Drug maker Lexicon Pharmaceuticals (NASDAQ:LXRX) reported positive data from a phase two study of its experimental drug LX1032, designed to treat a chronic condition that causes severe diarrhea.

Carcinoid syndrome is caused by neuroendocrine tumors that usually originate from the gastrointestinal tract, and is characterized by severe diarrhea and flushing episodes, with long-term consequences including malnutrition, heart disease, and death.

Symptoms of the disease have been linked to excess production of serotonin by metastatic tumor cells. The company’s drug, taken orally, is designed to reduce serotonin production.

The phase two trial was carried-out as a randomized, placebo-controlled study made up of 23 patients with carcinoid syndrome, in the United States.

Patients in the study averaged six bowel movements a day, with subjects receiving either a placebo or one of four doses of the drug daily for 28 days.

The study’s endpoint was safety and tolerability. Efficacy measures included change in bowel movements, relief of symptoms and reduction in serotonin production.

Data from the study showed five patients taking the drug reduced bowel movements per day by 30% for two weeks or more during the study.

Further, nine patients had a complete biochemical response of a 50% reduction in urinary 5-HIAA, a biomarker of serotonin production. at the end of the study, six patients also reported sufficient relief of carcinoid symptoms.

According to the results, the drug was also well tolerated and adverse events were mild to moderate, the company said.

Additionally, preliminary data from a separate, single-arm study of the drug in Europe reported five out of six patients with carcinoid syndrome showed sustained reductions o of at least 30% in bowel movement frequency.

Two of these responses occurred in the first four weeks of therapy, while three more occurred in the fourth and eighth week of treatment, under the 12-week protocol.

“The phase two data clearly show a reduction in bowel movements and symptom relief in a population that has exhausted all standard-of-care treatment options,” Chief Medical Officer of Lexicon, Pablo Lapuerta, said in a statement.

“The phase two data are also consistent with preliminary results from the European study, where there appear to be some early responses and additional benefit with continued treatment.”

The LX1032 drug is being developed under fast track designation from the FDA and orphan drug designation from the European Medicines Agency (EMA).

Currently, Lexicon is planning a phase two trial of the drug for mild to moderate ulcerative colitis, an inflammatory bowel disease that affects the lining of the large intestine.

Shares in the biopharmaceutical company rose one penny to $1.41 on Tuesday morning.

“Pulmonary dysfunction has long been considered the primary cause for morbidity and mortality in CF (Pilewski and Frizell 1999), with malnutrition appearing as a compounding detrimental factor (Borowitz 1996). more recently however, malnutrition is being recognized as playing a primary role in disease progression (Borowitz1996; Schoni and Casaulta-Aebischer 2000) possibly even being responsible for lung pathology and infections (Yu et al., 2000).

Fatty Acid Balance

A fatty acid imbalance is observed in CF patients, including elevated levels of arachidonic acid and reduced levels of DHA. (…) the low DHA levels  have been shown to play a fundamental role in the pathogenesis in the organs  affected by the CF disease: lungs, pancreas and ileum (Freedman et al. 1999).

Thus, further decreasing DHA levels in cftr-/- mice worsened pathological manifestations, while elevating DHA levels by oral supplementation corrected the lipid imbalance and reversed the pathology of the affected organs.

Is it possible that the synthesis of endocannabinoids, being fatty acid derivatives, is also modulated by CFTR proteins? There are a number of striking parallels between the clinical manifestations of CF and the domains of cannabinoid and endocannabinoid influence, including lack of appetite, nausea, diarrhea, and lung disease. Low endocannabinoid levels could explain the appearance of these symptoms. However, even in the absence of a causative role, it is proposed here that by stimulating the cannabinoid system, some of the CF pathology symptoms may be alleviated.

Antiemetic Effects

Vomiting induced by coughing (Blecker et al. 2000) often exacerbates the development of malnutrition in cystic fibrosis. Antiemetic benefits of THC have been demonstrated in its effective relief of chemotherapy-induced nausea and vomiting (Mechoulam et al., 1998b; Abrahamov et al. 1995). Therefore the antiemetic potential of cannabinoids would be expected to contribute to appetite enhancement induced by cannabinoids in CF patients.

