The original Yellow Wiggle is back, but he’ll never be free of the disorder that forced him to stop Wiggling five years ago.

Greg Page, 40, a.k.a. Yellow Wiggle, left the “The Wiggles,” the wildly popular kids’ singing group he helped found 21 years ago, because of symptoms related to orthostatic intolerance, a little-understood condition. in January, though, the Australia-based Wiggles announced Page had rejoined the group, set to tour the United States this summer.

And that was welcome news for Vanderbilt University cardiologist Dr. Satish Raj, a Wiggles aficionado who also happens to specialize in studying and treating the disorder that forced Page to pass his yellow “skivvy,” or t-shirt, on to replacement Sam Moran.

“The Wiggles have not been the same with Sam since Greg left,” says Raj, father of a 7-year-old daughter and 2 ½-year-old son. Raj has been known to sing Wiggles songs to his adult patients at the Vanderbilt Autonomic Dysfunction Center.( Autonomic refers to the autonomic nervous system, which controls automatic functions such as heart and breathing rate.) when he read of Page’s illness, Raj says, he emailed the Wiggles through their website to offer his services, but got a “no, thank you” in reply.

Page, who suffered from fainting spells, chronic fatigue and poor balance and coordination, according to the Wiggles website, was tested for a variety of conditions before doctors figured out he had orthostatic intolerance, “a beautifully vague term,” Raj says, that describes anybody who has trouble standing up.

“What Greg had sounded more like what we call POTS,” or “postural tachycardia syndrome,” a type of orthostatic intolerance, Raj says. Page is unusual for a POTS patient, Raj says, because four out of five sufferers are women, and only about one in five have fainting spells.

When POTS patients stand for as little as five minutes, their heart rate can jump up to 30 beats per minute, Raj says. They often have high levels of norepinephrine, the stress hormone that underlies the “fight-or-flight” response. and most have decreased blood volume due to low levels of two hormones, regulated by the kidneys,that promote salt retention and increase plasma volume.

Patients frequently report that their symptoms began following stressors such as pregnancy or major surgery, Raj says, and Page has said his got worse after he underwent hernia surgery.

Treatments include small doses of beta-blockers and increased salt and water intake to increase blood volume. That’s where a Wiggles song plays a role in Raj’s practice. “Gulp, Gulp, Drink Some Water,” he’ll sing to patients to encourage them to chug the stuff, especially before they get out of bed in the morning.

Page apparently is practicing what he sings. “My health is much better now due to having a greater understanding of the disorder (Orthostatic Intolerance) and how to manage it appropriately. It is not a curable condition and will always be with me,” he wrote on his website in February.

“There’s no doubt about it,” Raj says, “people have good days and bad days.”

On top of a much-reduced quality of life, comparable to that of kidney failure patients on hemodialysis, POTS patients have to deal with disbelieving doctors, Raj says. “Doctors often think that they’re crazy.”

One of his patients is a medical student who talks to undergraduates about her condition. Raj says the woman tells fellow students:  ”The worst part of this is if I didn’t have it myself, I wouldn’t believe me.”

“Nonprescription medicine should be taken with the same care as prescription medicine, especially for those with complicating factors such as pregnancy or chronic long-term medical conditions such as diabetes or asthma,” says Dr. Braga. “And your pharmacist is there to help you make good choices for treating your cold symptoms and can help you with dosage instructions.”

Consider these factors when evaluating whether or not it is time to see the doctor. 1. do you have a fever? 2. have you been treating your symptoms for a week with no improvement or have the symptoms gotten worse? 3. Are you experiencing unusual symptoms such as dizziness, new pain, coughing blood, etc.? 4. do you have complicating factors such as diabetes, or are you pregnant?

If you can answer “yes” to two or more of the above questions, it is probably time to seek medical attention.

