There are lot of ways I can feel about it, but there is one way I can deal with it. I can buck up and try not to freak out,” he said.

Hoover said her son is “mentally tough” about his diagnosis and his treatment. she said sometimes she worries when her son talks about doing things like camping and hunting when he is going through his treatments, but said she understands why he needs to keep living his life.

“It gives him something to look forward to so he doesn’t think about this,” she said.

Instead, when Chase talks about the cancer relapsing and his future — and there is a future when he talks — he talks about the disappointment of having to give up a good job cleaning pools. He talks about the months he will have to spend in the hospital when he receives a bone marrow transplant and how he will miss his girlfriend.

He also has dropped out of high school, although he plans to go back, while he goes through treatment and in preparation to spend three months in the hospital while he receives a transplant.

“It’s hard to predict how I will feel (going through chemotherapy),” Chase Hoover said. “When you have a geometry class or a world history class, missing one day is hard. When you have to miss three weeks, or more, and try to get back to that, it is impossible.”

Doctors began talking to the Hoovers about a bone marrow transplant for Chase, testing his brothers — Austin, 23, and Jerred, 20 — to see if they would be a match. Siblings have a one in four chance of being a match, according to Macarthur.

The boys came to the hospital and were tested. When testing to see if the marrow is a match, doctors look for a donor who matches their patient’s tissue type, specifically their human leukocyte antigen (HLA) tissue type. HLAs are proteins — or markers — found on most cells in the body, according to BetheMatch.org, a nonprofit organization that seeks to find matches for patients who need bone marrow nationwide.

One’s immune system uses these markers to recognize which cells belong in the body and which don’t. the closer the match between the patient’s HLA markers and the donor’s, the better for the patient, according to the website.

Then came the waiting, more than two weeks’ worth, while tests were run to see if either brother is a match. if not, Chase would have to look for an unrelated donor, since parents are typically not a match to their children.

She sent the text messages out Tuesday.

The message is good news for Chase.

According to Macarthur, Chase’s best chance at survival is if his brother is a match for a bone marrow transplant.

“There are fewer complications than with an unrelated donor,” he said. “With an unrelated donor, there is an increased chance of toxicity, increased risk of failure, increased risk of graft vs. host disease. the chances of survival with a sibling match are 60 percent, compared to 40 percent to 50 percent with an unrelated donor. He would have a less than 25 percent chance if we treated his condition with chemo alone.”

The news means that after the new year, Chase will receive a bone marrow transplant. Doctors want him to finish this course of chemotherapy treatment before starting to work the transplant.

When it finally happens, the transplant will require Chase to be in the hospital for about three months, including a month in isolation as he receives chemotherapy and maybe radiation to kill the rest of his bone marrow so that when his brother’s marrow is introduced to his system, cancer cells will not infect it.

“It’s a bit like having a lawn full of weeds,” Macarthur said. “You want to make sure they are all gone before you put down the seed to grow.”

Macarthur said the typical treatment for Chase would be to have chemotherapy and radiation to kill his bone marrow. then, doctors will take bone marrow from his brother and give it to Chase through an IV.

This weekend, Chase was trying to take his mind off of his illness. He went to the Redneck Yacht Club in Charlotte County for a weekend of camping and hanging out with his girlfriend and friends.

“Once I really get into the treatment, it is all downhill,” he said. “My immune system is going to suck. I am not going to be able to do what I want to do.”
