999 operator rapped over fatal error

THE grieving brother of a York man said he was “shocked” and “angry” after the 999 operator who failed to send him an ambulance avoided being struck off.

Retired hotel manager John Barker, of Acomb, rang 999 in severe pain, but Yorkshire Ambulance Service nurse Alan Thomas Haugh overrode a computer system which would have triggered an immediate response by paramedics.

instead, Mr Haugh, a father of three, told the 65-year-old that his condition did not appear life-threatening, and classed him as a Category C patient rather than Category A.

But Mr Barker was found dead the next day from severe pancreatitis.

A Nursing and Midwifery Council tribunal this week heard it was the latest in a series of blunders.

The hearing yesterday issued a five-year caution but did not strike off Mr Haugh, meaning he can continue practising as a nurse.

Mr Barker’s brother, Richard, called the decision “lenient”. He said: “My initial feeling is one of anger.

“I am shocked that the picture has been painted of a man who has learned from his mistakes and posed no continuing risk to patients and the public.

“It was very clear that he was not fit to practise and he admitted that. I am not sure the decision is adequate, and it has been harrowing to hear that there was a sequence of cases and there was no intervention sooner in getting him out of the control room.”

Had Mr Barker been classed as a Category A patient, an ambulance would have been dispatched.

Mr Barker had outlined his history of heart pain, but Mr Haugh phoned him back and told him to see his doctor and take painkillers.

it was one of four errors between 2005 and 2007 by Mr Haugh, who is in his early 40s and from York, and who now works at Moorlands Care Home in Strensall.

All four saw him overlooking crucial critical symptoms and choosing not to send an ambulance when patients called.

At the tribunal, Mr Haugh, who previously worked for full Sutton Prison and York Hospital, apologised for letting down the patients’ families. He said the role was not suited to his skills.

He had previously been disciplined after ignoring computer system prompts and failing to send ambulances to a caller who he assumed was hungover when he had actually suffered a skull fracture, to a young girl who burst a Persil liquid capsule over her face and to a woman with abdominal problems who he decided had back trouble. Mr Haugh also admitted speaking to a member of Mr Barker’s family in an unprofessional manner when his actions were challenged.

He said he was better suited to “one-to-one” nursing, and said; “I feel I can be a good nurse again.”

Moorlands manager Helen Lewis told the tribunal Mr Haugh was “confident and experienced”, liked and respected by staff, and would be missed if he was struck off. Christie Brown, representing Mr Haugh, said: “He is somebody who has learned from his mistakes and I submit he is still an asset to the profession.”

Mr Barker’s brother said: “Some of the things said about the way YAS was operating at the time were shocking, but nothing excused him as a professional who did not give the right level of care.

“The family has been left for a long time for the case to come to a consequence, and I think if it had happened immediately after the 2005-2007 period, Haugh would have been struck off and it would have been the right decision.”

The tribunal found Mr Haugh’s fitness to practise was impaired due to malpractice.

Panel member Michael Cann said he failed to uphold YAS’s reputation and provide the duty of care expected of him.

On Thanksgiving Day last year, Lee Sheppard ended up in the emergency room suffering from kidney failure. he had been diagnosed many years earlier with polycystic kidney disease, a hereditary condition that had killed his father and grandfather. he had lived with the disease for years with few symptoms until that day.

Doctors had told him his kidneys would only last about a year in 2010. but large, dark cysts caused severe pain for Sheppard and brought concern about cancer. Doctors removed both of his kidneys, which were functioning at less than 20 percent of normal.

Following the surgery, Sheppard began dialysis and his wife began to ponder giving him her kidney.

“It was amazing how many lovely people came out of the woodwork and volunteered their kidneys. It’s not something that’s easy to say ‘thank you’ to when somebody offers you such a great gift,” Pam Sheppard said. “I felt so privileged that I became the best match.”

