Despite recommendations that pregnant women have tests for certain sexually transmitted diseases, many may not be getting them, a new study suggests.

Researchers found that of nearly 1.3 million U.S. women who had blood work done during pregnancy, only 59 percent were tested for Chlamydia — a common STD that can cause pregnancy complications or be passed on to newborns.

That’s despite the fact that experts generally recommend pregnant women be screened for Chlamydia.

In addition, 57 percent of women in the study were screened for gonorrhea — a test recommended for some pregnant women.

The U.S. Centers for Disease Control and Prevention (CDC) says that all pregnant women should be tested for Chlamydia at their first prenatal visit. The American College of Obstetricians and Gynecologists (ACOG) has said the same since 2007.

As for gonorrhea, the CDC and other groups recommend screening for pregnant women who are at increased risk. that includes women younger than 25 and those living in areas of the country where gonorrhea is common.

“As our study shows, there’s a significant gap between the recommendations and actual practice,” said Dr. Jay M. Lieberman, who is medical director for infectious diseases at Quest Diagnostics inc. and worked on the study.

Quest is a diagnostic testing provider that operates labs throughout the U.S. The company offers STD testing and funded the new study.

Since not all pregnant women are advised to have gonorrhea testing, it’s difficult to tell whether the 57-percent rate in this study is appropriate or not, according to Lieberman.

However, he said, even women who should be screened, based on guidelines, are sometimes not. Of pregnant women ages 16 to 24, 69 percent were tested for gonorrhea, the researchers report in the American Journal of Obstetrics and Gynecology.

Screening plays an important role in catching Chlamydia or gonorrhea because both STDs often have no symptoms, Lieberman pointed out.

“These two infectious diseases are easy to diagnose, and easy to treat and cure,” he said.

But without screening, Lieberman added, many cases will be missed.

If left untreated, Chlamydia and gonorrhea can cause pelvic inflammatory disease, which can lead to infertility or an ectopic pregnancy — a dangerous condition in which the fertilized egg grows outside the uterus.

Both STDs can also infect the baby during childbirth. Chlamydia can cause eye infections or pneumonia in newborns, while gonorrhea can lead to joint infections or serious blood infections.

A limitation of the current study is that it’s based on pregnant women who had blood tests done between 2005 and 2008. and during those years, screening guidelines were in flux; the ACOG recommendation came out in 2007, for example.

“Clearly, this is still evolving,” Lieberman said. But, he added, “there’s also no evidence that screening rates have improved.”

A 2009 CDC study, for example, found that only a minority of U.S. women who should be screened for Chlamydia actually had been. Screening is advised not only for pregnant women, but for certain other at-risk groups — like women age 25 or younger (see Reuters story of April 16, 2009).

Exactly why some pregnant women are not tested as recommended is unclear. “Our data don’t speak to that,” Lieberman said.

For some women, access to prenatal care is a hurdle, he noted. But the data in this study all came from women already in prenatal care.

Lieberman suggested pregnant women talk to their doctors if they have not been tested for STDs or are not sure if they have had testing. other STD tests done during pregnancy include ones for syphilis and HIV.

Sometimes, Lieberman noted, women are put off by the suggestion that they have STD testing. “But doing these tests is not a judgment on them or their behavior,” he said. “We’re just trying to do what needs to be done to make sure you’re healthy and your newborn is healthy.”

According to the CDC, about 100,000 pregnant women in the U.S. are infected with Chlamydia each year. Just over 13,000 have gonorrhea.

Safe storage: Store all fertilizers, chemicals, tools, etc. in closed cabinets or/and in pet proof containers. Don’t leave bags out for dogs to chew through. Keep all products well-labeled, ideally in the original package. Before purchasing and using, we recommend that you read all labels to understand potential precautions that should be taken and to keep labels for reference if you need to call your vet or poison control.

Safe design: planning a garden overhaul or starting from scratch? Consider non-metal lawn edging to avoid lacerated paws. Review the list of toxic plants on the American Society for the Prevention of Cruelty to Animals (ASPCA) website (aspca.org/pet-care/poison-control/plants) and either avoid planting these plants or ensure your pet won’t have unsupervised access to these potential toxins. A few examples include lilies (many varieties), tulips, gardenias, irises, avocados and grass palms. Learn what plants are currently in your yard and determine if you need to keep your pet away.

