Despite recommendations that pregnant women have tests for certain sexually transmitted diseases, many may not be getting them, a new study suggests.

Researchers found that of nearly 1.3 million U.S. women who had blood work done during pregnancy, only 59 percent were tested for Chlamydia — a common STD that can cause pregnancy complications or be passed on to newborns.

That’s despite the fact that experts generally recommend pregnant women be screened for Chlamydia.

In addition, 57 percent of women in the study were screened for gonorrhea — a test recommended for some pregnant women.

The U.S. Centers for Disease Control and Prevention (CDC) says that all pregnant women should be tested for Chlamydia at their first prenatal visit. The American College of Obstetricians and Gynecologists (ACOG) has said the same since 2007.

As for gonorrhea, the CDC and other groups recommend screening for pregnant women who are at increased risk. that includes women younger than 25 and those living in areas of the country where gonorrhea is common.

“As our study shows, there’s a significant gap between the recommendations and actual practice,” said Dr. Jay M. Lieberman, who is medical director for infectious diseases at Quest Diagnostics inc. and worked on the study.

Quest is a diagnostic testing provider that operates labs throughout the U.S. The company offers STD testing and funded the new study.

Since not all pregnant women are advised to have gonorrhea testing, it’s difficult to tell whether the 57-percent rate in this study is appropriate or not, according to Lieberman.

However, he said, even women who should be screened, based on guidelines, are sometimes not. Of pregnant women ages 16 to 24, 69 percent were tested for gonorrhea, the researchers report in the American Journal of Obstetrics and Gynecology.

Screening plays an important role in catching Chlamydia or gonorrhea because both STDs often have no symptoms, Lieberman pointed out.

“These two infectious diseases are easy to diagnose, and easy to treat and cure,” he said.

But without screening, Lieberman added, many cases will be missed.

If left untreated, Chlamydia and gonorrhea can cause pelvic inflammatory disease, which can lead to infertility or an ectopic pregnancy — a dangerous condition in which the fertilized egg grows outside the uterus.

Both STDs can also infect the baby during childbirth. Chlamydia can cause eye infections or pneumonia in newborns, while gonorrhea can lead to joint infections or serious blood infections.

A limitation of the current study is that it’s based on pregnant women who had blood tests done between 2005 and 2008. and during those years, screening guidelines were in flux; the ACOG recommendation came out in 2007, for example.

“Clearly, this is still evolving,” Lieberman said. But, he added, “there’s also no evidence that screening rates have improved.”

A 2009 CDC study, for example, found that only a minority of U.S. women who should be screened for Chlamydia actually had been. Screening is advised not only for pregnant women, but for certain other at-risk groups — like women age 25 or younger (see Reuters story of April 16, 2009).

Exactly why some pregnant women are not tested as recommended is unclear. “Our data don’t speak to that,” Lieberman said.

For some women, access to prenatal care is a hurdle, he noted. But the data in this study all came from women already in prenatal care.

Lieberman suggested pregnant women talk to their doctors if they have not been tested for STDs or are not sure if they have had testing. other STD tests done during pregnancy include ones for syphilis and HIV.

Sometimes, Lieberman noted, women are put off by the suggestion that they have STD testing. “But doing these tests is not a judgment on them or their behavior,” he said. “We’re just trying to do what needs to be done to make sure you’re healthy and your newborn is healthy.”

According to the CDC, about 100,000 pregnant women in the U.S. are infected with Chlamydia each year. Just over 13,000 have gonorrhea.

To guard against whooping cough, health officials recommend this vaccination schedule. The shots offer protection against whooping cough, diphtheria and tetanus.

Children age 6 and under should have five shots, including four in their first 18 months. The fifth shot of DTaP should be given between ages 4 and 6.

The infection has three basic stages, and the first 21 days are the most contagious:

1. The first two weeks, it will look like the common cold. Usually those infected notice an occasional cough.

2. Fits of coughing begin, followed by a distinct “whooping” sound, a phase that lasts about six weeks.

3. Recovery starts as the cough begins to die out.

OLYMPIA — Gov. Chris Gregoire opened an emergency fund Thursday to help contain a spreading whooping-cough epidemic, and officials urged residents to get vaccinated against an illness that particularly threatens infants.