“How overwhelmingly grateful I am for my wife for making such a sacrifice. we feel so liberated and energized for what the future holds, so, we’re deeply grateful,” Lee Sheppard said.    

“we already enjoyed a rich love together, but this last year — which has been a journey — has reinforced it in such a way that it’s hard to put a finger on it,” Lee Sheppard said. “The transplant center team is very, very profession and does everything it can to make sure nothing goes wrong … but also (shows) real concern, really compassionate.”

Dr. James Stinson recalled the first such donation — 28 years ago — when a sister gave a kidney to her brother.

“It freed him from dialysis and gave him many years of life that he would not otherwise have had,” he said.

Today, the Intermountain Transplant Program performs more than 135 heart, kidney, liver and pancreas transplants each year.

Dr. Diane Alonso, another surgeon with the Intermountain Transplant Team, recognized the courage and generosity of the human spirit.

“It cements that living donation is a vial source of organs not only in this state but in the country, but particularly in this state,” she said. “At our institution, 50 percent of our kidney donations are from live donors.”

Dear Mayo Clinic: My son was diagnosed with Crohn’s disease at age 13, and he’s now 19. He used to be active, but now he’s exhausted. He’s been on numerous medications, but nothing works for him. what can be done? He has been bleeding a lot, and has severe pain when going to the bathroom, up to eight times each day.

Reply: Crohn’s disease can indeed be a very distressing and frustrating illness, but a determined approach to treatment can often result in symptom relief.

A chronic condition, Crohn’s disease causes inflammation of the digestive tract. While inflammation can occur anywhere from one’s mouth to the anus, it has a special predilection for affecting the small intestine. the cause is unknown. Symptoms can be mild or severe enough to disrupt everyday activities.

Crohn’s disease can occur in patients of all ages, but peak incidence is between ages 15 and 30 years. Symptoms can be nonspecific, but often include diarrhea, abdominal pain, reduced appetite and weight loss. Crohn’s disease can’t be cured but it may go into remission for long periods of time, particularly with medical treatment.

For patients like your son, who continue to have problematic symptoms despite aggressive treatment, I would recommend a thorough reassessment by a physician who specializes in this area of medicine. Seek out a board-certified gastroenterologist with expertise in treating Crohn’s disease.

The doctor will also need to consider other conditions that could be driving the symptoms. one example could be an undiagnosed infection. Aggressive treatment of any coexisting conditions could help relieve your son’s symptoms.

There’s a long list of medication options to manage Crohn’s disease. Even if you’ve tried several, there may be other choices and combinations to consider. your son might also think about participating in a clinical trial to evaluate new ways to treat the disease.

Stress can have an adverse affect on various diseases, and stress reduction may play a role in managing symptoms of Crohn’s disease.

I wish there were an easy formula to improve your son’s health. but the treatment of Crohn’s disease can be complicated, and your son will need to work closely with his medical team.

Even when it seems like you’ve tried everything, there are likely to be more options to consider. Did you get pregnant symptoms before or after your period?

i was 10 1/2 weeks before i got any symptoms. they were peeing alot, my belly grew a little and was hard, and the obvious i missed that time of the month.

My symptoms before period were – at 10 days past ovulation i had periody kind of cramps that lasted just a few minutes which were implantation – i said to my husband, if i am not pregnant then i will be amazed as i have felt implantation before….. and also my boobs were slightly tender when taking my bra off at night from implantation too, they got more painful and still hurt now at 17weeks.

after my period was due i just had a few period cramps again, also i had slight lower back ache, you have to be careful with backache as with severe pain it can indicate miscarriage (which it has done on 2 occassions with me)….. i didnt feel pregnant until i started getting morning sickness at 7 weeks, and i went off certain foods at this point.

the first time i found out i was pregnant i was already about 5 weeks pregnant (5 weeks since the first day of my last period) and i didnt have any symptoms until about a week later at 6 weeks and i was extremely nauseated and throwing up, and very achey, sleepy even depressed. that pregnancy was a surprise. the second time was planned, so i found out immediately when my period should have started (4 weeks) and by 5 weeks i was deathly ill with morning sickness again…

Help – I’ve had an attack of acute pancreatitis

“Alcohol” and “Pancreas”.  These two words are not usually associated with one another.  But too often these words become painfully and permanently connected after a person has experienced a piercing case of acute pancreatitis. 