Safe training: Consistent, regular behavior training can help you keep your pet safe and away from dangers.

The following list contains some of the most common examples of pet emergencies related to gardening season.

Cocoa mulch: contains theobromine, the same chemical compound found in chocolate, which is toxic to dogs. Dogs may be attracted to the scent of this mulch, and if ingested, it can cause illness. Mild symptoms include gastrointestinal upset e.g., vomiting, diarrhea, and loss of appetite. More severe symptoms can progress to muscle tremors, weakness, seizures and, in extreme or rare cases, death. the toxicity level depends on the amount of the mulch ingested and the size of the dog. best rule of thumb: keep this type of mulch out of muzzle reach of your pooch.

Other types of mulch: (e.g., cedar, pine, fir, and spruce) can contain oils that cause gastrointestinal upset, and in very high concentrations possible neurologic symptoms.

Overconsumption of grass, mulch: Some of our pet companions are simply fascinated with eating anything and large amounts of it. These pets may get themselves into a life-threatening situation by eating so much that it causes stomach bloat or/and intestinal obstruction. These situations may require emergency intervention — including surgery — to cure. These pets should be supervised or not permitted in areas where they might ingest mulch, grass, etc. Working with a trainer may be a tool to help train your dog to avoid this behavior.

Fertilizers or weed killers: Proper use of these substances is key to protecting your pets. Read all warning labels of any products that you are using. Some will indicate that the substance is not harmful to pets or people once it has dried. Some may recommend a certain number of hours or days before coming into contact with treated grass. Be aware of the forecast: if it is going to be a windy day, avoid applying these products. If rain is predicted too soon after you will be applying the product, it would be best to wait. Exposure to these chemicals can cause skin and pad irritation, oral (tongue, gum tissue) irritation if eaten, possibly gastrointestinal upset if swallowed. for cats, permethrin-based products are especially toxic, causing neurologic changes, seizures, death.

Compost ingestion: Compost bins often contain many foods that are toxic to our pets, including coffee and onions. Keep these bins in an area that your pet can’t reach or a place that is well enclosed.

Metal lawn edging and other sharp tools: Many wounds that occur on the paws of our pets can be prevented by eliminating our pets’ exposure to these sharp objects.

Parks and open fields: Watch for flags that indicate recent pesticide usage. Consider consulting with your local grounds maintenance about the products that they use.

If your pet is exposed, the following are general guidelines. In all cases, consultation with your veterinarian is the best recommendation.

Skin or pad exposure: rinse well with water, bathe with mild shampoo (i.e., oatmeal based) to remove oils and chemicals from skin surfaces.

Ingestions: Call your veterinarian for advice. Depending on what was eaten, the amount, and the time frame of ingestion, your vet may counsel you regarding inducing vomiting and emergency treatment, or they may determine that no treatment is needed.

Leading diagnostic test solutions provider Quidel Corporation (QDEL) has won the European CE Mark for its new virus test dubbed the “Quidel Molecular RSV + hMPV Assay” designed for the detection of respiratory syncytial virus (“RSV”) and human metapneumovirus (“hMPV”). RSV and hMPV are two distinct viruses that cause respiratory illness with very similar symptoms.

the Quidel Molecular RSV + hMPV assay is among the several promising upcoming assays from the California-based company’s expanding molecular diagnostics programs. it is the company’s third molecular diagnostic test to be cleared for marketing in Europe. Quidel received CE Mark for the Influenza A+B and hMPV tests (its first and second molecular diagnostic tests, respectively) in August and September 2011, respectively.

Quidel noted that the European approval of the RSV + hMPV assay has opened the path for the launch of another molecular test for infectious disease in Europe. the test is yet to be approved for sale in the U.S.

I was inspired to write this column by a recent patient who swore she was “losing her mind.” She attributed it to either the onset of menopause or the summer homecoming of her college freshman daughter.

