MANILA, Philippines – It should come as no surprise that the back hurts several times in a “whole lifetime’s length.” It was anatomically predestined in the time we evolved from crouching, four-legged beasts into upright, walking animals. So, really, it’s not a question of when it’s “oh-my-aching-back” but of how and what kind of back pain it is.

Worrisome back pain. But since back pain is so common, it is most often ignored. For older people, it’s explained away as one of the givens of aging (it’s not) and for active young adults, back pain becomes a grin-and-bear-it affair. What kind of back pain should be cause for concern?

Acute lumbar strain. By far, the most explainable of back pains is acute lumbar strain. A direct cause and effect can be elicited. that is, the patient comes complaining of an aching back after a day of lifting heavy objects, or of stooping to do some house work like gardening or plumbing. sometimes it can be due to carrying a baby day in and day out, especially big ones. back muscles are in spasm; they feel tight and may feel shortened or contracted.

Herniated disk. Intervertebral disks are the shock absorbers of the spine. In youth, they are sturdy, flexible, and elastic. as the spine ages, disks become rigid and dry. now with continued intense pressure, a disk can rupture its center (imagine a chocolate éclair with the creamy filling) and this center called the nucleus pulposus can pinch an adjacent nerve. This is the impingement that causes pain, numbness, or weakness of the extremities. In theory, all disks can displace. However, the most common herniations are found in the lower back and in the neck.

Signs and symptoms. The following are textbook descriptions of a ruptured disk:

* Sharp “electric” pain shooting down from the back to one or both legs immediately after lifting a heavy object or falling on your back or some other back injury.

* back pain, especially after sneezing, coughing, or straining during a bowel movement.

* Numbness or tingling of the arms and hands (cervical or neck disc) or of the legs and feet (lumbar or lower back disk).

* Difficulty in bending or straightening the back because it is very painful.

* as leg pain worsens, the back pain may disappear.

Non-orthopedic causes. there are good reasons for not ignoring back pain of more than two to three weeks. The back is such a large area that pain from non-musculo-skeletal sources makes its presence known in a treacherous kind of way. For example, infections of the kidney and urinary tract, tumors of the uterus, or gut problems can all present as back pain.

Diagnosis. A thorough history and physical examination should be able to re-create back pain in the clinic. A reliable sign is a positive SLR or Straight Leg Raising test in which shooting pain down the leg occurs when the examiner lifts the patient’s leg from a lying position. X-rays of the back may show loss of the normal curve signifying muscle spasm and/or loss of intervertebral space. to properly identify the offending disc, the doctor may request a CT (computed tomography) Scan or an MRI (magnetic resonance imaging).

Treatment. start by resting for one or two days. Lie down on a firm mattress and not on a hard surface. new studies have pointed out that staying in bed for longer periods leads to a loss of muscle tone. This further delays recovery. Pain medications range from over-the-counter drugs such as acetaminophen and NSAIDs (non-steroidal anti-inflammatory drugs) that are popular from marketing overload. Examples of NSAIDs are mefenamic acid, ibuprofen, diclofenac, naproxen, piroxicam, and indomethacin. COX-2 Inhibitors like celecoxib and etoricoxib, in higher doses (400mg and 120 mg, respectively) may also be used. Other drugs for back pain are tricyclic antidepressants and narcotics. These will need the approval and prescription of doctors. Physiotherapy may be helpful. A rehabilitation medicine specialist designs a physical therapy program for a back patient. This program may include cold or heat therapy, hydrotherapy, bracing, exercises, and TENS or transcutaneous electric nerve stimulation. TENS is thought to relieve pain by stimulating the release of endorphins or pain-inhibiting molecules.

Surgery. An operation (usually performed by either a neurosurgeon or an orthopedic surgeon) is suggested for the following reasons: Progressive increase in pain, failure of conservative treatment, loss of bowel or bladder functions; sudden or progressive muscle weakness; loss of sensation in the buttocks and perineal area (saddle anesthesia), and loss of movement. all these signify that the back will not be better unless there is surgery. The operation usually involves removing the disk (diskectomy) and unroofing the spine (laminotomy/laminectomy). sometimes, portions of the back bones are also joined together (fusion) to prevent pain and instability.

Most back pains go away. some don’t. all must be investigated.

Dear Dr. Gott: I have been ill for quite some time, and no one seems able to help me. It’s frustrating that I go to all these specialists and they will give me answers like “I couldn’t find anything, therefore you’re fine and should go to school.” I am 18 now, but when I was 13, I had my gallbladder removed because it was functioning at 13 percent. I have been ill ever since. Also, when I was 5, I was diagnosed with Hashimoto’s thyroiditis. I have a chromic problem of vomiting almost daily and, on bad days, several times a day. as of last spring, I have had a terrible pain that accompanies the illness and is enough to cripple me.

My doctors are at a loss and are now guessing at what it is. one doctor diagnosed me with GERD, another with IBS, and a third told me there was nothing wrong me with. I have been put on all sorts of medications, and nothing works or even helps with the symptoms.

I have recently discovered that even though I do not test positive for celiac, I react to flour in the same way, and if things are too processed, my body will not break them down. I eat healthfully and basically do the “caveman’s diet,” but nothing seems to help and, even though I am vomiting, I gain weight like crazy.