Gregoire is making $90,000 in crisis cash available to help strengthen a public-awareness campaign about the need for the pertussis vaccination. The state Department of Health is already looking to spend about $200,000 on the effort.

“Pertussis is very serious, especially for babies,” Secretary of Health Mary Selecky said in a statement. “It’s vital that teens and adults are current on their immunizations because they’re often the ones who give whooping cough to babies.”

The state has also received approval from the federal government to divert some federal cash toward the purchase of 27,000 doses of the whooping-cough vaccine. Those will be available for the uninsured.

Pertussis, or whooping cough, is a highly contagious respiratory illness spread by coughing and sneezing. Infants are particularly vulnerable to whooping cough because they can’t be immunized before 4 to 6 weeks.

So far this year, 20 children under the age of 1 have been hospitalized for whooping cough, according to state data. Last year, two babies in Washington died from the illness.

Babies often get the illness from adults and family members because the shots children get wear off over time. Officials want residents to get a whooping-cough booster, called a Tdap.

Washington has already recorded 1,132 cases of whooping cough this year — about 10 times more than the same time last year, according to disease investigators at the Department of Health. The state is recording more than 400 cases of pertussis each month — four times more than the threshold that state officials consider “epidemic” levels — and Washington is on pace for as many as 3,000 cases in 2012.

Those are numbers that haven’t been seen in decades.

“In my 13 years as secretary this is the first time I’ve had to use the word ‘epidemic’ about disease in our state,” Selecky said.

Health officials say only about 10 percent of cases are typically reported, so the number tabulated by state officials shows only a fraction of the total cases statewide. Skagit County is somewhat of an epicenter this year, with more than 200 cases. That’s a rate about three times higher than in the next nearest county — Jefferson.

The public-awareness campaign will include a radio ad airing statewide, as well as public-service announcements, said health-department spokesman Tim Church. Officials also plan advertisements on Facebook and Google, and they are exploring bus and billboard ads.

On Friday, Selecky and U.S. Sen. Maria Cantwell, D-Wash., plan to ask the CDC to send investigators and epidemiologists to help the state study and contain the epidemic. such a study would help state health officials with their response and identify why the epidemic is growing so quickly, according to Cantwell’s office.

Should pregnancy be treated as a disability?

A recent study by a University of Dayton law professor, Jeannette Cox, asserts that pregnant women should be covered by the Americans with Disabilities Act, to protect them from being fired or forced to perform labor that could be harmful to mother or child. (The paper is forthcoming in March in ?the Boston College Law Review.)

The ADA doesn?t recognize pregnancy as a disability, leaving pregnant women physically and financially vulnerable on the job, concluded Cox, who studies employment discrimination. She found that pregnant women are at risk for losing their jobs when ?reasonable adjustments? aren?t made, such as retail workers fired for drinking water at work or pregnant police officers forced to perform rigorous assignments (while injured officers were given lighter duty).

Cox argues that the ADA currently covers people with ?minor temporary physical limitations comparable to pregnancy’s physical effects.” under current ADA protections, disabled workers may forgo tasks involving heavy lifting, repetitive bending, reaching, prolonged sitting or standing or working under high-temperature conditions. not so for pregnant women.

The women most affected are low-income workers who are more likely to work in physically demanding jobs than higher-paid desk jockeys, Cox found. Lower-income workers often face the dilemma of continuing to work in high-risk jobs or leaving work altogether, forgoing income.

At this point, Cox? argument is merely academic. Extending the ADA?s protections would involve significant legal and political hurdles, and business groups would probably oppose such measures as costly, burdensome and over-reaching. In addition, some women may object to pregnancy being classified as a disability because it may signal to employers that they are less fit to work. (There are some six million pregnancies a year in the U.S., according to the American Pregnancy Association.)

Courts, meanwhile, have reasoned that the ADA does not require employers to accommodate pregnant employees because judges have considered “all of these symptoms, at some degree of severity … part and parcel of a normal pregnancy,? Cox wrote.

