<p>WASHINGTON &#8211; &nbsp;A 24-year-old American soldier died &#111;&#102; rabies &#097;&#102;&#116;&#101;&#114; being bitten by a dog last year in Afghanistan, US health officials &#115;&#097;&#105;&#100; Thursday &#102;&#111;&#108;&#108;&#111;&#119;&#105;&#110;&#103; &#097;&#110; investigation &#105;&#110;&#116;&#111; &#116;&#104;&#101; rare case.</p>
<p>The otherwise healthy soldier &#115;&#116;&#097;&#114;&#116;&#101;&#100; experiencing symptoms &#111;&#102; shoulder &#097;&#110;&#100; neck pain &#097;&#110;&#100; tingling sensations in &#104;&#105;&#115; hands soon &#097;&#102;&#116;&#101;&#114; arriving &#097;&#116; Fort Drum, N.Y., in mid-August 2011.</p>
<p>His condition escalated to include nausea, vomiting, anxiety &#097;&#110;&#100; trouble swallowing. By &#116;&#104;&#101; time he was admitted to &#097;&#110; emergency room, he was dehydrated &#097;&#110;&#100; hydrophobic, meaning he developed &#097;&#110; intense &#102;&#101;&#097;&#114; &#111;&#102; drinking liquids because &#111;&#102; &#116;&#104;&#101; painful muscle spasms he experienced while swallowing.</p>
<p>&#8220;He was lucid &#097;&#110;&#100; &#100;&#101;&#115;&#099;&#114;&#105;&#098;&#101;&#100; &#104;&#097;&#118;&#105;&#110;&#103; received a dog bite on &#116;&#104;&#101; &#114;&#105;&#103;&#104;&#116; hand during January 2011 while deployed to Afghanistan,&#8221; &#115;&#097;&#105;&#100; &#116;&#104;&#101; report by &#116;&#104;&#101; US Centers &#102;&#111;&#114; Disease Control &#097;&#110;&#100; Prevention (CDC).</p>
<p>The soldier tested positive &#102;&#111;&#114; a strain &#111;&#102; rabies associated with dogs in Afghanistan &#097;&#110;&#100; doctors attempted &#097;&#110; experimental treatment to save &#104;&#105;&#109; &#102;&#114;&#111;&#109; &#099;&#101;&#114;&#116;&#097;&#105;&#110; death.</p>
<p>But &#119;&#105;&#116;&#104;&#105;&#110; days, &#116;&#104;&#101; patient was suffering &#102;&#114;&#111;&#109; severe brain hemorrhaging &#097;&#110;&#100; &#104;&#105;&#115; family &#100;&#101;&#099;&#105;&#100;&#101;&#100; to &#116;&#097;&#107;&#101; &#104;&#105;&#109; &#111;&#102;&#102; life support. He was &#116;&#104;&#101; &#102;&#105;&#114;&#115;&#116; US service member to die &#111;&#102; rabies &#115;&#105;&#110;&#099;&#101; 1974.</p>
<p>At &#116;&#104;&#101; time &#111;&#102; &#116;&#104;&#101; bite, &#116;&#104;&#101; soldier told family members &#097;&#110;&#100; close friends that he &#104;&#097;&#100; been bitten by a feral dog in Afghanistan &#097;&#110;&#100; &#104;&#097;&#100; sought medical treatment, &#8220;which he &#100;&#101;&#115;&#099;&#114;&#105;&#098;&#101;&#100; as wound cleansing &#097;&#110;&#100; injections,&#8221; &#116;&#104;&#101; report &#115;&#097;&#105;&#100;.</p>
<p>But &#097;&#110; investigation by &#116;&#104;&#101; US Army turned up no documentation &#111;&#102; a reported bite wound or treatment, &#110;&#111;&#114; any record &#111;&#102; a dog tested &#102;&#111;&#114; rabies, &#097;&#099;&#099;&#111;&#114;&#100;&#105;&#110;&#103; to &#116;&#104;&#101; report.</p>
<p>The soldier&#8217;s case prompted a scare that he may &#104;&#097;&#118;&#101; spread &#116;&#104;&#101; disease &#111;&#116;&#104;&#101;&#114; people &#097;&#110;&#100; a CDC investigation tracked down &#097;&#098;&#111;&#117;&#116; 190 individuals he interacted with while potentially infectious.</p>
<p>At least 22 &#111;&#102; them received treatment as a preventive measure.</p>
<p>Rabies &#099;&#097;&#110; spread &#116;&#104;&#114;&#111;&#117;&#103;&#104; saliva, tears, spinal fluid or brain tissue that enters &#116;&#104;&#101; body &#116;&#104;&#114;&#111;&#117;&#103;&#104; a bite a broken skin. With &#116;&#104;&#101; exception &#111;&#102; transplantation, &#116;&#104;&#101; CDC &#115;&#097;&#105;&#100; &#8220;human-to-human rabies transmission &#104;&#097;&#115; &#110;&#111;&#116; been laboratory-documented.&#8221;</p></p>

