Common Characteristics of Sexually Transmitted Diseases

Sexually transmitted diseases (venereal diseases) are infections that are contracted through intimate sexual contact. Once established, sexually transmitted diseases such as gonorrhea and syphilis can spread and cause extensive, serious physical damage. It is important to make a doctor’s appointment immediately if you suspect that you have contracted–or have been exposed to–such an infection. if it is found that you have been infected, you should abstain from sexual contact until your doctor tells you that you are no longer contagious. you should also inform your sexual partners so that they can be examined and treated also, whether or not they have symptoms.

Although gonorrhea and syphilis are the most serious of the sexually transmitted diseases, there are a number of others that cause considerable discomfort or embarrassment. Venereal warts, for example, can be acutely painful. Pubic lice (crab lice) cause intensive itching.

Gonorrhea is a bacterial infection, and an extremely contagious one. An estimated 2.5 million Americans contract this disease annually.

The symptoms of gonorrhea usually appear within one to three weeks after infection. in men, these symptoms include a white to yellow-green penile discharge, burning pain while urinating and deep, aching pain or pressure in the genitals. in women, there may be painful and frequent urination, deep, aching pain in the lower abdomen and, rarely, a vaginal discharge. Pharyngeal gonorrhea (in the mouth and throat) may produce a sore throat; rectal gonorrhea occasionally causes discomfort in the area around the anus and a slight discharge. However, in 10 to 20 percent of men and up to 80 percent of women, there are no perceived symptoms at all.

Untreated, gonorrhea commonly affects the urethra in men, making urination extremely painful and difficult; it may progress to chronic obstruction and infertility. in women, the disease can attack the fallopian tubes and other pelvic organs (pelvic inflammatory disease), causing pain, fever and, very likely, infertility. The risk of infertility increases with each infection: 75 percent of women who have had three infections involving the pelvic organs are infertile. in both sexes, untreated gonorrhea may result in arthritis, or in a generalized bacterial infection affecting the heart and nervous system. if a woman has active gonorrhea at the time of delivery, her baby may develop permanent blindness.

Fortunately, once it is diagnosed, gonorrhea can be treated quickly and effectively with antibiotics.

A diagnosis of gonorrhea can be made by a microscopic examination of the discharge or a cervical smear; or, more reliably, by a culture that takes two days to incubate. The culture should be repeated one week after treatment.

Syphilis is caused by an organism called a spirochete that is spread through sexual contact. About 20,000 cases a year are reported to United States health authorities, but it is highly probable that several thousands more are not reported.

The disease has three stages. First, skin ulcers (chancres), which are usually painless, appear in the genital area. The chancres may erupt anywhere from 10 to 90 days after infection. Men may see them on the penis, but in women they usually form inside the vagina and may easily be missed. these sores heal in a few weeks, leaving little or no scarring. meanwhile, the spirochetes circulate in the bloodstream and, in a few weeks, produce the symptoms of the second stage of the disease: fever, swollen glands and reddish rash. these signs then also disappear, even without treatment, within 10 days to two weeks; the disease becomes latent and, within two years, no longer infectious. the third stage may develop without warning, years later. in this final stage, there is tissue damage in the brain and the nervous system, the heart, liver, bone and skin. in as many as one-third of untreated individuals, this damage may result in death.

If a woman infected with syphilis becomes pregnant, or contracts the disease during pregnancy, there is a high risk that her baby will be stillborn or suffer from severe birth defects.

Syphilis is most accurately diagnosed by a blood test. It can be treated successfully with penicillin or other antibiotics. Periodic tests should be done for two years after treatment to make certain that the disease is cured. As is the case with all sexually transmitted diseases, sexual partners should be informed immediately, and examined and treated.

Venereal warts (condylomata acuminata), cauliflower-like and red, are caused by a virus and are thought to be almost exclusively sexually transmitted. in women, they appear around the vagina and rectum, growing more rapidly where they come in contact with any vaginal discharge and being greatly aggravated by pregnancy. in men, they appear on the penis and rectal area.

A wart-removing compound may be prescribed, although it is not recommended for use during pregnancy since it can be harmful to the fetus. The compound should be used with care and applied only to the surface of the warts. About six hours after application, a sitz bath should be taken to remove any excess. Over-the-counter preparations should never be used; the genital tissues are too sensitive for such products. Occasionally it is necessary to remove venereal warts by electro-cauterization (burning) or by surgery.

Pubic lice (crab lice) are almost always transmitted sexually. The lice, which can be found attached to the pubic hair, are yellow-gray in color, becoming dark after they are engorged with blood.