Actually, I’ve heard many women claim that their concentration and memory is not the same as it used to be. These are common complaints of women in menopausal transition; changes during menopause can be frightening and may lead some women to think they have early symptoms of dementia. This, fortunately, is usually not the case.

When a woman becomes more forgetful, she may wonder if decreasing estrogen is to blame. there is, however, little evidence that lower estrogen levels affect the brain. These changes are usually due to normal aging or mood. They usually subside after menopause.

However, younger women who have had “surgical menopause” (having had both ovaries removed) may experience immediate and severe menopausal symptoms. Studies have shown that this process can have a negative effect on memory. Mental skills like remembering words can be difficult immediately after surgical menopause.

Since the link between circulating hormone levels and cognitive impairment is weak, hormone therapy for cognitive symptoms or prevention of dementia is not recommended.

As we age, our mental abilities can slow. Also, stress at mid-life can affect sleep. Work demands, teenage children, the “empty nest” syndrome, aging parents, relationships, health concerns and financial woes take their toll.

Lack of sleep adversely influences the ability to concentrate and resultant fatigue during the day can cause you to be less alert and lead to problems at work and daily life.

The good news is that as you age, you have the ability to continue to gain knowledge, skills and expertise.

There are many ways to keep our minds strong. Physical and mental activity and a balanced social life all have positive effects on the mind. They may ward off the effects of aging and disease that can contribute to dementia.

Proper nutrition is also key. Foods that are healthy for your heart are usually also healthy for your brain.

Perhaps the best strategy for cognitive maintenance hews closely to common wisdom: Eat right, exercise (physically and mentally), minimize stress and strive for balance in your life.

Across the nation, and especially in communities that attract a lot of older Americans, the free-love generation is continuing to enjoy an active – if not always healthy – sex life.

At a stage in life when many would expect sexually transmitted diseases to be waning, aging baby boomers are once again busting stereotypes, setting records and breaking rules.

In the five years from 2005 to 2009, the number of reported cases of syphilis and chlamydia among those 55 and older increased 43 percent, according to an Orlando Sentinel analysis of data provided by the Centers for Disease Control and Prevention.

In the Sunbelt where retirees have formed large communities, the rise was even more dramatic.

for instance, in Arizona’s Maricopa and Pima counties – home to large retirement communities just outside Phoenix – the percent of reported cases of syphilis and chlamydia increased twice as fast as the national average from 2005 to 2009. Reported cases were up 87 percent among those 55 and older in those counties.

In Central Florida, where the Villages and other retirement communities sprawl across several counties, reported cases of syphilis and chlamydia increased 71 percent among those 55 and older in that same period. and South Florida saw a 60 percent rise in those two sexually transmitted infections among the same age group, according to the Florida Department of Health.

In Riverside County, Calif., home to retirement Mecca Palm Springs, reported cases were up 50 percent over the five-year span, according to data from that county’s health department.

the reported cases of syphilis and chlamydia among older adults outpaced the nation’s average, according to the analysis. Among all age groups nationwide, reported cases of syphilis increased 60 percent between 2005 and 2009, while in the 55 to 64 age group it increased 70 percent. Meanwhile, the incidences of chlamydia rose 27 percent among all ages, and double that among those age 55 to 64.

as a result of the national trend among seniors, Medicare is considering providing coverage for STD screenings for seniors. In March, the Centers for Medicare and Medicaid office announced that it was looking into adding STD exams to the national health-insurance program, which already pays for HIV screenings. Medicare also is weighing the benefits of paying for behavioral counseling for sexually active seniors.

the factors driving the rise of STDs in the older set include Americans living longer, healthier lives and a new class of medications, which include Viagra, making more sex possible. many older adults didn’t get the safe-sex messages that younger generations received, say experts, so their condom use is lower. In addition, more seniors are living in group retirement communities where there’s more socializing.

“These seniors may lose a spouse, then get lonely,” said Dr. Jason Salagubang, a geriatrician on staff at Florida Hospital Apopka. “They’re living in retirement communities with others in the same boat, and sparks fly.”

Joanne Williams, director of Baltimore County Department of Aging, gets that and takes a proactive approach to STD education at the 20 senior centers she oversees.