Diarrhea

Diarrhea appears in CF as a result of inadequate digestion due to pancreatic insufficiency (Rolles 1998). Cannabinoids inhibit intestinal motility via local CB1 (Colombo et al. 1998; Tyler et al. 2000) and/or via CB2 (Fride 1995; Hanus et al. 1999) receptors. Therefore administration of cannabinoids to CF patients may counteract diarrhea and thereby help prevent loss of nutrients.

Inflammation

Most destruction of lung tissue in CF is now thought to be secondary to a very aggressive neutrophilic inflammatory response (Konstan & Berger 1997; Wagener et al. 1997). This ultimately leads to respiratory failure. The antiinflammatory potential of cannabinoids is well documented (Klein et al. 2000; Straus 2001) and is thought to occur by interference with the arachidonic acid-eicosanoid synthetic pathways (McPartland 2001). We have demonstrated in a mouse model of arachidonic acid-induced ear inflammation that cannabinoids and endocannabinoids are effective antiinflammatory agents acting via CB receptors (Hanus et al. 1999; Fride et al. unpublished observations).

Since cannabinoid receptors are present in lungs (Calignano et al. 2000), THC may be of additional benefit for CF patients, by reducing inflammatory processes in the lungs.

It has been demonstrated recently that bronchodilating and cough-reducing activity of endocannabinoids in irritated lungs are mediated by local CB1 receptors (Calignano et al. 2000). Therefore cannabinoids may also benefit CF patients by their bronchodilating and cough suppressing effects.

Pain

CF patients suffer pain from a variety of sources (Ravilly et al. 1996) including abdominal pain related to steatorrhea and malabsorption (Zeltzer et al. 1996), chest pain due to impacted sputum, pleuritic involvement with lung inflammation and infection, or chest wall pain associated with developing kyphoscoliosis and decreased chest wall mobility (Massie et al. 1998). Pain may also occur from gall bladder or kidney stones or from osteoporosis (Haworth et al. 1999; Lambert 2000; Ravilly et al. 1996). Cannabinoids are analgesics effective in a variety of conditions (Mechoulam et al. 1998b; Martin and Lichtman 1998), acting via cannabinoid receptors within as well as outside the brain and spinal cord and suppressing both acute and chronic pain (Pertwee 2001).

In this paper a novel therapeutic target for cannabis is proposed, based on recent developments in research on cannabis on one hand, and on research on cystic fibrosis on the other. recent findings suggest that the primary factors in the pathogenesis of CF includes fatty acid imbalance, possibly leading to such major manifestations of CF as chronic inflammation of the lungs and pancreatic disease (Greener 2000; Freedman et al. 1999). in the final stages of the disease malnutrition accompanied by a lack of appetite is frequently seen (Anthony et al. 1999; Schoni and Casaulta-Aebischer 2000). Additional symptoms of the disease may include pain due to a variety of sources (Ravilly et al. 1996), diarrhea (Rolles 1998) and nausea (Blecker et al. 2000).

Intriguingly, the therapeutic effects of cannabinoids include the potential to counteract each of these conditions. Thus appetite enhancement (Beal et al. 1997) and a critical role in food ingestion (Fride et al. 2001), analgesic, antiemetic, antiinflammatory, inhibition of intestinal motility and bronchodilating effects have been demonstrated (Calignano et al. 2001; Colombo et al. 1998; Fride 1995; Mechoulam et al. 1998b; Hanus et al. 1999; Tyler et al. 2000).

It is proposed here, that CFTR not only regulates fatty acid balance but also endocannabinoid biosynthesis. Such mechanism predicts that low levels of endocannabioids in CF patients and in cftr-/- mice will be found, which could be responsible for many symptoms. it is hoped that affirmative data will eventually lead to the use of cannabinoids at more initial stages of cystic fibrosis (Figure 2).”

After months of stomach cramping that left her crying in pain, Katie Tricarichi, saw a doctor. She had a colonoscopy on her 16th birthday. two days later, she got the diagnosis — Crohn’s disease.

Crohn’s disease is an inflammatory bowel disease (IBD) that causes diarrhea, cramping, and loss of appetite. in severe cases, it can lead to malnutrition. Crohn’s can affect any part of the digestive tract, but it often affects the small intestines or colon.