<p>Ask &#097;&#110;&#121;&#111;&#110;&#101; with a black car if tree pollens &#097;&#114;&#101; &#111;&#117;&#116; &#116;&#104;&#105;&#115; time of year, as you can &#097;&#099;&#116;&#117;&#097;&#108;&#108;&#121; see the pollen &#111;&#110; the surface of a clean car. Later &#105;&#110; the season grass pollens &#102;&#105;&#108;&#108; the air &#102;&#111;&#108;&#108;&#111;&#119;&#101;&#100; &#098;&#121; the weeds late &#105;&#110; the summer. Some dogs can be itchy &#119;&#105;&#116;&#104;&#111;&#117;&#116; &#097;&#110;&#121; skin lesions. &#111;&#116;&#104;&#101;&#114;&#115; scratch &#116;&#104;&#101;&#109;&#115;&#101;&#108;&#118;&#101;&#115; &#097;&#110;&#100; &#099;&#114;&#101;&#097;&#116;&#101; open bleeding sores.</p>
<p>Owners &#109;&#097;&#121; learn &#116;&#111; avoid exposure &#116;&#111; the outdoors with allergic pets. Keeping pets &#097;&#119;&#097;&#121; &#102;&#114;&#111;&#109; fresh cut grass &#111;&#114; high weeds can help prevent reactions. Even a simple bath can remove allergens &#097;&#110;&#100; reduce itching. Some of &#116;&#104;&#101;&#115;&#101; dogs do not &#110;&#101;&#101;&#100; treatment, but some do better with a prescription &#102;&#111;&#114; antihistamines.</p>
<p>Moderately affected dogs &#109;&#097;&#121; require antihistamines &#097;&#110;&#100; corticosteroids. Veterinarians must &#117;&#115;&#101; corticosteroids carefully &#100;&#117;&#101; &#116;&#111; the side effects of increasing water consumption &#097;&#110;&#100; urine output. &#109;&#097;&#110;&#121; a dog has &#104;&#097;&#100; urine accidents &#105;&#110; the home &#102;&#114;&#111;&#109; &#116;&#111;&#111; &#109;&#117;&#099;&#104; medication. Owners &#097;&#114;&#101; always cautioned &#116;&#111; &#117;&#115;&#101; as little corticosteroid medication as &#112;&#111;&#115;&#115;&#105;&#098;&#108;&#101; &#116;&#111; provide relief &#102;&#114;&#111;&#109; allergy symptoms. &#097;&#102;&#116;&#101;&#114; controlling atopy &#119;&#101; taper the dose of corticosteroids &#116;&#111; &#097;&#110; &#101;&#118;&#101;&#114;&#121; &#111;&#116;&#104;&#101;&#114; day schedule when &#112;&#111;&#115;&#115;&#105;&#098;&#108;&#101; &#116;&#111; not further suppress the adrenal gland. Some of the severe atopy &#105;&#110; dogs requires allergy testing &#097;&#110;&#100; hypo sensitization. &#116;&#104;&#105;&#115; process is similar &#116;&#111; allergy injections &#105;&#110; people with a slow increase &#105;&#110; exposure &#116;&#111; antigens. The process can be difficult &#097;&#110;&#100; not always successful &#097;&#102;&#116;&#101;&#114; months &#111;&#114; even a year of injections. The benefit is, if injections work, &#119;&#101; can decrease the &#117;&#115;&#101; of &#111;&#116;&#104;&#101;&#114; medication.</p>
<p>Recently &#119;&#101; &#104;&#097;&#118;&#101; been &#117;&#115;&#105;&#110;&#103; cyclosporine as &#097;&#110; immune modulator &#116;&#111; decrease the reaction &#116;&#111; allergens. &#097;&#108;&#116;&#104;&#111;&#117;&#103;&#104; expensive &#097;&#110;&#100; with some potential side effects, cyclosporine can be useful &#116;&#111; decrease the chronic &#117;&#115;&#101; of corticosteroids. &#116;&#104;&#105;&#115; &#110;&#101;&#119; treatment &#109;&#097;&#121; be safer &#105;&#110; the long run &#116;&#111; help animals be &#109;&#111;&#114;&#101; comfortable.</p>
<p>In pets &#116;&#104;&#097;&#116; &#097;&#114;&#101; itchy all year &#119;&#101; sometimes suspect foods as &#097;&#110; additional factor. <p>“It’s not just spring flowers &#116;&#104;&#097;&#116; &#099;&#097;&#109;&#101; out early &#116;&#104;&#105;&#115; year due &#116;&#111; &#111;&#117;&#114; unseasonably mild winter, ticks &#104;&#097;&#118;&#101; &#097;&#108;&#115;&#111; become active &#105;&#110; recent weeks,” MHP Public Health Coordinator Patty Malloy, RN, said. “It’s important &#116;&#111; protect &#121;&#111;&#117;&#114;&#115;&#101;&#108;&#102; because ticks can carry &#097;&#110;&#100; spread many viruses.”</p>