A new report links remnants of a once-common pesticide to lung infections and wheezing in kids exposed to the chemical before birth.

Known as DDE, the compound is a broken-down form of the harmful pesticide DDT and is found in many places around the world. it is absorbed into a person’s body when they eat contaminated food or breathe contaminated dust.

The study, published in the European Respiratory Journal, shows babies exposed to high levels of DDE in the womb grew up to have higher rates of pneumonia and bronchitis.

“We found that the risk of infections and wheeze increased with increasing DDE exposure,” Martine Vrijheid, associate professor at the Centre for Research in Environmental Epidemiology in Spain, told Reuters Health by email.

Vrijheid and her colleagues took blood samples from a large group of pregnant women in Spain, measuring the women’s exposure to three different pollutants.

Later, when the babies were about a year old, the researchers asked the mothers whether their toddlers had had breathing trouble or lung infections.

About 35 percent of the 1,342 children developed a lung infection within their first year, the team found, and the risk rose in tandem with the DDE levels in the mother’s blood.

Vrijheid estimates that with undetectable or very low levels of DDE, only 30 percent of the children would have developed infections.

The new study is the largest of its kind, according to the researcher. Past work has yielded mixed results, and until now has failed to show a significant association between DDE exposure and lung infections within a child’s first year of life.

DDT was once widely used to kill mosquitoes and prevent malaria outbreaks, but its use was called into question in the 1960s and was banned in the U.S. in 1973. But it and its breakdown products can last in the environment for hundreds of years.

People exposed to large amounts of DDT may experience tremors, seizures, sweating, headaches and vomiting, according to the Centers for Disease Control and Prevention. But those symptoms disappear when the person is no longer exposed to the pesticide.

Barbara Cohn, the director of Child Health and Development Studies at the Public Health Institute in California, told Reuters Health that DDT and its remains have been hard to study because of the time needed for it to show up in people.

“People thought it was a miracle compound,” said Cohn. “Nobody really knew what might be happening.”

DDT is still used in some parts of the world, because it is considered the best way to kill malaria-carrying mosquitoes, said Vrijheid.

In 2006, the World Health Organization sanctioned an altered technique for using DDT, which involves spraying the pesticide on the inside of a building to kill mosquitoes that land on its walls and other surfaces.

Vrijheid told Reuters Health that it’s hard to avoid chemicals like DDT and DDE, which are stored in fat and found in higher concentrations at the top of the food chain.

“One way of reducing exposure is by reducing the consumption of fatty foods and increasing the consumption of vegetables and fruits,” she said. “This does not mean we should stop eating meat, yogurts or certain types of fish, but just that we should have (a balanced) diet.”

Check-ups during pregnancy tend to focus around the waist. But there’s growing debate about which mothers-to-be should have a gland in their neck tested, too.

Numerous studies since 1999 have found that an underactive thyroid can raise a woman’s risk of miscarriage, premature birth, or a lower IQ for her baby — even if it’s so mildly sluggish that she feels no symptoms.

The problem: while serious cases are treated with a hormone pill, so far there’s little evidence that treating the milder cases makes a difference. So guidelines about who should be tested vary widely.

Now a peek at prenatal testing from one of the country’s largest medical labs suggests that nearly a quarter of pregnant women are getting the simple thyroid blood test regardless of whether they have symptoms.

Researchers at Quest Diagnostics examined records for half a million pregnant women. of those who got tested, a higher-than-expected number — 15 percent — had an underactive thyroid. That’s five-fold higher than some previous estimates, partly because the way in which the condition is diagnosed has changed recently, says the study published by the Journal of Clinical Endocrinology & Metabolism.

The vast majority of those women were in the gray zone, with milder cases where no one knows for sure if a diagnosis helps or wastes money on testing and thyroid medication.

The finding adds pressure for science to settle this long-running controversy.