Tricarichi’s treatment started with a number of medications that she took for four years. “Then I developed a fistula [an abnormal connection] between my bladder and my ileum,” she says. “The fistula went undiagnosed for almost six months. by the time it was found, I needed emergency surgery.”

The surgery involved a small bowel resection. Surgeons removed 12 inches of her small intestine and the first inch and a half of her colon. they also took out part of her bladder and repaired a hole on the top of her uterus.

For Tricarichi, who is now the walk director of the Crohn’s and Colitis Foundation of America, the surgery for Crohn’s changed her life.

“It gave me six blissful, almost Crohn’s-free years before I had an active flare,” she says.

Eight years later, Tricarichi takes similar medications to the ones she was on when she was first diagnosed. about her surgery, she has no regrets.

“I do believe Crohn’s surgery should only be used when medication fails, but it was definitely the best decision for me,” she says. “The surgery saved my life.”

Tricarichi’s story mirrors that of many people with Crohn’s disease. many have surgery only for an emergency or when other treatments fail. almost 3 out of 4 people with Crohn’s will need some kind of surgery eventually. Most get relief from symptoms for a while, and some can go off a few or all of their medications, at least temporarily.

“Surgery gets rid of the diseased bowel,” says colorectal surgeon Jon Vogel, MD, of the Cleveland Clinic. Vogel says surgery can help people eat and drink without pain. It also frequently eliminates the need for ongoing medications. “For all these reasons, surgery for Crohn’s improves quality of life,” he says.

At the same time, medications for IBD are also getting better, with the development of biologics and other drugs that suppress an overactive immune system. “Crohn’s disease medications improve quality of life for some people,” Talamini says.

People considering Crohn’s surgery should be cautiously optimistic, however.

“Surgery doesn’t cure Crohn’s disease,” says mark Talamini, MD. Talamini is chairman of surgery at the University of California, San Diego. Half of people who have one surgery require a second procedure or more. Tricarichi is a prime example. her Crohn’s disease is no longer in remission, and she may need additional surgery down the road.

My ex husband is saying he has stage 4 Crohn's Disease…I have looked everywhere and can't find anything over stages of this disease…I guess either you have it or you don't…Cancer has stages not this disease from what I see…Feel free to correct me if I'm wrong…I want to know when you have this disease….what can you physically not do and so on because…he won't pay child support and I'm taking him to court…I have proof the he goes shopping all the time and even hand washed three cars by hand…I was told he's filing for disability…Yeah he sounds real sick…So please anything at all you know about this disease will be much appreciated…Thank you

Crohn's disease is an inflammatory bowel disease. it can cause inflammation in the digestive tract leading to abdominal pain, severe diarrhea, and malnutrition. There is no known cure but with therapy a person can function well.

There are three categories of disease presentation in Crohn's disease: stricturing, penetrating, and inflammatory.
* Stage 1- Mild to Moderate Crohn's Disease.
* Stage 2- Moderate to Severe Crohn’s Disease.
* Stage 3- Severe Crohn's Disease.
No other mention is made in the medical literature regarding Stage 4.

A recent study of more than 500 Crohn’s disease patients in Switzerland found that almost one in five suffer from post-traumatic stress disorder, or PTSD, a type of anxiety disorder. This is felt to be due to the stress experienced during the diagnostic process, treatment regimen and everyday battles with Crohn’s physical symptoms. Further, those patients with both Crohn’s disease and PTSD were 13 times more likely than other Crohn’s patients to have worsening of their digestive symptoms.

Crohn’s disease is a type of inflammatory bowel disease that causes abdominal pain, diarrhea and malnutrition. Most patients eventually require surgery but it does not cure this chronic disease. Patients often become distressed due to the diarrhea and the need to significantly modify their lifestyle due to the illness.

Post-traumatic stress disorder, long recognized for it’s association with war and other horrific events, has recently been recognized as an unfortunate manifestation of serious, chronic illnesses. Treatment for PTSD is available from mental health professionals, such as psychiatrists and psychologists.