<p>The Iowa Department &#111;&#102; Public Health (IDPH) said the &#098;&#101;&#115;&#116; way &#116;&#111; prevent tick bites is &#116;&#111; avoid wooded &#097;&#110;&#100; grassy areas, where ticks are &#117;&#115;&#117;&#097;&#108;&#108;&#121; &#102;&#111;&#117;&#110;&#100;. If you do spend time &#105;&#110; these areas, &#116;&#104;&#101;&#121; recommend:</p>
<p>· Wearing long-sleeved shirts &#097;&#110;&#100; long, light-colored pants tucked into your socks or boots.</p>
<p>· Staying &#111;&#110; trails &#119;&#104;&#101;&#110; walking or hiking &#097;&#110;&#100; avoiding tall grass.</p>
<p>· &#117;&#115;&#105;&#110;&#103; insect repellents &#116;&#104;&#097;&#116; &#099;&#111;&#110;&#116;&#097;&#105;&#110; DEET (closely follow the label instructions, not recommended &#102;&#111;&#114; children under 2 months &#111;&#102; age).</p>

<p>Glidden isn&#8217;t &#116;&#104;&#101; only &#111;&#110;&#101;. Fed &#117;&#112; with &#097; company &#116;&#104;&#101;&#121; &#115;&#097;&#121; is bullying them, &#115;&#111;&#109;&#101; SmartMeter critics &#104;&#097;&#118;&#101; had &#116;&#104;&#101; devices removed, and &#115;&#101;&#101;&#110; &#116;&#104;&#101;&#105;&#114; power cut as &#097; result. &#097;&#099;&#099;&#111;&#114;&#100;&#105;&#110;&#103; to PG&amp;E, fewer than 20 people &#104;&#097;&#118;&#101; disconnected &#116;&#104;&#101;&#105;&#114; SmartMeters.</p>
<p>PG&amp;E maintains that disconnecting SmartMeters is &#100;&#097;&#110;&#103;&#101;&#114;&#111;&#117;&#115;, and &#116;&#104;&#101; company doesn&#8217;t &#119;&#097;&#110;&#116; &#116;&#104;&#101; idea to spread. Hence &#116;&#104;&#101; decision to cut off power to people &#119;&#104;&#111; &#100;&#111; &#115;&#111;.</p>
<p>&#8220;If this &#100;&#097;&#110;&#103;&#101;&#114;&#111;&#117;&#115; practice is allowed to continue &#119;&#105;&#116;&#104;&#111;&#117;&#116; &#115;&#111;&#109;&#101; sort of consequence, &#111;&#116;&#104;&#101;&#114; people could &#100;&#111; &#116;&#104;&#101; same &#116;&#104;&#105;&#110;&#103;, and that&#8217;s what we&#8217;re &#116;&#114;&#121;&#105;&#110;&#103; to deter,&#8221; said company spokesman Jeff Smith.</p>
<p>Suspicious of meter

<p>While opposition &#097;&#116; &#102;&#105;&#114;&#115;&#116; focused on &#116;&#104;&#101; meters&#8217; accuracy, &#105;&#116; has now turned &#105;&#110;&#116;&#111; &#097; battle in &#116;&#104;&#101; &#108;&#097;&#114;&#103;&#101;&#114; fight &#111;&#118;&#101;&#114; wireless technology and its possible effects on human health. </p>
<p>People convinced that wireless signals &#099;&#097;&#110; &#099;&#097;&#117;&#115;&#101; debilitating headaches, insomnia, ringing in &#116;&#104;&#101; ears and &#111;&#116;&#104;&#101;&#114; symptoms &#104;&#097;&#118;&#101; repeatedly asked state regulators to halt &#116;&#104;&#101; SmartMeter program.</p>
<p> &#116;&#104;&#101; notion that wireless technology &#099;&#097;&#110; &#104;&#097;&#118;&#101; &#115;&#117;&#099;&#104; effects remains in deep dispute &#097;&#109;&#111;&#110;&#103; researchers.</p>
<p>&#8216;Opt-out&#8217; alternatives
<p>The California Public Utilities Commission in January &#119;&#105;&#108;&#108; consider two &#8220;opt-out&#8221; alternatives for SmartMeter opponents.</p>
<p> &#098;&#111;&#116;&#104; would &#103;&#105;&#118;&#101; PG&amp;E customers &#116;&#104;&#101; choice of receiving &#097; new digital meter that does &#110;&#111;&#116; transmit its data to &#116;&#104;&#101; utility &#118;&#105;&#097; wireless communication. </p>
<p>Both would &#097;&#108;&#115;&#111; force customers to pay extra for that privilege, an idea that opponents liken to extortion. &#110;&#101;&#105;&#116;&#104;&#101;&#114; alternative would let customers keep &#116;&#104;&#101;&#105;&#114; old analog meters.</p>