“The discussion certainly happens here,” Williams said. She applies a portion of the funds her agency receives from the state for health education promoting safer senior sex and getting tested.

Julia Gill, director of the Division of Disease Control for the Florida Department of Health, said the heavy marketing for sex-enhancing pharmaceuticals aimed at seniors is also pushing the trend.

besides Viagra, other medications such as hormone replacements are helping seniors remain sexually active longer. Progesterone and estrogen creams help make sex more comfortable for women, while testosterone-replacement drugs boost libido in men and women.

Finally, a more open sexual attitude has contributed to the rising infection rate.

“The flower children who were in their 20s back in the 1960s are now in their 70s,” said Salagubang. “They’re the make-love-not-war generation, and old habits die hard.”

Anna Fowlkes is of that generation. five years ago, the 64-year-old widow, mother and grandmother from Baltimore, tested positive for HIV. She picked it up from a man she dated whom she’d known since her childhood. “We grew up together. We’d started going to church together. I never thought to ask,” Fowlkes said.

Today, Fowlkes reaches out to other seniors by speaking to groups about her experience and showing a brief video she produced called “Senior Dating: Older, Wiser, Safer.”

Just because seniors are older and wiser doesn’t mean they’re not susceptible to the same diseases as everyone else, Salagubang said. In fact, they’re more susceptible.

as people age, their immune systems tend to weaken, and other health problems make them more prone to infection. Medications for heart disease, hypertension and diabetes also cause seniors to be more likely to pick up what’s going around.

because STDs often have no symptoms, they frequently go untreated and make seniors more prone to other infections, Salagubang said. and these infections will make other conditions, such as diabetes or heart disease, worse.

“If I think a patient may be sexually active, I suggest he or she gets screened,” Salagubang said. “I let patients know that STDs and HIV are on the rise among seniors and are a lot more common than many seniors think.”

although older Americans account for a relatively small proportion of new STD diagnoses overall, providing them education and services to help protect them from infection is critical, said Rachel Powell, CDC spokeswoman.

“Many older Americans face unique prevention challenges, including discomfort in discussing sexual behaviors with physicians and partners, and discomfort discussing condom use,” she said.

given the changing sexual climate for seniors, Dr. Connie Micklavzina, a gynecologist at Winnie Palmer Hospital for Women & Babies in Orlando, Fla., has started asking her older patients more questions, including whether they would like to be screened for sexually transmitted diseases.

“Often I see a huge look of relief on their faces, because they are too embarrassed to ask,” said Micklavzina, who’s been in practice 25 years. “The responsibility of bringing this up should be on the practitioner, not the patient, to make the conversation easier.”

But according to what she hears from seniors, Fowlkes said, that’s not happening. “Doctors aren’t asking seniors if they are sexually active. and if doctors don’t ask, seniors don’t volunteer the information.”

Micklavzina also broaches the subject of condom use with patients. “I’m surprised by the number of women in their 50s and 60s who aren’t insisting that their partners use condoms.”

a study by sex researchers at Indiana University found that in the United States, condom use was lowest among men 50 and older. Men in their 50s reported using a condom only 28 percent of the time with a casual partner. by comparison, men in the 18-to-39 age range used a condom with casual partners at least 50 percent of the time.

Marcia Suss, a well-informed 68-year-old from Brooklyn, simply insists. “You have to be very careful because you don’t know where the person you’re going out with has been,” she said. “If the person is not willing to get tested or wear a condom, then I’m not interested. I’m not looking to pick anything up because it’s forever.”

“We often assume that younger people are at greater risk for sexual health challenges like HIV and other sexually transmitted disease,” said Michael Reece, director of Indiana University’s Center for Sexual Health Promotion. “However, these data suggest that younger Americans are using condoms more consistently than older Americans.”

the fact that older Americans aren’t worried about preventing pregnancy accounts for some of the lower condom use, but this age group clearly needs more education about the risks of unsafe sex, Reece said.

According to a 2007 study reported in the New England Journal of Medicine, 67 percent of men and 39 percent of women aged 65 to 74 surveyed reported having had sex in the previous year; 38 percent of men aged 75 to 85 reported the same.