Dear Reader: my first impression is that you may have a condition known as sphincter of Oddi dysfunction. this ring of muscle is responsible for regulating bile and pancreatic flow into the duodenum (a short section of the small intestine attached to the stomach), preventing the contents of the duodenum from backwashing into the pancreaticobiliary tract and sending hepatic bile into the gallbladder.

There are two types of dysfunction. The first, papillary stenosis, occurs when the sphincter becomes narrowed, often due to fibrosis (scarring). The second, sphincter of Oddi dyskinesia, refers to a number of pressure-related abnormalities.

The primary symptom is abdominal pain. Nausea and vomiting may be present. some patients may also experience acute recurrent pancreatitis.

There are two types of treatment: medication and surgery. Dyskinesia may respond to muscle relaxants that work on smooth muscle. Calcium channel blockers and long-acting nitrate drugs have been shown to improve symptoms. despite this, side effects occur in up to one-third of patients, and only about 75 percent of patients will experience improvement, making this a hit-or-miss option. those with papillary stenosis are not likely to respond to medication.

Surgery to remove excess tissue and/or relieve sphinctal pressure is often helpful. this provides long-term benefits for up to 70 percent of patients. a newer treatment with Botox is currently being used, but long-term effects are not fully known at this time.

Request a referral to a gastroenterologist familiar with this disorder. this condition is fairly uncommon, so your best bet at finding a qualified physician is to contact your local teaching hospitals.

Write to Dr. Gott c/o United Media, 200 Madison Ave., 4th fl., new York, NY 10016.

Yes, I have strep, it feels horrible, pain when I swallow, fever (which is going down I think), muscle pains, headaches, and a hoarse voice. I was not able to get a same day appointment with my doctor, and I really want some fast relief from the symptoms, not a cure (Im pretty sure I need antibiotics for that) but some relief. Im drinking a lot of liquids and taking tylenol but its not doing much for the pain, anybody know any kind of method I could use to feel better? because I really feel like crap…

Take something for the pain and fever, maybe a sore throat spray, or throat drops
keep up on your vitamins and drink water or liquids.

gargle with listerine or warm salt water

How do you know it's strep is a culture has not been taken? Gargle several times a day with hot salt water, rest, drink chicken broth and try Advil until you can see the doctor. Strep is a serious condition.

when ever i have a problem with my t hroat i drink hot tea with sugar of course (personal taste) and make ur throat so muuuuucchccccchhhhh better, hope you do

I have had that HORRIBLE disease many times (1 yr…every month for 5 months in a row). I found that (in addition to the above remedies) just getting lots of sleep. may not do much for the pain, but do it also AFTER you get on the meds…it will prevent the above recurring problem that I had. So off to bed with ya (NOT on the computer!).

swallow ice covered in salt, because the salt sticks to the ice. kinda let it melt in the back of your throat, and while you do that, gargle satly water.

Easy you have a HOT cup of tea. let it sooth your throat. Make sure you stay away from cold or hard things that will hurt your throat trust me been there done that!!!

Gargle warm, salt water. Tylenol Sore Throat (I believe it has 1,000 mg of acetimenophen). Cough drops or throat lozenges or just some hard candy. all this really worked for me. Hope you feel better soon.

very salty, very hot water gargle over and over every hour or so. plus gargle with listerine and don't spit it out let it kind of slide down your throat. i don't share this secret very often but i felt generous today. it sunday afternoon and sunny. lol

for a VERY temporary fix, chloroseptic (sp)

there is a kind of sweet in drug stores in name strepsils. it can reduce ur sorethroat

Where Can I buy Quercetin

The natural health researchers at Institute for Vibrant Living have investigated the epidemic of men’s enlarged prostate glands in America - and the natural remedies that are now available to them. Here is their report:

Men’s prostate problems are more common than people like to think, with most men who are over the age of 50 suffering from the symptoms of an enlarged prostate.

Fortunately, an enlarged prostate, known as benign prostatic hyperplasia (BPH), does not necessarily indicate prostate cancer, or the need for prostate cancer treatment.

However, regular checks should be taken to rule out the diagnosis of this killer disease.

Prostate Enlargement Symptoms

An enlarged prostate can cause considerable inconvenient and embarrassing symptoms which can greatly affect the sufferer’s quality of life.

An enlarged prostate is likely to cause:

Incontinence, loss of sleep, and/or a need to urinate several times during the night

Inability to pass urine

Incomplete emptying of the bladder

A weak stream

A strong and sudden urge to urinate

Lower sex drive

How to Fix It

There are drastic procedures and natural curative strategies available for those who find the problem of BPH affecting their quality of life. these include surgery, pharmaceutical medication and natural supplementation.

Although pharmaceutical drugs are common, many men prefer to try a natural answer first – to avoid the unpleasant side effects and interactions with other prescribed drugs which may occur with chemical products – alongside the most commonly reported side effect of chronic constipation.

Ensuring the best prostate health should include regular check-ups as part of a cancer prevention strategy, along with taking one or more of the natural herbal supplements which have been found to improve the symptoms of BPH.