<p>The people removing &#116;&#104;&#101;&#105;&#114; SmartMeters &#115;&#097;&#121; they&#8217;re frustrated with PG&amp;E and &#116;&#104;&#101; utilities commission alike. </p>
<p>&#8220;I called &#116;&#104;&#101; CPUC, and &#105;&#116; &#115;&#101;&#101;&#109;&#115; &#108;&#105;&#107;&#101; they&#8217;re in bed with PG&amp;E,&#8221; said Bianca Carn of Santa Cruz. She paid an electrician to remove &#104;&#101;&#114; home&#8217;s new SmartMeter &#097;&#102;&#116;&#101;&#114; &#104;&#101;&#114; children &#115;&#116;&#097;&#114;&#116;&#101;&#100; suffering headaches and nosebleeds soon &#097;&#102;&#116;&#101;&#114; its installation.</p>
<p> &#116;&#104;&#101; company cut &#104;&#101;&#114; power on Tuesday, and &#104;&#101;&#114; children &#097;&#114;&#101; now sleeping &#097;&#116; &#116;&#104;&#101;&#105;&#114; grandmother&#8217;s home.</p>
<p>&#8220;They &#107;&#110;&#111;&#119; &#116;&#104;&#101;&#121; &#104;&#097;&#118;&#101; &#116;&#104;&#101; upper hand, &#098;&#101;&#099;&#097;&#117;&#115;&#101; they&#8217;re &#097; monopoly,&#8221; Carn, 45, said of PG&amp;E. &#8220;If people think &#116;&#104;&#101;&#114;&#101; &#097;&#114;&#101; health effects, &#103;&#105;&#118;&#101; them &#097; choice. I don&#8217;t think that&#8217;s &#097;&#115;&#107;&#105;&#110;&#103; too much.&#8221;</p>
<p>The company initially insisted that &#105;&#116; would restore service only &#105;&#102; &#116;&#104;&#111;&#115;&#101; customers agreed to &#104;&#097;&#118;&#101; &#097;&#110;&#111;&#116;&#104;&#101;&#114; SmartMeter installed.</p>
<p> &#098;&#117;&#116; &#097;&#102;&#116;&#101;&#114; news of &#116;&#104;&#101; shutoffs appeared in &#116;&#104;&#101; Santa Cruz Sentinel and on local television stations, &#116;&#104;&#101; company &#100;&#101;&#099;&#105;&#100;&#101;&#100; to offer &#116;&#104;&#101; option of &#097; digital, nontransmitting meter as well. </p>
<p>Several customers &#104;&#097;&#118;&#101; accepted that offer, &#097;&#099;&#099;&#111;&#114;&#100;&#105;&#110;&#103; to &#116;&#104;&#101; company. &#111;&#116;&#104;&#101;&#114;&#115; remain undecided.</p>

<p> <strong>(Medical Xpress) &#8212; Scientists at the Friedrich Miescher Institute for Biomedical Research (FMI) of the Novartis Research Foundation have elucidated the mechanisms underlying the repair of UV-induced &#100;&#097;&#109;&#097;&#103;&#101; &#105;&#110; DNA, which frequently &#099;&#097;&#117;&#115;&#101;&#115; skin cancer. The protein structures additionally determined by &#116;&#104;&#101;&#115;&#101; researchers &#119;&#105;&#108;&#108; improve our understanding of how the body protects itself against skin cancer. &#116;&#104;&#101;&#115;&#101; studies lay the foundations for the development of a &#110;&#101;&#119; class of anti cancer agents. The findings were published today &#105;&#110; <i>Cell</i>.</strong> </p>
<p>Without capturing public attention, the number of &#110;&#101;&#119; cases of skin cancer &#105;&#115; continuously increasing. This type of cancer &#105;&#115; now more common than breast, colon &#097;&#110;&#100; lung cancer combined. &#110;&#111; &#108;&#101;&#115;&#115; discouraging &#105;&#115; the fact &#116;&#104;&#097;&#116; the number of deaths &#102;&#114;&#111;&#109; skin cancer &#105;&#115; &#097;&#108;&#115;&#111; steadily rising &#8211; &#105;&#110; contrast to the trends observed for &#109;&#097;&#110;&#121; other cancers, &#119;&#104;&#101;&#114;&#101; survival rates are improving thanks to effective treatments. But reliable treatments can &#111;&#110;&#108;&#121; be developed if the molecular mechanisms underlying skin cancer are better understood. Here, scientists at the FMI have made an &#105;&#109;&#112;&#111;&#114;&#116;&#097;&#110;&#116; breakthrough, as &#116;&#104;&#101;&#121; report &#105;&#110; the latest issue of <i>Cell</i>.</p>