The most common form of cancer of the female reproductive tract in the United States is endometrial carcinoma — cancer of the lining of the uterus, or endometrium. Endometrial cancer accounts for over 95 percent of uterine cancers, which themselves represented approximately six percent of all cancers diagnosed among U.S. women in 2010. It is estimated that 46,470 women will be diagnosed with uterine cancer and that 8,120 will die of this disease in 2011. The median age at which women are diagnosed with endometrial cancer is 63.

Traditionally, women with endometrial cancers have been divided into two groups, depending on the form of cancer they have. Type I tumors are estrogen-dependent, meaning their growth is stimulated by estrogen, like some breast cancers. usually women with Type I tumors are younger and have a better prognosis. Type II tumors account for 10-20 percent of all spontaneously occurring endometrial cancers (those without a family history). The prognosis for Type II tumors, unfortunately, tends to be poor.

In this article, we discuss the risk factors, symptoms, diagnosis, and treatment options for endometrial cancer. we also go over syndromes that are associated with a higher risk of endometrial cancer, like Lynch syndrome and Cowden syndrome, which also increase one’s risk of other forms of cancer, including breast, ovarian, thyroid, and intestinal. RISK FACTORS

The risk factors for Type I endometrial cancer include obesity, diabetes, high blood pressure, taking medications like Tamoxifen (a breast cancer medication), estrogen replacement therapy without progesterone, chronic anovulation (lack of periods), never being pregnant, late menopause, and genetics.

Patients with Type II endometrial carcinomas are usually older when they are diagnosed and they typically haven’t had estrogen replacement therapy. The risk factors for Type II tumors are not as well defined as those of Type I endometrial carcinomas. The fact that Type II tumors are less common makes this cancer type more challenging for doctors to study and characterize. several studies have reported that Type II cases are more likely to occur in older, normal weight women, and those who have been pregnant multiple times or African American women.

THE SYMPTOMS OF ENDOMETRIAL CANCER

A case history illustrates the risk factors and the symptoms associated with endometrial cancer:

A 45-year-old female who had been pregnant twice and given birth to one child came to her primary care physician complaining of worsening and excessive bleeding for the last six months. five years earlier she had had her tubes tied in order to prevent further pregnancies. in the last 10 years, the patient had gained weight, her BMI increasing from 29 to 37 (normal is 18.5-24.9), and she developed type II diabetes and high blood pressure.

Upon reviewing her family history, her doctor learns that the patient had family members with cancer: her brother was diagnosed with colon cancer at the age of 47, and her mother was diagnosed with colon cancer at the age of 44. in addition, the patient’s sister and maternal aunt were diagnosed with ovarian and endometrial cancer at the ages of 55 and 38, respectively.

After a thorough evaluation by an Ob/Gyn, the patient was found to have an enlarged uterus, a large fibroid, fluid in the endometrial cavity, and a growth in the lining of the uterus that was about an inch thick. An in-office biopsy revealed that it was indeed endometrial cancer.

One of the most common symptoms of endometrial cancer is abnormal vaginal bleeding, especially bleeding between periods or after menopause. very long, heavy episodes of bleeding, abdominal or pelvic pain, and clear or white vaginal discharge after menopause are also regular symptoms. however, there can be other causes of any or all of these symptoms, so it is important to see your doctor if you experience any of them. As the case study above suggests, there can be a family or genetic predisposition for endometrial cancer if certain syndromes (like Lynch and Cowden) are present. These syndromes increase risk for a variety of cancers.

DIAGNOSING ENDOMETRIAL CANCER

As in the case history above, abnormal bleeding is the first symptom of more than 90 percent of cases of endometrial carcinomas, which luckily makes early diagnosis somewhat more likely. a physical or pelvic exam does not usually indicate much at all, although one’s doctor might feel a slight enlargement of the uterus. An in-office biopsy of the lining of the uterus is often accurate (more than 90 percent sensitive) and detects many cases of endometrial carcinoma as well as precancerous lesions that are often precursors to the disease.