“We should not be profiling people based on their age, and making assumptions about their sexual activity,” said Dr. Stacey Lindau, ob-gyn at the University of Chicago and author of the study. “Removing age-based profiling with respect to STD screenings is a good idea.”

Statistically, in both human and animal medicine, preventive care has a higher ratio of positive outcomes than reactive. in addition, early detection and treatment of disease or illness provide a much higher comfort level for people and their pets. it is also a less costly way to care for yourself and your companions. think about it: If you identify a problem early on, it is less advanced, and there are fewer complications. the sooner treatment is initiated, the greater the likelihood of a good recovery, and the less likely there will be secondary illnesses.

There are several disease processes that I see in practice on a daily basis in which the clients who participate in preventive efforts for their companions not only greatly improve the quality of life for their dogs and cats but also greatly minimize their veterinary expenses.

Heartworm prevention is often neglected. Heartworm is a life-threatening disease that is passed to both dogs and cats from mosquitoes. it is an issue in our area, and indoor pets also are at risk. There is no treatment for cats. for dogs, the treatment involves the use of arsenic, and is risky and costly. Patients need to be strictly cage-confined for several weeks following medications. for a large breed dog, treatment is almost $2,000. A year of preventive care with Heartgard for a small dog is about $55. That is less than $5 per month. for a large dog, it is less than $7 a month. Trifexis is another preventive option – it is oral and also includes flea protection. these same products also protect against some internal parasites.

Routine exams at least once a year for every pet are really important. in addition to identifying disease in the early stages, this also helps to make sure your companion is receiving appropriate care for his or her particular life stage. As your companion becomes a senior, twice-yearly evaluations as well as laboratory panels are recommended. Remember that animals age much faster than we do, and changes in organ systems can happen rapidly. Animals, especially cats, are very good at hiding illness. often you will not see symptoms of a problem until it is advanced. it is all about identifying issues in the early stages, because early detection of illness leads to better outcomes.

Dental care is a huge problem in veterinary medicine, with 80 percent of patients having some level of dental disease. it often goes undetected, and many clients are fearful of having their pets sedated for dental cleanings. Dental disease is not just confined to the mouth. As oral infections progress, bacteria from the mouth goes into the bloodstream and affects organs such as the kidney, liver and heart. There are some easy steps you can take today to help prevent your dog or cat from developing significant dental disease. first, feed a high-quality diet and eliminate or limit people food. A good diet will leave less residue on the teeth. Provide some level of routine oral care; there are many options available including brushing; treats and chews; oral drops and rinses; oral paste; and products to add to water. Schedule dental cleanings at the earliest point that this procedure is recommended. When a cleaning is done early on in the disease process, the anesthetic time is shorter, extractions are less likely, and it slows down the progression of oral pathology so your cherished companion experiences less pain overall.

Do not feed your pets people food. This is a common practice that frequently results in vomiting, diarrhea, decreased appetite, and generalized digestive upset. Blood work and X-rays are commonly taken. Antibiotics and a bland diet are common treatments. Many patients have to be hospitalized for pancreatitis and other problems. This is a lot to spend and have your companion go through because he or she shared your holiday ham.

Not only is important to try to prevent disease, but also to act quickly when a problem arises. This helps minimize the chance of secondary illnesses and provides comfort for your dog and/or cat as soon as possible. again, when problems are treated in the early stages, there is a much greater chance for a positive outcome. By acting on problems in the early stages, you often minimize costs because often a shorter cycle of treatment is needed. One of the best ways of detecting cervical cancer is to undergo routine/ regular cervical cancer screening tests. The cervical cancer screening test (i.e. the pap test or the pap smear test) is one of the best ways of catching cervical cancer in early stages. This test is not at all painful and just involves brushing off of cells from your cervix for microscopic examination. The pap test results are generally given as a rating on a scale of 1 to 5 where in 1 indicates normal cervix cells and 5 indicates serious cancer signs. Though medical research is constantly trying to create new and better ways of detecting cervical cancer, Pap tests are the best technique that we have available today for early detection of cervical cancer.