<p>It &#105;&#115; &#119;&#101;&#108;&#108; &#107;&#110;&#111;&#119;&#110; &#116;&#104;&#097;&#116; skin cancer arises &#102;&#114;&#111;&#109; &#100;&#097;&#109;&#097;&#103;&#101; caused by the sun&#8217;s ultraviolet (UV) radiation. This radiation produces mutations &#105;&#110; DNA which influence cellular processes, possibly leading to cancer. &#097;&#108;&#116;&#104;&#111;&#117;&#103;&#104; the body has various strategies for repairing such &#100;&#097;&#109;&#097;&#103;&#101;, the &#109;&#111;&#115;&#116; frequent type of UV lesion turns &#111;&#117;&#116; to be &#112;&#097;&#114;&#116;&#105;&#099;&#117;&#108;&#097;&#114;&#108;&#121; problematic. &#105;&#110; this case, &#116;&#119;&#111; components of DNA combine to form structures &#107;&#110;&#111;&#119;&#110; as cyclobutane pyrimidine dimers (CPDs). &#116;&#104;&#101;&#115;&#101; are difficult for the cell&#8217;s repair system to detect &#8211; partly &#098;&#101;&#099;&#097;&#117;&#115;&#101; &#116;&#104;&#101;&#121; are inconspicuous, &#103;&#105;&#118;&#101;&#110; the size of the entire genome, but &#097;&#108;&#115;&#111; &#098;&#101;&#099;&#097;&#117;&#115;&#101; &#116;&#104;&#101;&#121; remain hidden &#119;&#105;&#116;&#104;&#105;&#110; tightly coiled DNA.</p>
<p>Group Leader Nicolas Thom&#228; &#097;&#110;&#100; &#104;&#105;&#115; team at the FMI, working together with colleagues &#102;&#114;&#111;&#109; Kobe &#097;&#110;&#100; Osaka, have now identified the molecular machinery which &#097;&#108;&#108;&#111;&#119;&#115; the cell to recognize lesions not detected by the normal repair system.</p>
<p>In &#116;&#104;&#101;&#115;&#101; processes, a key role &#105;&#115; played by a protein complex &#107;&#110;&#111;&#119;&#110; as DDB1-DDB2-Cul4 ubiquitin ligase. A subunit of this complex, DDB2, specifically recognizes CPDs &#101;&#118;&#101;&#110; if &#116;&#104;&#101;&#121; are located &#105;&#110; parts of the DNA which are tightly coiled. The researchers &#097;&#108;&#115;&#111; &#115;&#104;&#111;&#119;&#101;&#100; how another protein &#105;&#110; this complex, ubiquitin ligase, &#105;&#115; activated by the binding of DDB2 to damaged DNA &#097;&#110;&#100; &#116;&#104;&#117;&#115; directly triggers degradation processes &#105;&#110; cells. Thom&#228; comments: &#8220;With this work, for the first time, we&#8217;ve traced the steps &#119;&#104;&#101;&#114;&#101;&#098;&#121; &#116;&#104;&#101;&#115;&#101; common but treacherous genomic UV lesions are recognized &#097;&#110;&#100; the cells are ultimately eliminated. The mechanism works &#108;&#105;&#107;&#101; molecular sun cream, protecting cells &#102;&#114;&#111;&#109; &#100;&#097;&#109;&#097;&#103;&#101; &#097;&#110;&#100; &#116;&#104;&#117;&#115; preventing skin cancer.&#8221;</p>
<p>But the findings are &#097;&#108;&#115;&#111; relevant to diseases caused by defects &#105;&#110; this repair mechanism, such as Cockayne syndrome or xeroderma pigmentosum. &#105;&#110; both of &#116;&#104;&#101;&#115;&#101; conditions, patients are extremely sensitive to UV radiation. &#104;&#111;&#119;&#101;&#118;&#101;&#114;, &#116;&#104;&#101;&#121; &#097;&#108;&#115;&#111; exhibit other symptoms attributable to the accumulation of DNA errors. As Thom&#228; explains, &#8220;The protein structures we&#8217;ve determined are a foundation on which specific inhibitors can be developed. Of &#112;&#097;&#114;&#116;&#105;&#099;&#117;&#108;&#097;&#114; &#105;&#110;&#116;&#101;&#114;&#101;&#115;&#116; here are the ubiquitin ligases, which not &#111;&#110;&#108;&#121; play a role &#105;&#110; rare diseases, but have &#098;&#101;&#101;&#110; implicated &#105;&#110; the development of &#109;&#097;&#110;&#121; types of cancer.&#8221;</p>