Perhaps you or a loved one was recently admitted to the hospital with the symptoms of acute pancreatitis: nausea, pain, fever, bloating, etc. then you probably have many questions about pancreatitis.  For example, what caused the acute pancreatitis?  Acute pancreatitis has many causes, and one of them is excessive alcohol consumption.  A common second question: what happens after the patient is released from the hospital?  if people continue to drink alcohol after their first attack of pancreatitis they are much more likely to develop chronic pancreatitis. 

A person living with chronic pancreatitis experiences an array of digestive complications, such as ongoing indigestion and severe pain. moreover, chronic pancreatitis ultimately increases the risk of pancreatic cancer.

 Can I still drink alcohol?

In a word: NO.  you are wrong if you think you can continue to drink alcohol because you’ve only had ONE attack of acute pancreatitis.  and it’s also a bad idea to continue drinking because you think that modern medicine will cure your acute or chronic pancreatitis.  there is currently no treatment in Western medicine which is able to stop or reverse the root causes of chronic alcoholic pancreatitis: the death of pancreatic cells and the slowed production of digestive enzymes.  Simply treating alcoholic pancreatitis is complicated, involves numerous steps, and requires a variety of professionals (read about treatments below).

 Research shows that a general increase in alcohol consumption may be the cause of more and more patients suffering from acute pancreatitis.  Consistently over-consuming alcohol for 5-10 years usually precedes the first attack of acute alcoholic pancreatitis.  Acute pancreatitis currently strikes 80,000 to 200,000 in the U.S. each year, but the numbers are increasing every year.  Although not all alcoholics develop pancreatitis, after a pancreatic attack it is very important to stop drinking as alcohol becomes even more toxic for the pancreas.

It has been proven again and again that it is not safe for a patient to drink ANY amount of alcohol after an attack of acute pancreatitis.  Scientific research has shown that after an attack of acute pancreatitis the lifespan of the patient eventually becomes dependent upon their ability to stop drinking alcohol.  Patients that continue drinking alcohol will almost certainly develop chronic pancreatitis due to the progressive destruction of their pancreas.

 How do I stop drinking?

Hopefully this article has already informed you of the dangers of continuing to drink alcohol after an attack of acute pancreatitis.  In a nutshell, it is VERY important for patients to stop drinking alcohol once and for all.  But how?  Some people can give up drinking alcohol with no problem whatsoever.  However, it is incredibly hard for many people to stop drinking completely.  as a matter of fact, many experts feel it is next to impossible to completely overcome alcohol addiction without the assistance of an experienced licensed practitioner – or even a team of licensed specialists.

Another hurdle facing patients and their families is the inability of the sick person to admit that they have an alcohol problem.  sometimes it helps to assess honestly the alcohol consumption in your life.  Try taking the test below, and see if you might have a problem with alcohol.

 What does treatment involve?

The mind and the body need to be treated at the same time for an alcohol cessation treatment to be successful.  In addition, if the patient has just had an attack of acute pancreatitis, the health of the pancreas must be taken into account when planning the treatment. While most of us are tuned into our own stress levels it is easy to overlook the needs of our pets, particularly dogs.