Pursuing the Warning Signs

With bleeding as the most common early warning sign, doctors must rule out all possible causes of bleeding, of which there can be several. Unusual bleeding in women over 35, as well as in a younger women who have a history of chronic missed periods are signs that should immediately prompt a doctor to test for endometrial cancer.

In some instances, women may have a negative biopsy but still have significant symptoms that suggest endometrial cancer. in these cases, doctors will often opt for a procedure called dilation and curettage (D&C), in which the lining of the uterus is scraped and the cells examined more closely. another technique, called hysteroscopy, in which the uterus is evaluated with an endoscope (a camera on the end of a thin tube, inserted into the uterus through the vagina), may also be recommended.

A transvaginal ultrasound is another common technique, as it is sensitive and noninvasive. It can help evaluate postmenopausal patients with vaginal bleeding, and determine whether the bleeding is worrisome or not. Postmenopausal women who have a minor thickening of the endometrium, as visualized with ultrasound, generally have a low risk of endometrial disease. however, even in this group, if bleeding is recurrent or persistent, endometrial sampling must be carried out to rule out endometrial cancer or its precursor lesions.

After one is diagnosed with endometrial cancer, other lab tests may be done to look at blood counts and liver and kidney function. X-rays may also be taken to determine how advanced the disease is. Chest x-rays are often done to determine whether the cancer has spread to the lungs, since they are a common site of the metastasis of endometrial cancer.

Finally, a specific blood test may be done to measure levels of a glycoprotein called CA-125, since it can be elevated in the presence of endometrial cancer, as well as certain cervical and lung cancers. if it is elevated in endometrial cancer patients, it could mean that there is disease outside the uterus, and knowing this can help determine the best course of treatment.

Screening in High-Risk Women

Some doctors may choose to screen women who are at especially high risk. Women at risk include those who have a genetic predisposition, postmenopausal women who have been treated with estrogen replacement therapy without progesterone, premenopausal women with chronic missed periods that are untreated, and patients with estrogen-producing tumors. taking the breast cancer medication Tamoxifen does not mean that a woman should automatically have endometrial “surveillance” with ultrasound or biopsy if they do not have other symptoms that could indicate endometrial cancer.

TREATMENT OPTIONS FOR ENDOMETRIAL CANCER

Endometrial cancer can be treated in a number of ways, including surgery, radiation therapy, hormone therapy, and chemotherapy. Depending on the type of cancer cell and the stage (I, II, III, or IV), these techniques can either be used alone or in combination with one another.

Surgery to Determine the Stage of Cancer

Patients who are healthy enough to do so typically have exploratory surgery so that one’s doctor can determine how advanced the cancer is (that is, what stage it is), and begin to treat it. The doctor will also determine whether the patient is at low, intermediate, or high risk for cancer recurrence, based on various factors, like the stage of the cancer, the woman’s age, and several variables having to do with the cancer cells themselves and whether or not they have spread. if the woman is determined to be at higher risk, chemotherapy and/or radiation may be required.

WOLFEBORO — Total knee arthroplasty (TKA), or a total knee replacement has become a commonly practiced surgical procedure. The procedure of “replacing” one’s knee involves the removal of the knee joint and replacing it with a mechanical component. People often venture down the path of surgical contemplation when their knee symptoms have become so severe, that day-to-day function becomes drastically limited. If one is indeed a candidate for the surgery, physical therapy (PT) has the potential to offer a host of benefits for the patient and their new knee.

Odds are that you know someone who has had their knee replaced or have first-hand experience with this surgical procedure. according to the National Center for Health Statistics, over 500,000 total knee arthroplasties were done last year in the United States. At this frequency rate, orthopaedic surgeons literally have the procedure down to a science.

