A 23-year-old pregnant woman hailing from Dhule, who had tested positive for H1N1 virus, died on Thursday at city?s JJ Hospital, a day after she delivered a baby there. following the death, the doctors have said that pregnant women should especially be cautious as they fall under the high-risk group.

?The immunity levels in such women are already compromised owing to their pregnancy. they should be careful and should stay away from anyone who shows flu-like symptoms such as cold or cough. in case, they start showing symptoms themselves, there should be no delay in starting a treatment before the situation gets aggravated,? said Dr Abhay Chaudhary, director of Haffkine?s Institute.

Chaudhary said that testing for the flu is not necessary and people can start taking Tamiflu as a preventive measure on showing flu symptoms. Mumbai has seen 14 positive cases of swine flu over the past one month.

Meanwhile, public health minister Suresh Shetty said there are no reports of mutation in the H1N1 virus but promised to launch an extensive awareness drive at railway and bus stations Speaking in the Legislative Council, Shetty said National Institute of Virology, Pune and CMC, Vellore have not reported any mutation in the strain of the virus. He said there was stock of 1.8 lakh Tamiflu tablets in Mumbai and 11.5 lakh tablets in rest of Maharashtra.

Asked about the availability of swine flu vaccines in the state, Shetty said the Centre had supplied vaccines to medical practitioners and the use of vaccine was not recommended for general public. ?The vaccine is effective for eight to ten months,? said Shetty.

Incompetent cervix is a condition during which the cervix of a pregnant woman opens before the completion of gestational period and is mainly because of the weakness of the cervix or thinning or shortening of the cervical wall.

This condition leads to premature birth and may even leads to the loss of baby during the second trimester. When the cervix is weak then it is difficult to hold the weight of the baby, fluid and the placenta inside the womb.

This is because when the baby grows it will exert more pressure on the cervix and then the cervix dilates on its own without having any contractions of the uterus and pain.

This dilation of cervix leads to the bulging of amniotic membranes through the opening and finally ruptures the amniotic membrane and this stimulates the uterus and results to pre-term labor. Cervical incompetence is the major reason for most of the pregnancy losses in second trimester and is noticed mostly when the labor is most advanced and can’t stop.

Causes of Incompetent Cervix

Those who had a previous cervical surgery are more prone to develop cervical incompetence. Similarly women exposed to DES (diethylstilbestrol) or had a previous pregnancy with incompetent cervix, cervical anatomic abnormalities etc are also at greater risk of having incompetent cervix.

Women having a family history of incompetent cervix or shortened cervix are also at risk. Any damage to cervix due to a D&C, difficult birth, cervical cautery, cone biopsy etc may also cause this condition.

Symptoms of Cervical Incompetence

It is difficult to detect the cervical incompetence before or during the first trimester of pregnancy. usually women have silent cervical dilation that has minimal uterine contraction and is difficult to sense and this happens mainly during 16th week to 28th week of pregnancy.

When the cervical dilation are less than 4 cm, symptoms won’t be pronounced, but when it exceed to 4 cm or more then significant uterine contractions occur and may end up with the rupture of amniotic membrane.

Cervical incompetence can be diagnosed either manually or through ultrasonography. if the cervical opening is 2.5 cm or more or when the cervical length is less than 20 mm the chances for cervical incompetence are more.

If the incompetent cervix is detected earlier in pregnancy or during earlier abortions then the doctor recommends cervical cerclage which is a process of stitching the cervix and is done in between 14 to 16 weeks of pregnancy.

This stitch helps to reinforce the cervix and continue the pregnancy and is usually removed between 36 to 38 weeks of pregnancy. Cervical cerclage helps about 80 to 90% women to give birth to healthy children.

Acute pancreatitis is usually caused by excess alcohol consumption or a disease of the gall bladder or the bile duct. Acute pancreatitis symptoms in pregnant women can be because of two reasons, viz. increased estrogen effect and blockage of the pancreas by gallstones. At least 1 out of 3,333 pregnant women are known to pick up this syndrome and the symptoms are usually present throughout the pregnancy.Acute Pancreatitis Symptoms in Pregnant Women: As you can see, the incidence of acute pancreatitis in women is rare. But a pregnant woman who develops this disease can have the following symptoms-

• mid epigastric pain, pain in the central abdomen due to the presence of gallstones
• radiating pain in the left flank, usually towards the top
• nausea
• vomiting
• low temperatures and symptoms of fever
• anorexia (an eating disorder)
• pulmonary issues which can also lead to respiratory distress syndrome with the symptoms being jaundice, tenderness in the abdominal region, hypercalcemia (presence of excess calcium in the blood) and muscle rigidity.

It is very important to diagnose the underlying pancreatitis disease in women especially in the first trimester as a mis-diagnosis can cause death or result in fetal loss.

Reasons for the Disease: The most common cause of the disease in pregnant women is the presence of gall stones which obstruct the pancreatic duct. This condition is known as cholelithiasis. another very common reason is the hormonal imbalance of estrogen which can lead to high triglyceride (fat) levels. High triglyceride levels block oxygen from reaching the pancreas, thus causing pancreatitis.

Apart from the above two mentioned reasons, alcohol consumption during pregnancy, reaction to medications, and pancreatic duct trauma can also cause pancreatitis.

Treatment: The treatment of this condition in pregnant women is similar to that in non-pregnant women.