Those suffering from Crohn’s disease or other chronic illnesses who have symptoms of PTSD, such as upsetting dreams, irritability, difficulty maintaining healthy relationships, insomnia and difficulty concentrating should discuss their symptoms with their treating physician.

dear Dr. Gott: my 19-year-old grandson was diagnosed with Crohn’s disease two years ago. he has taken every medication I can imagine, including Humira injections into his stomach. At present, he is taking hyoscyamine and Apriso plus pain medication when it gets too severe. he has been to the hospital at least 15 times in the past two years and has seen at least six different doctors. we hope that you can help us. please.

dear Reader: Crohn’s disease is a type of inflammatory bowel disease (IBD). While there is no cure, today’s treatments can greatly reduce symptoms and may lead to long-term remission.

Common symptoms include diarrhea, abdominal pain and cramping, reduced appetite, weight loss, ulcers and blood in the stool. Those with severe Crohn’s, may also experience inflammation of the liver or bile ducts, arthritis, fever, fatigue, skin disorders and eye inflammation. Complications include bowel obstruction, malnutrition, anal fissures, ulcers, fistulas (an abnormal connection between different parts of the intestine). there is also an increased risk of colon cancer; however, the majority of sufferers never develop it.

Several types of treatment are available. The first type is anti-inflammatory drugs, such as the Apriso (mesalamine) as well as azulfidine and corticosteroids. Immune-system suppressors are also used, and Humira (adalimumab) is one. Antibiotics may be helpful in treating ulcers, abscesses and fistulas. Re-

searchers believe that antibiotics will reduce levels of harmful bacteria within the intestine, as well as suppress its immune system.

Commonly used medications include pain relievers, antidiarrheals, iron supplements, laxatives, vitamin B12 injections, calcium and vitamin D, and/or special diets, such as nutrients introduced directly into the veins, which can bypass the stomach and intestine.

If diet, lifestyle changes, medication and other treatment fail to relieve symptoms, surgery to remove a damaged portion, close fistulas or remove scar tissue may be recommended. At best, surgery can provide years of remission, but it will be temporary. nearly three-quarters of patients who undergo surgery will experience recurrence, with approximately half of them requiring a second procedure or more.

What is Fatty Liver?

Fatty liver is otherwise called as the fatty liver disease (FLD). This is a reversible condition which is caused by the triglyceride fat accumulation in liver cells. it is seen world wide and it has multiple causes which mainly includes excessive alcohol intake.

Progressive inflammation of the liver occurs in accumulation of fat and it is called steatohepatitis. Contribution of this condition by alcohol is called alcoholic steatosis. Fatty liver is commonly caused either by alcohol or metabolic diseases (ex.diabetes, obesity) and other causes include nutritional, drugs and toxins, and other causes.

Metabolic: glycogen storage disease, lipodystrophy and wolman disease.

Other: Inflammatory bowel disease and Hepatitis C.

Fatty change represents the intra-cytoplasmic accumulation of triglyceride. in the beginning stage the hepatocytes present small fat vacuoles around the nucleus. in the later stage liver cells are filled with multiple fat droplets that do not displace the centrally located nucleus. in the later stages, the size of the vacuoles increases pushing the nucleus to the periphery of the cell giving characteristic signet ring appearance and it is called macrovesicular fatty change.

Inflammation is accompanied in severe fatty liver disease and this condition is called the steatohepatititis. Most of the affected individuals are asymptomatic. The fatty liver is noticed while undergoing incidental check ups and by abnormal liver function tests or the hepatomegaly is noted in unrelated medical condtions. There are cases where elevated liver biochemistry is found, this is applicable in 50% of patients with simple steatosis. Ultrasonography reveals a bright liver with increased echogenicity. Histological diagnosis by liver biopsy is sought when assessment of severity is indicated.

The treatment is based on treating the underlying causes and it depends on what is causing it. The process of steatosis is able to be reversed if implemented at an early stage. Fatty liver disease, unlike other diseases, it proceeds in a gradual and harmful way. in fact, fatty liver symptoms are very larger than other diseases. Recognizing the fatty liver disease is easier when it is known to them.

