Grisalea Uwabaho does not know her daughter’s result yet, but – whatever it is – she says she will believe it.

The 30-year-old mother of five walked an hour this morning to bring her youngest child – her wailing, feverish one-year-old daughter, Mariam – to this tin-roofed, two-room clinic, which is the most advanced health facility she could hope to reach in Kyankwanzi district, an impoverished corner of central Uganda. Uwabaho fears that her baby may have malaria, and she has brought her here to find out for sure.

A nurse pricks the baby’s finger then places a drop of her blood on a tiny window inside a small strip of plastic. Fifteen minutes later, he announces the result: negative. the nurse gives Uwabaho medicine to fight off Mariam’s cough and diarrhoea.

Here in Kyankwanzi district, and across sub-Saharan Africa, that little strip of plastic – better known as a rapid diagnostic test (RDT) for malaria – is revolutionising both the management and the understanding of a disease which threatens nearly half of the world’s population and that killed close to 800,000 people in 2009, 90% of them in Africa. Clinicians in far-flung African villages used to have no choice but to diagnose malaria based on symptoms alone. But, today, health workers who have access to RDTs – which now cost less than $1 (63p) apiece – can determine with relative certainty whether patients have any malaria parasites in their blood.

It is a simple feat, perhaps, but it represents a crucial step in the struggle to control this deadly, but highly treatable, disease. where they have been introduced, RDTs have helped to improve patients’ chances of recovery by ensuring more appropriate treatment for fever-causing conditions and conserving precious anti-malarial drugs. they have also helped to paint a much more accurate picture of the malaria problem the world faces today.

“As RDTs were introduced, it became clear that there was nowhere near as much malaria in many areas as people thought,” says Dr David Bell, the head of the malaria diagnostics programme at the Geneva-based Foundation for Innovative new Diagnostics (Find).

“Everyone who had a fever used to be classified as having malaria, and now maybe 20% of those people have malaria, and 80% have something else,” he says. People have to start thinking differently about the importance of accurate diagnosis, he says.

Thanks to an upsurge in funding, the global struggle to control malaria has made significant progress in the past 10 years. according to the Roll back Malaria Partnership, an umbrella organisation based in Geneva, global deaths from the disease have dropped 38% since 2000. the RDT, despite only being around for a few years, is now playing a very significant part in the continuing control of malaria.

Such is the power and accuracy of the little plastic strip that, in 2010, the World Health Organisation (WHO) did away with its longstanding recommendation that malaria should be treated based on clinical symptoms alone. Now, it says that health workers everywhere – even in sub-Saharan Africa, where conditions are most challenging – should aim for parasite-based diagnosis, either with an RDT or a microscope, the traditional and much more technically demanding method of detecting malarial parasites.

When WHO changed its official recommendation, many countries followed suit; Uganda was one of them. Health officials there have set an ambitious target: by 2015, they want 85% of all suspected malaria cases to be subjected to parasite-based diagnosis. In 2007, that figure stood at just 9%; four years later, it has nearly trebled to 25%.

The recent gains have come in part thanks to the work of the Malaria Consortium, a UK-based international NGO that has been supplying RDTs and working with the Ministry of Health to teach health workers how to use them in five districts in central and western Uganda. the organisation’s work has revealed both the advantages and the challenges of the tests; their experience holds critical lessons for others looking to promote the use of RDTs.

Getting the local health workers to change their attitudes has been one of the trickiest aspects of the project, says Robin Altaras, who co-ordinates Malaria Consortium’s Comic-Relief-funded Pioneer project in Uganda.

“They’ve gone from 20 years of practising presumptive treatment of malaria to suddenly radically changing how they manage fever,” he says. “That’s a process that needs to be supported continuously over the long term, starting with training and following with supervision.”

The particular brand of test that Malaria Consortium has chosen – one of only two the Ugandan government has approved for use – has a shelf life of two years, in common with all RDTs, and a sensitivity rate of 97%, according to ongoing product evaluations by WHO and Find. this makes it one of the best of the dozens of RDTs currently available.