<p><strong>NEWS RELEASE</strong>:  DPH LAB IDENTIFIES FIRST CASE OF TULAREMIA SINCE 2003On Oct. 6, &#116;&#104;&#101; Delaware Public Health Laboratory confirmed Delaware’s &#102;&#105;&#114;&#115;&#116; case &#111;&#102; tularemia &#115;&#105;&#110;&#099;&#101; 2003, in a 45 year-old Kent County man. &#116;&#104;&#101; man is hospitalized and responding to treatment. Although Delaware &#104;&#097;&#115; &#110;&#111;&#116; reported a case &#111;&#102; tularemia in eight years, &#101;&#097;&#099;&#104; year &#097;&#110; estimated 125 to 150 cases are reported nationwide.</p>
<p>Tularemia is associated with tick bites and is &#110;&#111;&#116; spread person to person. Domestic cats are very susceptible to tularemia and have &#098;&#101;&#101;&#110; &#107;&#110;&#111;&#119;&#110; to transmit &#116;&#104;&#101; bacteria to humans. Humans can &#098;&#101;&#099;&#111;&#109;&#101; infected &#098;&#121; handling infected animal tissue when hunting &#111;&#114; skinning infected rabbits, muskrats and other rodents; &#098;&#121; inhaling dust &#111;&#114; aerosols contaminated with &#116;&#104;&#101; bacteria, such as during farming &#111;&#114; landscaping activities, &#101;&#115;&#112;&#101;&#099;&#105;&#097;&#108;&#108;&#121; when tractors &#111;&#114; mowers &#114;&#117;&#110; over &#097;&#110; infected animal &#111;&#114; carcass. &#116;&#104;&#101; disease can &#097;&#108;&#115;&#111; be transmitted &#098;&#121; drinking untreated water contaminated with &#116;&#104;&#101; bacteria introduced &#098;&#121; animal contact.</p>
<p>Symptoms &#111;&#102; tularemia &#117;&#115;&#117;&#097;&#108;&#108;&#121; &#097;&#112;&#112;&#101;&#097;&#114; &#116;&#104;&#114;&#101;&#101; to &#102;&#105;&#118;&#101; days &#097;&#102;&#116;&#101;&#114; exposure to &#116;&#104;&#101; bacteria, but can &#116;&#097;&#107;&#101; as long as 14 days. Symptoms may include: sudden fever, chills, headaches, diarrhea, muscle aches, joint pain, dry cough and progressive weakness. People can &#097;&#108;&#115;&#111; develop pneumonia with chest pain, cough and difficulty breathing. Other symptoms &#111;&#102; tularemia depend &#117;&#112;&#111;&#110; &#104;&#111;&#119; &#116;&#104;&#101; person was exposed. These symptoms can include ulcers &#111;&#110; &#116;&#104;&#101; skin &#111;&#114; in &#116;&#104;&#101; mouth, swollen and painful lymph glands, swollen and painful eyes and a sore throat.</p>
<p>Anyone exposed to tularemia &#115;&#104;&#111;&#117;&#108;&#100; be treated as &#115;&#111;&#111;&#110; as possible &#115;&#105;&#110;&#099;&#101; &#116;&#104;&#101; disease can be fatal. Because &#116;&#104;&#101; disease is difficult to diagnose, &#105;&#116; is important to share with &#121;&#111;&#117;&#114; health care provider &#097;&#110;&#121; likely exposures, such as tick and deer fly bites, &#111;&#114; contact with sick &#111;&#114; dead animals. Blood tests and cultures can help confirm &#116;&#104;&#101; diagnosis. Delaware laboratories and healthcare providers are required to report &#097;&#110;&#121; diagnosed case &#111;&#102; tularemia to &#116;&#104;&#101; Division &#111;&#102; Public Health’s Bureau &#111;&#102; Epidemiology.“Preventing tick bites is &#116;&#104;&#101; best &#119;&#097;&#121; to protect &#121;&#111;&#117;&#114;&#115;&#101;&#108;&#102; from tularemia,” &#115;&#097;&#105;&#100; Dr. Karyl Rattay, DPH director. “Use insect repellent &#099;&#111;&#110;&#116;&#097;&#105;&#110;&#105;&#110;&#103; DEET &#111;&#110; &#121;&#111;&#117;&#114; skin, &#111;&#114; treat clothing with repellent &#099;&#111;&#110;&#116;&#097;&#105;&#110;&#105;&#110;&#103; permethrin, to prevent insect bites.” In addition, use care and wear gloves when handling sick &#111;&#114; dead animals. <p>Pressure ulcers &#8212; better &#107;&#110;&#111;&#119;&#110; &#097;&#115; bedsores &#8212; are lesions &#111;&#110; the skin that are caused by unrelieved pressure resulting &#105;&#110; tissue &#100;&#097;&#109;&#097;&#103;&#101;. &#116;&#104;&#101;&#121; &#117;&#115;&#117;&#097;&#108;&#108;&#121; develop &#111;&#118;&#101;&#114; bony areas of the body, &#111;&#102;&#116;&#101;&#110; &#105;&#110; the &#108;&#111;&#119;&#101;&#114; limbs (ankles and hips are common), but &#116;&#104;&#101;&#121; &#099;&#097;&#110; occur almost &#097;&#110;&#121;&#119;&#104;&#101;&#114;&#101; (for example, &#105;&#110; the nostrils of patients with feeding tubes, &#105;&#110; the corners of the mouth &#105;&#110; patients with endotracheal tubes, and &#098;&#101;&#116;&#119;&#101;&#101;&#110; fingers &#105;&#110; patients with rheumatoid arthritis).</p>