Stress symptoms in dogs can take many different forms and those that are easy to recognize are as follows:

1. Unusual irritability manifesting itself in unexpected growling or biting (particularly upsetting when to the hand that feeds them);

2. Excessive drooling or licking specific parts of their body. Returning time and time again to the same area can indicate a tick or injury;

3. Uncharacteristic moodiness or sadness. If your dog stays a long time in the one place and does not respond to calls or instructions it can indicate severe pain or stress;

4. Frequent vomiting or bowels movements that are irregular and unusual;

5. If your dog keeps their tail between their legs this can be indicative of stress;

6. The easiest symptom of all that suggests a problem is when your dogs turns their back on food they would normally enthusiastically consume;

7. Blueness around the gums can be indicative of heart problems.

If you detect any of these problems and your dog does not respond to attention or normal reinforcement you should consider taking them to your local Vet.

For a while now, I have been lactose intolerant; but i still eat dairy almost daily, and it usually just causes me to have diarrhea. More recently, every time I eat something(especially a large meal) I get terrible stomach pains. also, I used to be able to drink quite a bit of alcohol, and have nothing happen.. but now if I even have one or two drinks, my stomach is in such severe pain, I can't drink. Could this be a stomach ulcer?

I've had 3 bouts of pancreatitis in the past year and have been nauseas and having a ton of pain in my left side 4 the last few days which has just been getting worse. could this b gallstones? Wat r sum other symptoms? How's it treated? does this sound like it or pancreatitis

Pancreatitis usually confirmed by high Amalyse enzyme levels and X-ray or CAT scan. Gallstones confirmed by non-radiative ultrasound and high liver enzyme levels for ALT and AST. If you have severe pain, go to the ER right away, otherwise see urgent care or your regular physician.

Veteran Kerry Fuller: "it would have been far better to be killed in action"

Every week, Kerry Fuller counts out his medication into a plastic pill box.

He fills the compartments with the 20-30 tablets he takes each day for heart problems and severe pain in his muscles and joints.

Formerly a keen hill walker, Kerry now struggles to walk more than a few dozen metres from his home at Dudley in the West Midlands without stopping for breath. Some days, particularly in cold weather, he is forced to stay indoors because of crippling pain.

Twenty years ago this weekend, coalition forces began the air campaign to force Saddam Hussein's Iraqi troops out of Kuwait.

As Operation Desert Storm got under way, Kerry Fuller was a senior aircraftman with the RAF Support Helicopter Force stationed at Al-Jubail in Saudi Arabia.

As part of his pre-war preparations he was given around a dozen vaccinations to protect him in the event that Saddam Hussein's forces used chemical or biological weapons. within a week Kerry was taken to hospital with chronic fatigue.

He returned to active service a few days later but the bouts of ill-health recurred. he was in hospital again with chronic fatigue within a year of returning from the Gulf.

At the age of 40 he suffered a stroke, which has left him with a mild stammer.

Kerry Fuller believes his deployment to the Gulf caused what has come to be known as "Gulf War Syndrome".

"We were used as guinea-pigs, knowingly or unknowingly," he says.

"going to war didn't bother me but I didn't bank on being poisoned by my own side.

"Now, a lot of us feel it would have been much better if we had been killed in action and hadn't come back at all."

Unexplained illnesses

There is little doubt that some veterans developed unexplained illnesses following their service in the Gulf two decades ago.

Reported symptoms range from chronic fatigue, headaches and sleep disturbances to joint pains, irritable bowel, stomach and respiratory disorders and psychological problems.

Twenty years on, though, there is still disagreement over why rates of ill health are twice as high among Gulf War veterans than troops deployed elsewhere.

Campaigners and doctors also continue to disagree over whether Gulf War Syndrome actually exists as a medical condition unique to Operation Desert Storm.

Even estimates of the number of people made ill by their service in the Gulf vary widely. Official figures show that more than 1,500 people in the UK have claimed war pensions due to Gulf War illnesses but campaigners insist the true number of sufferers is many times higher.

In the years since Operation Desert Storm, theories over possible causes of ill health have ranged from vaccinations, depleted uranium used in armour-piercing weapons, organophosphate pesticides, exposure to nerve agents and the effects of inhaling toxic smoke from burning oil wells.

Psychological factors and battlefield stress have also been cited as possible factors.