When used correctly, Altaras says, RDTs can serve as a powerful control on drug stocks, reducing wastage by limiting anti-malarial medicines to only those patients who truly need them. That means that clinics are less likely to run out of stock; it also means that the medicines are more likely to remain effective in the long run.

Experts say it may only be a matter of time before the highly adaptable malaria parasite evolves to outsmart the compound artemisinin, which is currently the best – and last – line of defence against the disease. Handing out fewer courses of those precious artemisinin-based combination therapies (ACTs) – which can cure a person of malaria in just three days – means slowing down the clock on the ticking timebomb of resistance.

“ACTs should be taken only by those who have parasites, who have test results showing positive,” says Agaba Bosco, who is in charge of malaria diagnostics for Uganda’s National Malaria Control Programme. “The biggest threat now is the potential development of drug resistance,” he says, adding that such a scenario “would be a disaster” for malaria control.

A disaster in Uganda, and a disaster around the world. That is one more reason, experts say, why the global public health community should sharpen its focus on diagnostics.

The dengue fever threat could be over in just under two months as the summer rains begin to subside, the deputy chief medical officer in the Ministry of Health Delon Brennen told the Nassau Guardian.

He added yesterday during a press conference at the Ministry of Health, that while dengue hemorrhagic fever has symptoms of bleeding associated with it, one strain of the disease can also cause a drop in a person’s platelet count, which can also induce bleeding.

“What we have been trying to explain is that during the natural course of [the] dengue disease, people will have a drop in their platelet counts, and so you can have some minor bleeding that goes along with that,” he said.

“We have had episodes where people, because of their lowered platelet counts, will have blood like when they brush their teeth.  If you have a minor injury you will see a little more bleeding then, but we haven’t had the true classic definition for dengue hemorrhagic fever.”

Brennen added that they are continuing to test for other mosquito borne illnesses such as malaria in order to be certain that the dengue symptoms are not masking other illnesses.

“We will continue to test those cases to ensure that we don’t have something else the dengue is covering up,” he said.

“We continue to do rapid testing for malaria in the country because we did have malaria in the past in one of our islands and we have had to be very vigilant about making sure that we don’t reintroduce malaria into the populous.”

One man who visited Nigeria recently tested positive for malaria and was hospitalized and treated upon his return to new Providence.  Brennen said the man survived his ordeal.

He added that there have been no fatal cases of malaria in the Bahamas recently.

Brennen insisted that Bahamians in general, along with the Department of Environmental Health Services’ vector control team, have been doing their part to keep the mosquito population at bay during this summer’s dengue outbreak.

“The lucky thing is even without the rains going away we are optimistic that the declines we are seeing are because of the public,” he said.

He confirmed that the reported cases of dengue in new Providence have been decreasing and assured that as the Bahamas moves out of the summer rainy season at the end of October, mosquito breeding sites will decrease, minimizing the dengue threat.

Brennen also confirmed Tuesday that there have been some reported cases of dengue that appear to have the symptoms of the deadly hemorrhagic strain, though there have been no confirmed deaths from that particular strain of the disease. He said the number of hemorrhagic-like infections is less than ten.

Intermittent Prevetive Treatment for pregnant women (IPTp) is especially relevant for women who have lived and continue to live in areas with malaria. they generally have been exposed to malaria before, and in many cases malaria has been detected in their placentas, but they can be completely asymptomatic (i.e. they have no symptoms) of malaria like fever etc. Researchers found that giving a course of an antimalarial treatment at specified time points, reduced maternal anaemia and the likelihood of having a low birth weight baby.

The recommended schedule for IPT in pregnant women is as follows:

SP shall be given as single adult dose (3 tablets of 500mg Sulphadoxineand 25mg Pyrimethamine each) at scheduled antenatal care visits during the second and third trimesters.

The two doses of SP shall be given at least one month apart.1st. Dose: at the first antenatal visit after quickening (first fetal movement – from sixteen weeks)2nd. Dose: at least one month after the first dose.