<p>Pressure ulcers are &#113;&#117;&#105;&#116;&#101; common &#105;&#110; hospitals and other institutional settings. &#105;&#110; acute care hospitals &#116;&#104;&#101;&#121; occur &#105;&#110; about 3-15 percent of patients; (1) (2) (3) (4) &#116;&#104;&#101;&#121; occur &#105;&#110; about &#097; &#116;&#104;&#105;&#114;&#100; of elderly patients who have had hip fractures; (5) and the number rises to &#097;&#115; much &#097;&#115; 50 percent &#105;&#110; critical care patients. (6) &#116;&#101;&#110; to 35 percent of patients admitted to nursing homes have pressure ulcers, though &#116;&#104;&#105;&#115; rate decreases somewhat &#102;&#111;&#114; patients who have been &#116;&#104;&#101;&#114;&#101; longer. <p><b>TYPES OF ULCERS</b></p>
<p>There are several types of skin ulceration. Pressure ulcers or bedsores occur when the skin is subjected to constant pressure, which is why &#116;&#104;&#101;&#121; happen so frequently &#105;&#110; hospitals and &#105;&#110; older patients. &#116;&#104;&#101;&#121; generally start &#097;&#115; &#097; blister, and then &#098;&#101;&#099;&#111;&#109;&#101; an open sore, finally ending &#105;&#110; &#097; &#8220;crater.&#8221; &#105;&#110; addition to pressure ulcers, areas of skin breakdown may be due to other types of ulcers, having to &#100;&#111; with insufficient blood flow or to diabetic neuropathy.</p>
<p>Insufficient blood flow &#116;&#104;&#114;&#111;&#117;&#103;&#104; the veins &#117;&#115;&#117;&#097;&#108;&#108;&#121; occurs &#105;&#110; the &#108;&#111;&#119;&#101;&#114; legs, and &#099;&#097;&#110; result &#105;&#110; venous insufficiency ulcers, which are &#111;&#102;&#116;&#101;&#110; chronic and difficult to heal. &#116;&#104;&#101;&#121; &#099;&#097;&#110; &#099;&#097;&#117;&#115;&#101; pain &#105;&#110; the foot and &#117;&#115;&#117;&#097;&#108;&#108;&#121; &#097;&#112;&#112;&#101;&#097;&#114; purplish &#105;&#110; color. &#116;&#104;&#101;&#121; are &#110;&#101;&#118;&#101;&#114; &#102;&#111;&#117;&#110;&#100; above the level of the knee or &#105;&#110; the forefoot, and may occur singly or &#105;&#110; multiples.</p>
<p>A related condition, caused by insufficient blood flow &#116;&#104;&#114;&#111;&#117;&#103;&#104; the arteries, is &#107;&#110;&#111;&#119;&#110; &#097;&#115; arterial insufficiency ulcers, which are painful lesions that &#117;&#115;&#117;&#097;&#108;&#108;&#121; occur &#111;&#118;&#101;&#114; the ankle or other areas of the foot. &#097;&#108;&#116;&#104;&#111;&#117;&#103;&#104; &#116;&#104;&#101;&#121; may be &#115;&#101;&#101;&#110; near bony prominences (i.e., joints), &#116;&#104;&#101;&#121; are distinguished from pressure ulcers by &#116;&#104;&#101;&#105;&#114; &#8220;punched-out&#8221; or star-like appearance. The wound may be pale and dry, surrounded by red and taut skin, and &#099;&#097;&#110; include an area of dead skin. </p>
<p>Diabetic ulcers occur &#111;&#110; the foot, &#117;&#115;&#117;&#097;&#108;&#108;&#121; &#111;&#118;&#101;&#114; the joints or &#111;&#110; the top of the toes. &#116;&#104;&#101;&#115;&#101; ulcers &#111;&#102;&#116;&#101;&#110; occur &#111;&#110; the ball of the foot &#105;&#110; diabetic patients, due to neuropathy or repetitive injury. Diabetic foot ulcers are &#111;&#102;&#116;&#101;&#110; surrounded by &#097; significant thickening of the skin, and are &#117;&#115;&#117;&#097;&#108;&#108;&#121; insensitive to touch. </p>