Professor Simon Wessely is director of the King's Centre for Military Health Research in London and an adviser to the Ministry of Defence. he does not believe Gulf War Syndrome exists as a distinct illness.

Even so, he has no doubt that a significant number of Gulf veterans became ill as a direct result of their military service.

"The evidence is incontrovertible that there is a Gulf War health effect," he says.

"Something to do with the Gulf has affected health and no-one serious has ever disputed that.

"Is there a problem? Yes there is. Is it Gulf War Syndrome or isn't it? I think that's a statistical and technical question that's of minor interest."

The Ministry of Defence echoes Professor Wessely's view.

An MoD spokeswoman told BBC News: "We have long accepted that some veterans of the Gulf conflict are ill and that some of this ill health may be related to their Gulf service.

"The UK and the US have undertaken a substantial amount of research into Gulf veterans' illness. The research has indicated that there is no illness which is specific to Gulf veterans."

But some ex-servicemen regard this position as cowardly.

‘Left behind’

"The MoD is afraid of being accountable," says Shaun Rusling from the National Gulf Veterans and Families Association.

"if they admitted there was such a thing as Gulf War Syndrome they could be open to compensation claims for medical negligence.

"I would like the present government to apologise to the servicemen and give them proper care and pensions."

Hundreds, perhaps thousands, of veterans have suffered poor health since the 1991 war

Sue Freeth, director of welfare at the Royal British Legion, fears that with the passage of time, ex-servicemen who served in the Gulf in 1990 and 1991 are in danger of being forgotten.

"A lot of veterans, because they haven't been able to find treatment, have decided to stop looking because they think nobody cares any more," she says.

"Some of them are getting support from their GPs or their Primary Care Trusts but certainly some veterans we talk to are not getting any support unless they can pay for it privately.

"With the kind of injuries that are coming out of the current campaign in Afghanistan, the Gulf War veteran is very much being left behind and resources – tight as they are – are not now being directed at them."

As veterans remember the 20th anniversary of Operation Desert Storm, it seems increasingly unlikely that sufferers like Kerry Fuller will ever receive a definitive explanation for their ill health.

"I don't think we're ever going to be able to take it any further now," says Professor Simon Wessely.

"Even if you gave me £10m [for research], I wouldn't know what to do with it.

"I think the only thing worth spending money on is trying to help those who are ill."

As men reach middle age, many of them develop symptoms of an enlarged prostate gland. It’s just a natural piece of the aging process for a lot of men. Symptoms can range from minor to severe or serious ones. It’s not common, but sometimes an enlarged prostate gland compresses the urethra so strongly that urine can’t flow at all, which results in severe pain, and possibly an emergency room trip.

Here is some background, and perhaps some help.

Why a man’s prostate gland enlarges isn’t fully understood. When boys start becoming men, and as their testosterone levels start increasing, their prostate glands start enlarging. The glands stabilize and then even shrink a bit, and then later in life they enlarge once again, even as testosterone levels are falling. Men born with no testicles, or who lose them due to accident don’t get an enlarged prostate gland, so there clearly is a testosterone connection.

What doesn’t seem to make sense is that in puberty, the prostate enlarges while testosterone levels are surging, yet when the prostate starts enlarging, and enlarged prostate symptoms start appearing in middle age, men’s testosterone levels are generally falling.

That certainly doesn’t seem logical. Nonetheless, the most common way of treating enlarged prostate symptoms is to administer a drug that blocks the enzyme that converts testosterone to DHT. this doesn’t work for all men, but it does help many.

Another often-prescribed drug, and often prescribed along with the enzyme-blocking drug, is one that relaxes muscles in the neck of the bladder and around the urethra, which can allow the urethra to expand so urine can pass more freely.

Both classes of drug are unfortunately likely to cause side effects in many men.

This is why many men are looking for natural solutions. There are many– dozens, if not even hundreds, of herbal, non-prescription formulas promoted to help with enlarged prostate symptoms.