“We still don’t have perfect answers,” says Dr. Elizabeth Pearce, a well-known endocrinologist at Boston Medical Center, where a recent survey found widespread prenatal thyroid testing. But, “if it’s my patient in my office, or it’s me or my family member, I’m going to treat every time.”

Obstetricians seem more wary.

“There are studies on both sides of the fence,” says Dr. Dena Goffman of New York’s Montefiore Medical Center, which tests only women at high risk. “If you don’t know what to do with the results, you probably shouldn’t order the test,” she said.

The unassuming thyroid — a small bow tie-shaped gland nestled in the front of the neck — plays a big role in good health for everyone. it produces hormones that regulate metabolism and can affect almost every type of tissue in the body. About 20 million Americans are estimated to have a malfunctioning thyroid that, if serious enough, can contribute to heart disease, bone-thinning osteoporosis and infertility.

An overactive thyroid, called hyperthyroidism, speeds up bodily functions, causing such symptoms as weight loss, nervousness, anxiety and increased heart rate and vision problems.

Much more common is an underactive thyroid, called hypothyroidism. it slows body functions, causing such problems as fatigue, weight gain, depression, constipation and dry skin. it even can contribute to high cholesterol, according to the National Institutes of Health.

Thyroid problems increase with age, but they affect far more women than men — and pregnancy puts extra stress on the gland.

Having enough thyroid hormones is important for fetal brain development, especially during the first trimester, when the fetus depends solely on the mother for them. the hormones also play a role in avoiding miscarriage or premature birth.

Mothers also may harbor immune system cells called antibodies that subtly attack the gland and likewise are linked to miscarriage and prematurity. Italian researchers found that treating those women lowered their risk of encountering this problem.

There is broad agreement that women with overt hypothyroidism — a seriously underactive gland — should be treated, most likely given a once-a-day hormone pill long known to be safe in pregnancy. But it takes blood testing to diagnose overt disease because even those women don’t always report the vague symptoms.

Those $25 blood tests are sure to uncover women with mild hypothyroidism, too, the people in the so-called gray zone. Some research has raised questions about whether mild cases really pose a pregnancy risk, and preliminary results from a large British study recently found no overall IQ benefit to the resulting children if their mothers had been treated.

But the damage might already have been done by the time treatment began late in the first trimester, notes Boston’s Pearce. in the U.S., doctors are anxiously awaiting a similar National Institutes of Health study; results aren’t expected until 2015.

What’s the advice until then? the American College of Obstetricians and Gynecologists recommends testing only pregnant women who have thyroid symptoms, have had previous thyroid problems, or have similar autoimmune diseases such as Type 1 diabetes — those considered at risk for overt disease.

The American Thyroid Association goes further. last summer it advised also testing all pregnant women age 30 and older, and those with enlarged thyroids, previous pregnancy problems or those who are obese, says Pearce, who co-authored the guidelines.

Most guidelines cite the lack of evidence for treating mild cases. the thyroid association does urge treatment if those women harbor the worrisome antibodies.

A final tip: Pregnant women should check that their prenatal vitamins contain iodine, important for proper thyroid function, Pearce says. not all do. Most Americans get enough iodine from dairy products, bread, seafood and iodized table salt. But women need extra during pregnancy.

Crohn’s drug penetrates placenta Pregnant women taking a drug prescribed for Crohn’s disease may pass the medication on to a newborn through the placenta, a U.S. team of researchers from California and Georgia reported may 7. The drug, adalimumab (Humira), binds to and neutralizes an inflammatory immune protein called TNF-alpha, short for tumor-necrosis factor alpha. The drug turned up during the first three months of life in the blood of three of five children born to women taking it and in umbilical cord blood from all five newborns. The study authors say the findings don’t warrant stopping the drug during pregnancy, since it limits miscarriages and preterm births. But since adalimumab suppresses immunity, newborns exposed to it should be monitored for infections and not receive live-virus vaccines for the first six months. —Nathan Seppa