NOTE: HIV positive pregnant women shall receive at least 3 doses of SP instead of two doses (subsequent doses shall be given at least one month apart).

What is Intermittent Preventive Treatment of malaria in infants (IPTi)?

IPTi is the administration of an antimalarial drug at the time of childhood vaccinations in the first year of life and takes advantage of delivery alongside the well functioning WHO Expanded Programme for Immunisation (EPI). under this approach, infants receive an anti-malarial drug two or three times during the first year of life whether or not they have malaria and are provided with prophylactic protection for the duration the anti-malaria drug is present in the blood.

Why IPTi instead of chemoprophylaxis?

Continuous chemoprophylaxis in young children reduces malaria morbidity during the period of prophylaxis, however following the cessation of prophylaxis, a rebound of malaria was noted in the treatment group (Greenwood 1995, Menendez 1997), even though it is uncertain whether these children actually experienced more malaria than the children that received placebo when the whole period at risk was taken into account. Menendez et al gave weekly chemoprophylaxis to infants from 2 to 12 months of age which provided 60% protective efficacy against clinical malaria, however these children then experienced almost double the level of clinical malaria than the placebo group when the prophylaxis was stopped. when the children were followed up to 4 years of age the amount of malaria experienced by the two groups was similar (Aponte 2007). Due to the concern of a rebound in malaria, and because of the major logistic challenges involved in delivering weekly prophylaxis, and fears that large-scale drug use would hasten the spread of drug resistance, implementation did not occur in endemic countries.

How has Intermittent Preventive Treatment (IPT) developed as a treatment?

With the success in preventing malaria morbidity using IPTp, it was decided to investigate IPT in infants (IPTi), taking advantage of the regular contacts that babies have with the clinic in the first year of life when their caregivers bring them in to receive vaccinationas delievered through the WHO’s Expanded Programme for Immunisation (EPI).

First results for IPTi:A study conducted in Ifakara, Tanzania of IPTi with sulfadoxine-pyrimethamine (SP), delivered at the time of the second and third doses of Diptheria-Tetanus-Pertussis/Oral Poliovirus Vaccine (DTP/OPV) and measles vaccination, at approximately 2, 3 and 9 months of age resulted in a 59% reduction in the incidence of clinical malaria and a 50% reduction in the incidence of severe anaemia (a PCV < 25%) in the first year of life (Schellenberg et al Lancet 2001). Infants who received IPTi-SP were also less likely to present as outpatients with fever and less likely to be admitted. Safety monitoring suggested no adverse consequences of IPTi-SP and serological analysis of the response to routine vaccinations was also reassuring. Infants who received IPTi-SP also had 36% less malaria in the second year of life (Schellenberg et al Lancet 2005).

A simple blood-test can determine if it is malaria but your should begin treatment as early as possible and then stop it if it is not malaria.

In the onset of Lassa fever, the symptoms easily could be mistaken for malaria.

Anti-malarial drugs are prescribed, but the patient doesn’t seem to show improvement. The health, instead, is deteriorating.

Further actions are made to combat symptoms that mimic those of other diseases such as yellow fever and dengue fever. But nothing seems to take as the patient quickly approaches death.

As soon as five to seven days after infection — when the telltale signs of Lassa, a viral hemorrhagic fever, present themselves — it’s often too late.

Endemic to portions of West Africa, Lassa fever is estimated to infect anywhere from 100,000 to 500,000 people on an annual basis and official projections place the mortality rate in the 2 percent to 5 percent range. however, some in the medical field say those estimates are quite crude and that the cases and especially the mortality rates are much greater with 10 times the number of fatalities.

“I think what we believe is that (Lassa fever) is severely under-tested and severely undiagnosed,” said Douglass Simpson, president and chief executive officer of Broomfield-based Corgenix Medical Corp.

Corgenix, the locally based maker of diagnostic tests is one member of a multidisciplinary team that’s spent the past six years addressing that concern.