• Eating should be strictly avoided in order to rest the digestive tract which will also reduce the pain. instead, fluids would be given intravenously.
• If it is the gallbladder that is causing the problem, it needs to be removed, but only after the pregnancy. during this time, a stent is placed in the bile duct for temporary relief.
• If it is the triglycerides that are causing the problem, then diet restrictions are necessary to help prevent further attacks.

If a pregnant woman has to exhibit any of the above mentioned acute pancreatitis symptoms, then it is important to diagnose the condition and take the necessary precautions. Though very rare, maternal death can be a certainty if the condition is misdiagnosed.

Reflux, as heartburn is commonly called, can be one of the most frightening moments of early pregnancy, especially when the woman is unaware that it may occur at any time. the searing pain often arrives without warning and the burn can grow even more intense very quickly. Then, just when she feels about to ignite, the pain crests and fades gently away… only to rear it’s fiery head again a few minutes later. Heartburn should never come as a surprise to a pregnant woman, and that’s why I am writing about the syndrome in great detail today.

Gastro-esophageal reflux, the flow of gastric content back from the stomach into esophagus is associated with heartburn and is one of the most common complaints in pregnancy. the symptoms are generally limited to pregnancy and other than the associated discomfort, have no adverse effects on the mother or the developing fetus.

Incidence of Heartburn in Pregnancy

• this condition is extremely common and affects an estimated 80% of all pregnant women.• In the majority of cases reflux appears for the first time during pregnancy. In some however pregnancy exacerbates a pre-existing condition.• the incidence of heartburn is similar in the first or subsequent pregnancies.• Women with previous reflux during a pregnancy are more likely to have it again.• Caucasian women are likely to experience reflux earlier than black women.

Symptoms often begin early in pregnancy. However, reflux becomes notable in the 5th month and gradually worsens as pregnancy progresses due to the enlarging uterus.

In the last month as the fetal head descends into the pelvis, symptoms often improve. the majority of women experience complete resolution of their symptoms after delivery.

Causes of Heartburn in pregnancy

Reflux is caused by a combination of increased abdominal pressure and weakening of the muscle band constricting the lower esophagus (sphincter). the enlarging uterus compresses the stomach and increases the pressure within it. this increased pressure pushes fluid from the stomach into the esophagus. at the same time, the muscle band between the stomach and the esophagus, which normally prevents backward flow, becomes lax due to the effect of pregnancy hormones (estrogen and progesterone) and thus allows the stomach content to flow “backwards”.

Other more controversial explanations for reflux include:• Increased gastric acidity, making the gastric juice more irritating to the esophagus.• Increased stomach volume,• the emptying of the stomach is delayed during pregnancy,• Weakening of the muscle band between the stomach and intestine (pyloric sphincter) allowing bile to regurgitate from the small bowel into the stomach and from there into the esophagus.

Symptoms

Heartburn: the most common complaint. It presents as a sensation of burning in the chest that travels from the central part of the rib cage to the neck. Heartburn is more common and becomes more severe in the later months.Regurgitation: Passage of fluid from the stomach into the esophagus and occasionally into the mouth is extremely uncomfortable and causes significant distress. It worsens after eating and especially when reclining immediately after a meal.Less frequent symptoms include: Difficulty swallowing, chest pain, cough, wheezing, sore throat, hoarseness, asthma, ear, nose and throat abnormalities.

Diagnosis

Due to pregnancy and risk to the fetus, the diagnosis is often made by history alone. Endoscopy, telescopic visualization of the esophagus and stomach, which is usually done in the non-pregnant state, is deferred unless there are severe symptoms unresolved by routine measures. Severe symptoms may include bleeding, severe esophagitis, or strictures. (narrowing of the esophagus) other diagnoses that can present similarly such as peptic and duodenal ulcer disease, H. pylori infections, Zollinger Ellison syndrome, and gallbladder disease should all be ruled out. Women with GERD during pregnancy may be more susceptible to complications when anesthesia is administered during labor or for emergency situations due to chemical pneumonias that can occur from aspiration of gastric contents. Therefore, anesthesiologists will administer medication in labor for prevention.

Active management of heartburn can improve a pregnant woman’s quality of life, therefore my next blog will review several effective therapies.

Mother-of-four Fallon Devaney, 25, was struggling for breath and was admitted to the Queen's Medical Centre in Nottingham.

Her mother, Linda Fearney, said she had flu symptoms for more than a week and that the hospital told her she had swine flu.

Ms Fearney told the Derby Telegraph newspaper she was taken aside by a doctor and told that either miss Devaney, the unborn baby, or both could die.

Miss Devaney, from Kirk Hallam, Derbyshire, is five months pregnant with her fifth child and is in intensive care, a hospital spokesman said. "We have a no-change situation, meaning she is still critical but stable," he added.

Ms Fearney told the Derby Telegraph a GP visited her daughter, who has not been vaccinated against swine flu, at her home and diagnosed her with influenza and gave her antibiotics.

But her condition deteriorated and she was admitted to hospital last Tuesday.

Hundreds of messages have been sent to miss Devaney through a Facebook group called Get Well soon Fallon.

Figures out earlier this week showed that 17 people have died from flu so far this winter. the Health Protection Agency said 14 deaths were from swine flu and another three from flu type B.

All victims were under 65, including six under-18s. at least eight of the 17 were in an "at-risk" group, although none was pregnant and none had been vaccinated.