High fructose linked to pregnancy gallstones a diet high in carbohydrates, particularly fructose, may increase a woman’s risk of developing gallstones during pregnancy, researchers at the University of Washington in Seattle reported may 8. Ultrasound examination of more than 3,000 women during pregnancy showed that 10 percent had developed gallstones or biliary sludge, a particulate material in bile that has been linked to gallstone formation. Questionnaires revealed that women with diets highest in carbohydrates were roughly twice as likely to have gallstones or biliary sludge as were those consuming the lowest amounts, with fructose accounting for a greater risk than starch. The researchers adjusted for differences in weight, diabetes status and other dietary intake. —Nathan Seppa

Diabetes patients not getting hepatitis shots People with chronic liver disease or type 2 diabetes have poor rates of vaccination against hepatitis a and B, even though they are at risk of developing very severe disease from these viral infections. Researchers at Inova Fairfax Hospital in Falls Church, Va., analyzed data on several thousand people with either diabetes or liver disease. among the diabetes patients, vaccination rates were only 16 and 22 percent against hepatitis a and B in data from 2005 to 2008. Vaccination rates in people with chronic liver disease reached 32 percent for hepatitis B but only 20 percent against hepatitis a. The percentages are similar to those seen in the general population.  most chronic liver disease is due to fatty liver disease not related to alcohol. —Nathan Seppa

Very early obesity therapy works Obese children as young as age 2 benefit from therapy to reduce weight and improve fitness, researchers at Indiana University School of Medicine in Indianapolis reported may 8. The scientists recruited 462 obese children ages 2 to 21, including 44 who were age 2 to 5. All the children and family members received intensive therapy on behavioral techniques such as goal-setting, self-monitoring, positive reinforcement and stimulus control. Children at all ages showed weight loss and better fitness, but the youngest children exceeded the older kids. both groups showed metabolic improvements. Endurance athletes face GI problems College athletes engaged in swimming, crew and lacrosse have more gastrointestinal problems than do athletes playing less grueling sports, researchers at Drexel University in Philadelphia have found. The scientists obtained questionnaire data on health and lifestyle from 107 Division I college athletes and 108 non-athletes. The non-athletes were more prone to irritable bowel syndrome; otherwise there was no difference between the groups in digestive function. But 52 percent of athletes participating in one of the three high-intensity sports reported at least one gastrointestinal disorder, compared with 34 percent of college athletes in lower-intensity sports. Athletes in tennis and diving were among those with the fewest symptoms, the researchers found. —Nathan Seppa

Q. I am six months pregnant and have developed shooting pain down my leg, which I think is sciatica. What are the causes, and what course of treatment should I consider during my pregnancy?

It’s not unusual for pregnant women to have multiple maladies that temporarily manifest themselves then disappear after pregnancy. Some pregnant women may suffer from sciatica, which is caused by irritation of the sciatic nerve.

The sciatic nerve is actually a cluster of nerves that exits the lumbar spine, enters the buttocks, and then travels down the leg into the foot. for this reason, a major symptom of sciatica is shooting pain from the buttock down the back of the leg into the foot. The pain can vary from moderate severity to completely disabling.

The pain is usually caused by compression of the sciatic nerve and is commonly the result of a degenerative or herniated disk. While most cases clear up in time, patients suffering from extreme pain or loss of bowel and bladder function may require urgent surgical intervention to address the source of the issue.

If a herniated disk is indeed the cause of a patient’s sciatica, rest, pain management and physical therapy interventions such as core strengthening exercises are the first steps toward recovery. Normally the pain and symptoms will go away after pregnancy, but in a small percentage of patients these symptoms could persist or recur at a later time.

If the patient’s pain and quality of life has not improved after the pregnancy, the patient should consult her physician or a spine surgeon for an accurate diagnosis and a review of surgical and non-surgical treatment options.

They may elect a minimally invasive procedure called a microdiscectomy or microdecompression surgery to remove the protruding part of the disk. A slightly more invasive procedure, referred to as a lumbar laminectomy, entails a bigger incision and involves manipulating more tissue around the disc. an additional cutting-edge surgery for patients with severely damaged discs is the artificial disc replacement where the entire disc is replaced with a prosthesis.

Patients should consult their physician and/or a spine surgeon to determine the best treatment options.