<p>There are other, less common &#099;&#097;&#117;&#115;&#101;&#115; of ulcers &#105;&#110; the legs and feet, which include connective tissue diseases (e.g., rheumatoid arthritis), sickle cell disease, and &#099;&#101;&#114;&#116;&#097;&#105;&#110; forms of cancer. One&#8217;s doctor should take special precaution to rule out &#116;&#104;&#101;&#115;&#101; &#109;&#111;&#114;&#101; &#115;&#101;&#114;&#105;&#111;&#117;&#115; conditions before arriving at &#097; diagnosis of an ulcer.</p>
<p><b>CAUSES AND RISK FACTORS OF PRESSURE ULCERS</b></p>
<p><b>Internal and External Risk Factors</b></p>
<p>There are &#098;&#111;&#116;&#104; external and internal risk factors &#102;&#111;&#114; ulcers. Pressure, friction, shearing (two layers of skin sliding &#111;&#110; each other &#105;&#110; &#111;&#112;&#112;&#111;&#115;&#105;&#116;&#101; directions), and moisture (which &#099;&#097;&#110; &#099;&#097;&#117;&#115;&#101; softening of skin) are considered external (or extrinsic) risk factors. Internal or intrinsic factors have to &#100;&#111; with the patient&#8217;s state of health: &#116;&#104;&#101;&#115;&#101; including underlying medical conditions, immobility, inactivity, fecal and urinary incontinence, malnutrition, decreased consciousness, steroid &#117;&#115;&#101;, and smoking, which &#099;&#097;&#110; &#097;&#108;&#108; influence one&#8217;s likelihood of developing an ulcer.</p>
<p>Medical conditions linked to intrinsic risk factors are far-ranging and include anemia, infection, peripheral vascular disease, edema, diabetes, stroke, dementia, delirium, alcoholism, fractures, and malignancies. Simple, age-related factors &#115;&#117;&#099;&#104; &#097;&#115; reduced fat and muscle mass with which to dissipate pressure &#097;&#108;&#115;&#111; increase risk. &#108;&#111;&#119;&#101;&#114; levels of Vitamin C &#097;&#115; &#111;&#110;&#101; ages may &#097;&#108;&#115;&#111; increase the risk of pressure ulcers by causing blood vessels and connective tissue to &#098;&#101;&#099;&#111;&#109;&#101; fragile and reducing the number of blood vessels serving the tissue of the skin, and other factors.</p>
<p><b>The Underlying &#099;&#097;&#117;&#115;&#101; of Ulcers: Tissue Ischemia</b></p>
<p>A main &#099;&#097;&#117;&#115;&#101; of pressure ulcers is tissue ischemia, which occurs when &#116;&#104;&#101;&#114;&#101; is &#097; loss of adequate blood supply to the tissue (skin) &#105;&#110; &#113;&#117;&#101;&#115;&#116;&#105;&#111;&#110;, caused by excessive pressure &#111;&#110; the skin. Pressure to the skin lasting longer &#116;&#104;&#097;&#110; &#116;&#119;&#111; hours produces irreversible changes &#105;&#110; skin tissue. &#105;&#110; patients who tend to develop pressure ulcers post-operatively, &#116;&#104;&#101;&#114;&#101; is impairment &#105;&#110; skin blood flow &#111;&#118;&#101;&#114; bony areas of the skin during surgery. (11)</p>
<p>When &#097; person sits, &#116;&#104;&#101;&#105;&#114; sitting bones bear the greatest pressure, &#111;&#102;&#116;&#101;&#110; pressure &#116;&#104;&#097;&#110; your capillaries &#099;&#097;&#110; technically take. (12) Pressure ulcers &#099;&#097;&#110; actually occur from the &#8220;inside-out&#8221; &#103;&#105;&#118;&#101;&#110; that the muscle is &#109;&#111;&#114;&#101; sensitive to pressure &#116;&#104;&#097;&#110; is the skin. (13) &#098;&#101;&#099;&#097;&#117;&#115;&#101; the outer layer of the skin shows signs of tissue death &#114;&#101;&#108;&#097;&#116;&#105;&#118;&#101;&#108;&#121; late &#105;&#110; the development of an ulcer, (14) &#111;&#110;&#099;&#101; the skin starts to &#115;&#104;&#111;&#119; indications of the presence of an ulcer, the ulcer may be even &#109;&#111;&#114;&#101; progressed &#116;&#104;&#097;&#110; meets the eye.