An infestation of crab lice causes intense itching in the pubic area as a reaction to the parasites’ bites. Cervical cancer usually affects women aged 35-55 years.90% of cervical cancers derived from squamous cells lining the cervix and the remaining 10% comes from the mucus-producing gland cells in the cervical canal leading into the uterus.
CAUSE
Cervical cancer occurs when cervical cells become abnormal and divide uncontrollably. If the cervical cells continue to divide it will form a mass of tissue called a tumor can be benign or malignant. If the tumor is malignant, then the condition is called cervical cancer.
The cause of abnormalities in cervical cells are not known for certain, but there are some risk factors that influence the occurrence of cervical cancer:
1. HPV (human papillomavirus)HPV is the virus that causes genital warts (condylomata akuminata) that is transmitted through sexual intercourse. A very dangerous type is HPV types 16, 18, 45 and 56.2. SmokeTobacco damage the immune system and affect the body’s ability to fight HPV infection in the cervix.3. First sexual intercourse carried out at an early age4. Changing sexual partners5. Husband / partner first sexual intercourse at age under 18 years old, have multiple sexual partners and had been married to women who suffer from cervical cancer6. The use of DES (diethylstilbestrol) in pregnant women to prevent miscarriage (widely used in the year 1940 to 1970)7. Impaired immune system8. The use of birth control pills9. Infection with genital herpes or chronic chlamydia infections10. Economically weak groups (because it is not capable of doing Pap smears on a regular basis)In cervical precancerous conditions
The cells on the surface of the cervix sometimes appear abnormal but not malignant. Scientists believe that some abnormal changes in cervical cells is the first step of a series of slow changes, a few years later, can cause cancer. Therefore, some abnormal changes are precancerous conditions, which can turn into cancer.
It now used a different term for abnormal changes in cells on the surface of the cervix, one of them is squamous intraepithelial lesions (mean lesion tissue disorders, intraepithelial means that abnormal cells are found only in the surface layer).
Changes in these cells can be divided into 2 groups:
1. Low-level lesions: an early change in the size, shape and number of cells that form the surface of the cervix. some low-level lesions disappear by itself. But others grow into larger and more abnormal, forming a high-level lesions.Low-level lesion also called mild dysplasia or cervical intraepithelial neoplasia 1 (NIS 1).Low-level lesions most often found in women aged 25-35 years, but also can occur in all age groups.2. High level lesions: found a large number of precancerous cells that look very different from normal cells. These precancerous changes occur only in cells on the surface of the cervix. For months, even years, these cells will not become malignant and will not infiltrate into the deeper layers of the cervix.High-level lesion also called secondary or dysplasia severe dysplasia, NIS 2 or 3, or carcinoma in situ.High level lesions most often found in women aged 30-40 years. If abnormal cells spread deeper into the cervix or to other tissues or organs, then the condition is called cervical cancer or invasive cervical cancer.Cervical cancer is most often found at the age above 40 years.
SYMPTOMS
Precancerous changes in the cervix usually do not cause symptoms and these changes are not detected except if the woman underwent a pelvic examination and Pap smear.
Symptoms usually only appear when the abnormal cervical cells become malignant and infiltrate into surrounding tissue. At this time there will be the following symptoms:
- Abnormal vaginal bleeding, especially between the 2 periods, after sexual intercourse and after menopause- Menstrual abnormalities (longer and more)- Whitish who settled, with a watery liquid, pink, brown, contains blood or black and foul smelling.
Symptoms of advanced-stage cervical cancer:- Decreased appetite, weight loss, fatigue- Pelvic pain, back or leg- from the pussy out of urine or feces- Broken bones (fractures).
DIAGNOSIS
Diagnosis based on symptoms and results of the following:
1. Pap smearPap smears can detect up to 90% of cervical cancer cases are accurately and at a cost that is not too expensive. as a result the death rate from cervical cancer also decreased by over 50%.
Every woman who has been sexually active or have reached 18 years of age, should undergo regular Pap smear that is 1 times / year.
PROVO — the rate of chlamydia is increasing inUtah County along with the rest of the state.

After a 25 percent increase in cases in Uintah, Duchesne andDaggett counties, which are east of Utah County, the state healthdepartment joined with these counties to educate people on thesexually transmitted infection. but these same patterns, althoughless drastic, are happening here as well.

Lance Madigan, spokesman for the Utah County Health Department, said the numbers this year indicate about 700 or more cases by Dec.31, compared to 623 cases in 2009, 485 in 2008 and 442 in 2007. Allcases must be reported to the health department.

Statistics from the Utah Department of Health show that the rateof chlamydia has been increasing statewide since 2000; it isparallel to, though far behind, the national rates. Utah Countyalso had the lowest rate of chlamydia in the state according to the2008 numbers.

Madigan couldn’t point to one specific factor in the continuingincline, however. part of it is increased awareness and moredoctors doing the testing. part of it is an improved test from afew years ago. part of it is the health department’s policy to askfor an infected person’s sexual partners and then contact thosepeople to let them know they have been exposed to chlamydia and getthem tested. Some of it is not putting a condom on.

“At the same time people apparently aren’t becoming aware ofsafe sex and practices to prevent the spread of disease,” hesaid.

Chlamydia, which can be resolved through treatment, is mostcommonly seen in people between the ages of 18 and 25 years old; 97percent of cases in Utah County are in people between the ages of15 and 34 years old. Chlamydia also is reported twice as often inwomen, said Dr. Joseph Miner, the executive director of the countyhealth department.

The bacterial disease can resolve itself, but a person is acarrier for much longer and is at a greater risk of the infectionturning serious, Miner said. If left untreated, chlamydia canresult in infertility and other serious complications.

He attributed much of the increase to the improved testingsystem the state has been using; it’s a urine test instead of agenital swab, so people are more willing to be tested and theresults are more accurate.

The increase in population in Utah County also plays a role, hesaid. and while some of the numbers are the result of more sexualpartners and unprotected sex, other STD reporting rates are notincreasing, and those would also increase if promiscuity was thesole factor.