As a result of federal funding, a team of businesses and academics have worked in Sierra Leone, Africa, and surrounding areas to develop an array of diagnostic tests for the deadly Lassa fever. The program’s efforts also could result in potential therapeutics and vaccines and could play a role in the U.S. government’s biodefense efforts as well, Simpson said.

Seeing there were really no means of immediately diagnosing Lassa — let alone a dedicated treatment for the disease — the National Institutes of Health awarded a grant to Tulane University, Corgenix and others to develop new recombinant proteins and viral detection products that were deployed in Africa for clinical testing.

About $25.8 million in grants have been awarded through National Institutes of Health agencies for the three Lassa projects that started in 2005 and carry through 2015, Simpson said. of those funds, about $3 million went directly to Corgenix, he said.

The team — which also includes Autoimmune Technologies LLC, Vybion inc. and University of California-San Diego researchers, among others — is in the middle of a five-year grant to further develop the test kits, finalize the clinical studies and move the products into commercialization.

A third arm of the program involves research dedicated to creating therapeutics and possibly a vaccine for the disease that is treated now intravenously with ribavirin — an antiviral taken orally to treat hepatitis C, Simpson said.

While the results from the program should benefit Corgenix’s earnings and could help the United States diagnose a Class A biological agent in a matter of minutes, the greater cause of saving lives and advancing the on-site technologies sit at the heart for Matt Boisen, Corgenix’s research and development manager and a lead scientist in the Sierra Leone program.

“We’re really hoping that we can find an avenue to get these diagnostics distributed throughout West Africa,” he said.

Boisen last week was at his Broomfield lab preparing prototypes for Tulane University professor Robert Garry, who will travel to Sierra Leone in a couple weeks. The stateside work of Boisen and others does not involve any live samples of the virus, but rather irradiated samples or recombinant agents, Corgenix officials said.

In the coming months, Boisen expects to return to the West African country to further pre-clinical testing. The work he’s been involved with already has left a lasting impression.

“There’s a really huge public health awareness push, especially with the health care workers and midwives, because it could be very fatal in pregnant women,” Boisen said, noting a 90 percent mortality rate for women in their third trimester.

Boisen relayed one account of a woman in her third trimester of pregnancy who arrived at the Lassa ward in the city of Kenema very ill. She was diagnosed and treated.

But then there are the other cases — those that all too often include young children — where the sample is taken, the test is run, but the patient dies before treatment is administered.

“There’s big highs and there’s big lows,” Boisen said.

As a means of shifting that balance toward more positive results, a number of the program’s research efforts are expected to be published soon in medical journals, said Luis Branco, scientist at New Orleans-based Autoimmune Technologies.

“There’s been really tremendous progress; I think we’ve really changed the way that Lassa virus is diagnosed and therefore treated,” he said. “… I think that we have really gleaned new insights into the virus itself and how it operates at a molecular level and, at the same time, how to use that information.”

On a local level, Branco feels the program has helped to improve infrastructure in a country still recovering from the ravages of an 11-year-old civil war. The team invested in the diagnostics, lab equipment, power supplies and also has employed locals to assist in the efforts, he said.

“We try to develop the infrastructure in the country such that we can provide some sort of self-sufficiency,” he said.

The work for Corgenix and crew is far from over, although officials hope to have a slate of blood diagnostics — including a rapid, 10-minute test and more comprehensive, two-to-three-hour tests — ready for commercialization later this year, Simpson said.

“Ultimately, where Corgenix would like to go would be to have a battery of products, perhaps even configured on the same little cassette where we could detect three or four different viruses,” he said.

Corgenix (OTCBB: CONX) also expects to further diagnostic testing for other deadly viruses, he said.

Corgenix received the entirety of a $590,000 grant for a similarly focused Ebola virus and Marburg virus program that runs from 2010 to 2012, Simpson said.

“You’re never going to eradicate the virus,” he said. “What we want to do for the next 10 years is just get into a system where we can diagnose them quickly, treat them quickly and reduce the mortality rate.”