Fatty liver symptoms:

1. Fatty liver diseases in most of the cases experience no symptoms and especially in the early stage of the disease.

2. Mild form of fatty liver symptoms and its damage is not revealed in the liver tests. But, even a mild form of fatty liver disease affects two things which are dependent on the liver. One is the metabolism and another one is the drug clearing systems.

3. Several diseases are generated or manifested by the fatty liver diseases. they are such as diabetes, high cholesterol, obesity and it is due to low metabolism as well. Fatty liver symptoms and the disease may also cause frequent cold, rashes and allergies due to immune system deregulation.

4. The delicate or minute fatty liver symptoms are many as said earlier and they are very broad. Fatty liver disease symptoms include dizziness, coated tongue, (first fatty liver symptoms in most of the cases), tiredness, and the formation of circles under the eyes, a feel of bloating, headaches. The liver is enlarged in some cases. The enlarged liver may felt by pressing on the abdomen and by palpating. This enlarged liver is smooth normally during the earlier stages and it does not have any pain or tenderness.

5. Generally, when excessive alcohol intake is not stopped, or the underlying causes are not identified earlier, the liver gets changed, the liver can progress and lead to the development of other liver related diseases. These include alcoholic liver cirrhosis and alcoholic hepatitis.

6. Alcoholic liver cirrhosis is in its advanced stage, the liver becomes scarred, hardens and shrinked. in alcoholic hepatitis, the liver is inflamed and necrosis of liver tissue occurs due to alcohol. it is the fact that alcoholic cirrhosis and alcoholic hepatitis may occur together at the same time.

7. in the above disease, the skin and eyes become yellow in color, poor appetite, fatigue, weight loss and weakness appears. Pain originates from the right upper side of the stomach. in some cases bleeding occur, bruising, leg swelling (edema), swelling in abdomen (ascites) appears.

8. Portal hypertension is another symptom (increased blood pressure in the abdomen) and it causes the veins of the esophagus to swell. These swelling may result in burst and internal hemorrhage. The liver damage in severe cases causes failure to filter toxins from the blood resulting in brain damage called hepatic encephalopathy.

9. Advanced liver disease complications end in liver failure and kidney failure which are life threatening symptoms and in very rare cases liver cancer is caused.

Fatty liver treatment:

It is easy to start the fatty liver treatment once the disease is diagnosed. The fatty liver treatment could be classified in to four easy steps and the fifth one is optional.

1. The first step is very simple. it is nothing but removing the cause of the fatty liver disease symptoms. The causes may vary from individuals. it may be alcohol or caffeine. it may be a drug prescribed by the doctor. (A care should be taken before withdrawing the drug). Other than these things it may be due to recreational drugs or even homeopathic medicines.

2. The next step or the second one is with the diet and its adjustments. The diet should be modified for the patients who are having the fatty liver disease symptoms. The diet should have rich vegetables, fruits and it should have only a little quantity of animal products. These types of diet will reverse the liver damage. certain all other methods could only accelerate the process in advanced cases.

3. The third easy step in the fatty liver treatment is exercise. This third step is very helpful in burning the fat and as soon as the third step is started, it signals the liver to start burning the fat and the fat inside the cell is burnt.

4. The fourth step of the fatty liver treatment is using some herbal supplements. This enables the damaged liver to regenerate. The examples of herbal supplements are milk thistle, turmeric, ginger and many herbal products are available as supplements as well.

5. The fifth and final step in the fatty liver treatment is liver detoxification (reduction of the toxic properties of a substance and treatment designed to assist in recovery from the toxic effects of a drug). Radiocarpal syndrome

Those who are busy working with a computer, i.e. working with a keyboard, should beware that as a result of the uneven and strong tension of tendons in the wrists, a radiocarpal syndrome may develop. Tendons eventually begin to swell, squeezing the nerve, which leads to pain and a slow death of the nerve. the work of typists, seamstresses, knitters, pianists leads to such effects, because their work involves the presence of small, reiterated movements, repeated with short intervals.

to protect yourself from the development of radiocarpal syndrome your workplace should be well-appointed. It is necessary to choose the height of your desk so that when you sit at it, your arm bent at the elbow forms a right angle. you should also adjust the working regime so that you can take breaks every hour or two. during these breaks you should do special exercises to stretch the wrists and fingers.