For all intents and purposes, the knee replacement is just as it sounds — a replacement of the knee joint. more specifically, parts of the two main weight bearing bones which make up the knee joint are removed and replaced with a metal component, which is often titanium. These bones are the lower end of the femur bone (the long thigh bone) and upper end of the tibia bone (shin bone). These two bones articulate with one another and allow for normal function of the knee. this function can become compromised however, when degenerative conditions such as osteoarthritis (OA), wear away the cartilage linings of the joint resulting in pain and reduced mobility.

After you have had a knee replacement, there is some work that goes into the recovery to allow for a more productive return to function. Metaphorically speaking, it is not quite as simple as driving off in your car after you have had your brakes replaced. After going through the procedure, there is a loss of strength and range of motion (ROM) that is common after a knee replacement. The soreness from the surgery and the post-operative swelling can limit the ability of your muscles to contract and the ability to bend and straighten the knee is often compromised. this can make such routine tasks as transferring in/out of a chair and walking quite daunting tasks. Rehab can help bridge the gap between such a status and a return to mobility. to address strength deficits, physical therapists will use an array of therapeutic exercises to help regain strength and rebuild muscle. Neuromuscular stimulation devices are also used to facilitate muscle contraction with the strengthening process.

Long term studies have demonstrated that a return of strength in the leg muscles following a TKA is one of the biggest determinants of being able to function appropriately. more specifically, strengthening of the quadriceps muscles is key to allow for the execution of every day activities and hobbies. The quadriceps are the muscle group on the front of the thigh that help to straighten the knee and flex the hip. One need not be an Olympian weight lifter to reap the benefits of strengthening. Both high and low level resistance training appear to be equally as effective at promoting strength and allow for better function.

To address ROM limitations, physical therapists pull from a bag of exercises and manual techniques specifically designed to improve ROM by addressing muscle tightness and joint restrictions. Improved knee ROM is crucial following a TKA. Tasks such as negotiating stairs, getting in/out of the car, and squatting to retrieve items from the floor all benefit from progressed ROM after surgery. Exercises targeted to improve the bend of the knee (flexion) and the straightening of the knee (extension) are often conducted. according to research, obtaining knee flexion above 120 degrees (although the more the better) tends to lead to the most optimal outcomes. as far as extension, as close to straight as possible (0 degrees extension) is equally as desirable for optimal function with walking and general function.

Other aspects of rehab involve showing someone how to move their new knee. while it may seem an inherently easy task to move a new knee, it takes some practice and re-education of the muscles to move someone from point a to B. Learning how to move the lower extremity through the different phases of the walking or gait cycle is something that is reinforced. Research has also shown that less subtle movements, such as getting out of a chair, are accomplished with an altered movement pattern after surgery. for example, patients tend to lean forward more and use more of their buttock muscles versus their thigh muscles to complete the motion. Rehab can be very successful at retraining these altered movement patterns to get out of the chair and move in a more “normal’ fashion.

While replacing the knee may not be as simple as replacing the brakes on your car, for many, the procedure is a worthwhile venture to reduce pain and allow people to more fully participate the various activities that make up life. from improving strength, to managing symptoms, to developing an exercise program, the rehab is indeed something that is quite “kneedy.” It has the potential to not only make for a more full return to function, but also to build and reinforce practices that will last for the life of the new knee.

Who gets gonorrhea?
Although any sexually active person can get gonorrhea, it is most prevalent among those from 15 to 30. Women who have vaginal intercourse with an infected man have a 60 to 90 percent chance of becoming infected; while men who have vaginal sex with and infected women have a lower 30 to 50 percent chance of becoming infected by this sexually transmitted disease.

Cefixime 400 mg orally in a single dose,
Ceftriaxone 125 mg IM in a single dose,
Ciprofloxacin 500 mg orally in a single dose,
Ofloxacin 400 mg orally in a single dose, PLUS Azithromycin 1 g orally in a single dose,
Doxycycline 100 mg orally twice a day for 7 days
Patients who are unable to tolerate these treatments can be offered alternative pharmaceutical treatments.

How to prevent gonorrhea and other sexually transmitted diseases…
